Peripheral giant cell granuloma. Clinicopathologic study of 224 new cases and review of 956 reported cases

Central giant cell lesions of the mandible and maxilla: A clinicopathologic and cytometric study

A variety of tumor and tumor-like lesions of the mandible and maxilla occurred in children ranging in age from 3 months to 15 years with a mean of 8 years. The sex distribution and involvement of the mandible and maxilla were essentially equal for the entire series, with some differences in the histologic subgroups. Fibro-osseous lesions included fibrous dysplasia, ossifying fibroma, and cementifying fibroma, and together with giant-cell granulomas accounted for 64% of the tumors. The giant-cell granulomas pursued an unpredictable clinical course but they usually became quiescent in early adult life. Only the ossifying fibroma among the fibro-osseous lesions was entirely nonaggressive in its course. Other tumors which tended to recur locally were the ameloblastoma, an aggressive fibroblastic lesion, osteoblastoma, osteoma, and the aneurysmal bone cyst. The only death recorded in this study was a child with an extensive fibrous dysplasia of the mandible which recurred on numerous occasions with death finally from pneumonia.

Tumors of the mandible and maxilla in children. I. Clinicopathologic study of 46 histologically benign lesions.

Ninety-seven peripheral and 32 central oral giant cell granulomas have been studied clinically and histologically. The distribution according to age (below and over 15 years of age) is similar in the two types of granulomas. It applies to both lesions that the prevalence usually is higher in females than in males. Both granulomas are most frequently found in the mandible. The central giant cell granuloma occur anterior to the molar region. The recurrence rate of the lesions in this material is high as compared with that observed in earlier studies. The radiographic features are non-specific. The histopathology of the peripheral and central giant cell granulomas is fundamentally identical, both being composed of 1) a highly vascular and cellular granulation tissue containing, 2) a great number of multi-nuclear giant cells of foreign body type scattered in the tissue, 3) extravasation by red blood cells and the presence of haemosiderin, 4) occasionally bone formation, and 5) several mitoses in the stromal cells. Comparative statistical analysis of the clinical findings and comparative histomorphological evaluation of the present material suggest that both types of lesions are manifestations of the same pathological condition.

Oral giant cell granulomas. A clinical and histological study of 129 new cases.

175 'epulides' were studied clinically and histologically. The lesions were histologically reclassified into three groups: granulomatous, fibromatous and giant cell lesions. The granulomatous group was four times and the giant cell group 1.5 times more frequent in females. It is suggested that the expression epulis should be abandoned as a microscopic designation for lesions in the oral mucosa, as it is only a non-specific topographico-clinical concept comprising histogenetically and histomorphologically differing lesions and therefore does not represent a uniform group. The suggested new terms for the three types of hyperplastic lesions are oral mucosal granulomatous hyperplasias (including such diagnostic terms as granulomatous gingivitis, gingivitis of pregnancy, granuloma gravidarium, pyogenic granuloma, epulis angiomatosa and epulis telangiectaticum), oral mucosal fibrous hyperplasias (including such diagnostic terms as fibrous epulis, epulis fissuratum, fibroepithelial lesions, denture injury tumor and peripheral odontogenic fibroma) and oral mucosal giant cell hyperplasia (including such diagnostic terms as peripheral giant cell granuloma and giant cell epulis).
