The visualisation of living tissues at microscopic resolution is attracting attention in several fields. In medicine, the goals are to image healthy and diseased tissue with the aim of providing information previously only available from biopsy samples. In basic biology, the goal may be to image biological models of human disease or to conduct longitudinal studies of small-animal development. High-frequency ultrasonic imaging (ultrasound biomicroscopy) offers unique advantages for these applications. In this paper, the development of ultrasound biomicroscopy is reviewed. Aspects of transducer development, systems design and tissue properties are pre- sented to provide a foundation for medical and biological applications. The majority of applications appear to be developing in the 40 - 60-MHz frequency range, where resolution on the order of 50 mm can be achieved. Doppler processing in this frequency range is beginning to emerge and some examples of current achievements will be highlighted. The current state of the art is reviewed for medical applications in ophthalmology, intravascular ultrasound, dermatology, and cartilage imaging. Ultrasound biomicroscopic studies of mouse embryonic devel- opment and tumour biology are presented. Speculation on the continuing evolution of ultrasound biomicroscopy will be discussed. © 2000 World Federation for Ultrasound in Medicine & Biology.

Advances in ultrasound biomicroscopy

Piezoelectric materials have dominated the ultrasonic transducer technology. Recently, capacitive micromachined ultrasonic transducers (CMUTs) have emerged as an alternative technology offering advantages such as wide bandwidth, ease of fabricating large arrays, and potential for integration with electronics. The aim of this paper is to demonstrate the viability of CMUTs for ultrasound imaging. We present the first pulse-echo phased array B-scan sector images using a 128-element, one-dimensional (1-D) linear CMUT array. We fabricated 64- and 128-element 1-D CMUT arrays with 100% yield and uniform element response across the arrays. These arrays have been operated in immersion with no failure or degradation in performance over the time. For imaging experiments, we built a resolution test phantom roughly mimicking the attenuation properties of soft tissue. We used a PC-based experimental system, including custom-designed electronic circuits to acquire the complete set of 128/spl times/128 RF A-scans from all transmit-receive element combinations. We obtained the pulse-echo frequency response by analyzing the echo signals from wire targets. These echo signals presented an 80% fractional bandwidth around 3 MHz, including the effect of attenuation in the propagating medium. We reconstructed the B-scan images with a sector angle of 90 degrees and an image depth of 210 mm through offline processing by using RF beamforming and synthetic phased array approaches. The measured 6-dB lateral and axial resolutions at 135 mm depth were 0.0144 radians and 0.3 mm, respectively. The electronic noise floor of the image was more than 50 dB below the maximum mainlobe magnitude. We also performed preliminary investigations on the effects of crosstalk among array elements on the image quality. In the near field, some artifacts were observable extending out from the array to a depth of 2 cm. A tail also was observed in the point spread function (PSF) in the axial direction, indicating the existence of crosstalk. The relative amplitude of this tail with respect to the mainlobe was less than -20 dB.

Capacitive micromachined ultrasonic transducers: next-generation arrays for acoustic imaging?

The classification of ultrasonic liver images is studied, making use of the spatial gray-level dependence matrices, the Fourier power spectrum, the gray-level difference statistics, and the Laws texture energy measures. Features of these types are used to classify three sets of ultrasonic liver images-normal liver, hepatoma, and cirrhosis (30 samples each). The Bayes classifier and the Hotelling trace criterion are employed to evaluate the performance of these features. From the viewpoint of speed and accuracy of classification, it is found that these features do not perform well enough. Hence, a new texture feature set (multiresolution fractal features) based on multiple resolution imagery and the fractional Brownian motion model is proposed to detect diffuse liver diseases quickly and accurately. Fractal dimensions estimated at various resolutions of the image are gathered to form the feature vector. Texture information contained in the proposed feature vector is discussed. A real-time implementation of the algorithm produces about 90% correct classification for the three sets of ultrasonic liver images. >

Texture features for classification of ultrasonic liver images

A method for estimating structural properties of random media is described. The size, number density, and scattering strength of particles are estimated from an analysis of the radio frequency (rf) echo signal power spectrum. Simple correlation functions and the accurate scattering theory of Faran [J.J. Faran, J. Acoust. Soc. Am. 23, 405-418 (1951)], which includes the effects of shear waves, were used separately to model backscatter from spherical particles and thereby describe the structures of the medium. These methods were tested using both glass sphere-in-agar and polystyrene sphere-in-agar scattering media. With the appropriate correlation function, it was possible to measure glass sphere diameters with an accuracy of 20%. It was not possible to accurately estimate the size of polystyrene spheres with the simple spherical and Gaussian correlation models examined because of a significant shear wave contribution. Using the Faran scattering theory for spheres, however, the accuracy for estimating diameters was improved to 10% for both glass and polystyrene scattering media. It was possible to estimate the product of the average scattering particle number density and the average scattering strength per particle, but with lower accuracy than the size estimates. The dependence of the measurement accuracy on the inclusion of shear waves, the wavelength of sound, and medium attenuation are considered, and the implications for describing the structure of biological soft tissues are discussed.

Absfract-&mlytical studies have been conducted to investigate the importance of specific tissue features in determining ultrasonic spectral “signatures” that have proven to be diagnostically useful. Three models of tissue microstructure were considered, and calibrated power spectra were computed for a series of scatterer sizes, concentrations, and acoustic impedances. General results were obtained, then specific parameters were employed to simulate studies of the eye and liver. These results were compared to clinical data-base information, and excellent correspondence was found. Relationship of Ultrasonic Spectral Parameters to Features of Tissue Microstructure

Quantitative microstructural sonography differs from conventional sonography in several ways. The frequency as well as the amplitude of echoes is recorded and assessed using spectral analysis techniques. The effects of diffraction of the ultrasound beam are calibrated, as are the transfer characteristics of the system. Studies of patients with alcoholic liver disease revealed significantly increased attenuation, heterogeneity of scatterers, and local scattering strength in those with severe disease; in contrast, patients with mild cirrhosis exhibited decreased local scattering strength compared to normal individuals. Using retrospective three-parameter discriminant analysis, 23 out of 24 patients were successfully categorized, suggesting that the parameters used were capable of measuring microstructural changes associated with these disease states. Focal and diffuse liver disease studied by quantitative microstructural sonography.

Data obtained from a scanning laser acoustic microscope (SLAM) were used to examine several aspects of ultrasonic backscattering from the liver. Phase interferograms from normal and abnormal human-liver specimens were digitized, and a series of algorithms was used to compute images of propagation velocity within the specimens. The propagation velocity images were then employed to simulate A- and B-mode results. These initial simulations were used to investigate how ultrasonic echo signals are related to tissue microstructure. Among the topics examined were B-mode speckling, frequency and beamwidth effects, and angulation dependencies. >
