TL;DR: A 36-year-old man presented with facial nerve palsy, hearing loss, vertigo and headache, and Cerebrospinal fluid analysis revealed an elevated white blood cell count and positive RPR, confirming the diagnosis of neurosyphilis.
Abstract: A 36-year-old man presented with facial nerve palsy, hearing loss, vertigo and headache. He was initially diagnosed with Ramsay Hunt syndrome and treated with a systemic steroid and valaciclovir; however, his symptoms deteriorated. Serum rapid plasma reagin (RPR) and treponema pallidum hemagglutination tests were positive. Cerebrospinal fluid analysis revealed an elevated white blood cell count and positive RPR, confirming the diagnosis of neurosyphilis. Penicillin G (PCG) was administered, and his facial nerve function and headache improved. However, left-side hearing loss worsened temporarily, which was assumed to be a Jarisch-Herxheimer reaction. Betamethasone was administered along with PCG, and he recovered completely.

TL;DR: Ophthalmologic investigations revealed bacterial endophthalmitis and a systemic assessment with galliumscintigraphy showed multiple infectious foci, and blood cultures were positive for Streptococcus pneumoniae in both patients.
Abstract: Two elderly patients with uncontrolled diabetes (hemoglobin A1c: 7.9% and 14%) presented to our department with complaints of sudden-onset visual loss on the right eye. Ophthalmologic investigations revealed bacterial endophthalmitis and a systemic assessment with galliumscintigraphy showed multiple infectious foci (Picture). In addition, blood cultures were positive for Streptococcus pneumoniae in both patients. The pathogen was detected in the vitreous fluid in Case 1, but not Case 2. The capsular serotype of S. pneumoniae was identified to be 22 and any one
