CONGENITAL diaphragmatic hernia per se constitutes a threat to life in the newborn. If an operation is not performed, according to available statistics, most of these infants do not live beyond two months. The most serious of these defects is the complete absence of the hemidiaphragm. No report of an operative survival for the correction of this abnormality was found in a review of the American and British literature. It is the purpose of this paper to describe a new operative technique which has been successfully executed in man. Subsequent clinical experience and animal experiments make it appear that this is a reasonable approach to the problem. The technique involves the suture of the respective lobe of the liver to the endothoracic fascia to separate completely the thoracic and abdominal cavities. Congenital diaphragmatic hernia in children under one year of age: A 24 year review

Diaphragmatic hernia is one of the surgical emergencies of childhood because of the life-threatening cardiorespiratory embarrassment frequently associated with it. This congenital condition is characterized by varying degrees of protrusion of the abdominal viscera into the thoracic cavity through an abnormal opening in the diaphragm; this opening results from a defect in the complex embryologic development of the diaphragmatic partition between the thoracic and abdominal cavities. A review of the literature 30 years ago 9 showed a 75% mortality associated with this pathological condition if left untreated; one eminent pathologist stated 15 that "only one [such] infant delivered in our hospital has survived longer than 6 hours." At present, the decreasing rate of loss can be attributed to earlier recognition, immediate surgical intervention, improved anesthesia, and better postoperative care. Die Entstehung des Zwerchfells hangt eng mit der Entstehung der Perikard-, der Pleura- und der Peritonealhohle zusammen. Die drei Hohlen sind Abkommlinge der Colomhohle. Beim Embryo sind die Organe paarig angelegt. Erst spater wird die Perikardhohle und der unterhalb des Nabels liegende Teil der Peritonealhohle in den unpaarigen Zustand ubergefuhrt. In diese Colomhohle taucht von hinten der Verdauungskanal, aufgehangt am Gewolbe, durch das dorsale Meso, welches seine Entstehung Mesoblasten von der Haftstelle der Aorta verdankt. Die Colomhohle wird durch dieCuvierschen Kanale (Vereinigung der Vv. cardinales anterior et posterior) durchzogen. Diese Kanale bringen mesoblastisches Gewebe, welches das Septum transversum bildet, das die grose Colomhohle in eine Perikard- und in eine pleuro-peritoneale Hohle abtrennt. Das Septum transversum, die Scheidewand zwischen dem vorne gelegenen Kopfcolom und dem dahinter liegenden Rumpfcolom, ist ursprunglich vertikal orientiert. Mit dem machtigen Wachstum von Kopf und Neurairohr kommt es zu einer Verschiebung mit Drehung des Septums, so das dessen freies Ende, scheinbar caudal verlagert, horizontal zu liegen kommt. Bei den Amnioten setzt sich das Septum gegen vorne fort und vereinigt sich mit der ventralen Wand und schliest damit die Perikardhohle ab. Dieser Teil ist der Hauptbestandteil des spateren Zwerchfells des Erwachsenen. Aus ihm entstehen Zentrum und ein groser Teil der sternalen und costalen Zwerchfellpartien. Der hintere Teil des Septums ist noch frei, so das Pleura- und Peritonealhohle eine grose gemeinsame Hohle bilden.
