This paper reviews recent advances in biomedical and healthcare applications of Doppler radar that remotely detects heartbeat and respiration of a human subject. In the last decade, new front-end architectures, baseband signal processing methods, and system-level integrations have been proposed by many researchers in this field to improve the detection accuracy and robustness. The advantages of noncontact detection have drawn interests in various applications, such as energy smart home, baby monitor, cardiopulmonary activity assessment, and tumor tracking. While many of the reported systems were bench-top prototypes for concept verification, several portable systems and integrated radar chips have been demonstrated. This paper reviews different architectures, baseband signal processing, and system implementations. Validations of this technology in a clinical environment will also be discussed.

A Review on Recent Advances in Doppler Radar Sensors for Noncontact Healthcare Monitoring

The report builds on a growing realization within the health care community of the critical role information/ communications technologies, systems engineering tools, and related organizational innovations must play in addressing the interrelated quality and productivity crises facing the health care system. The report provides a framework for change and an action plan for a systems approach to health care delivery based on a partnership between engineers, health care professionals, and health care managers. The goal of the plan is to transform the U.S. health care sector from an underperforming conglomerate of independent entities (individual practitioners, small group practices, clinics, hospitals, pharmacies, community health centers, etc.) into a high-performance "system" in which participating units recognize their interdependence and the implications and repercussions of their actions on the system as a whole. The report describes opportunities and challenges to using systems engineering, information technologies, and other tools to advance a twenty-first century system capable of delivering safe, effective, timely, patient-centered, efficient, equitable health care — a system that embodies the six "equality aims" envisioned in Crossing the Quality Chasm.

A noninvasive and unconstrained real-time method to detect the respiration rhythm and pulse rate during sleep is presented. By employing the agrave trous algorithm of the wavelet transformation (WT), the respiration rhythm and pulse rate can be monitored in real-time from a pressure signal acquired with a pressure sensor placed under a pillow. The waveform for respiration rhythm detection is derived from the 26 scale approximation, while that for pulse rate detection is synthesized by combining the 24 and 25 scale details. To minimize the latency in data processing and realize the highest real-time performance, the respiration rhythm and pulse rate are estimated by using waveforms directly derived from the WT approximation and detail components without the reconstruction procedure. This method is evaluated with data collected from 13 healthy subjects. By comparing with detections from finger photoelectric plethysmograms used for pulse rate detection, the sensitivity and positive predictivity were 99.17% and 98.53%, respectively. Similarly, for respiration rhythm, compared with detections from nasal thermistor signals, results were 95.63% and 95.42%, respectively. This study suggests that the proposed method is promising to be used in a respiration rhythm and pulse rate monitor for real-time monitoring of sleep-related diseases during sleep

Real-Time Monitoring of Respiration Rhythm and Pulse Rate During Sleep

Healthcare information, and to some extent patient management, is progressing toward a wireless digital future. This change is driven partly by a desire to improve the current state of medicine using new technologies, partly by supply-and-demand economics, and partly by the utility of wireless devices. Wired technology can be cumbersome for patient monitoring and can restrict the behavior of the monitored patients, introducing bias or artifacts. However, wireless technologies, while mitigating some of these issues, have introduced new problems such as data dropout and “information overload” for the clinical team. This review provides an overview of current wireless technology used for patient monitoring and disease management. We identify some of the major related issues and describe some existing and possible solutions. In particular, we discuss the rapidly evolving fields of telemedicine and mHealth in the context of increasingly resource-constrained healthcare systems.

Wireless Technology in Disease Management and Medicine

A new physiologic monitor for use in the home has been developed and used for the Collaborative Home Infant Monitor Evaluation (CHIME). This monitor measures infant breathing by respiratory inductance plethysmography and transthoracic impedance; infant electrocardiogram, heart rate and R-R interval; haemoglobin O2 saturation of arterial blood at the periphery and sleep position. Monitor signals from a representative sample of 24 subjects from the CHIME database were of sufficient quality to be clinically interpreted 91.7% of the time for the respiratory inductance plethysmograph, 100% for the ECG, 99.7% for the heart rate and 87% for the 16 subjects of the 24 who used the pulse oximeter. The monitor detected breaths with a sensitivity of 96% and a specificity of 65% compared to human scorers. It detected all clinically significant bradycardias but identified an additional 737 events where a human scorer did not detect bradycardia. The monitor was considered to be superior to conventional monitors and, therefore, suitable for the successful conduct of the CHIME study.

Cardiopulmonary monitoring at home: the CHIME monitor.

An electronic simulator of physiologic signals used in infant monitoring has been designed, constructed and applied in the Collaborative Home Infant Monitor Evaluation (CHIME). A unique feature of the simulator is that it contains actual physiologic waveforms recorded from infants rather than artificial, idealized signals. The simulator stores breathing waveforms that can be used to test transthoracic-impedance- and inductance-plethysmography-based monitors, and heart rate channels are tested by playing a neonatal QRS complex at preset fixed rates or a variable rate as determined from infant recordings. The transfer characteristics of the simulator are constant over frequencies ranging from 0.5 to 8 Hz for the respiration channels. Data stored in memory are divided into 60 second epochs that can be presented to the monitor being tested in a programmable sequence. A group of 66 CHIME monitors was tested using a simulator programmed with 17 apnoea and bradycardia waveforms. The agreement between monitors as to the duration of detected apnoea decreases as the amount of artefact in the signal increases. Discrepancies between monitors in detecting apnoea duration were found to be similar to inconsistencies between CHIME investigators manually scoring similar waveforms.
