The 11th edition of Harrison's Principles of Internal Medicine welcomes Anthony Fauci to its editorial staff, in addition to more than 85 new contributors. While the organization of the book is similar to previous editions, major emphasis has been placed on disorders that affect multiple organ systems. Important advances in genetics, immunology, and oncology are emphasized. Many chapters of the book have been rewritten and describe major advances in internal medicine. Subjects that received only a paragraph or two of attention in previous editions are now covered in entire chapters. Among the chapters that have been extensively revised are the chapters on infections in the compromised host, on skin rashes in infections, on many of the viral infections, including cytomegalovirus and Epstein-Barr virus, on sexually transmitted diseases, on diabetes mellitus, on disorders of bone and mineral metabolism, and on lymphadenopathy and splenomegaly. The provision of social work and psychiatric expertise in casualty departments may be one means of preventing unnecessary medical admissions without risk to the patients. Dr Blake's and Dr Bramble's figures do not demonstrate, however, that any advantage would attach to medical teams taking over assessment from psychiatrists except that, by implication, assessments would be completed sooner by staff working on the ward full time. What the figures actually suggest is that if assessment of overdoses were left to house doctors there would be an increase in admissions to psychiatric units (by 19°U), outpatients (by 5O°'), and referrals to social services (by 140o). So for house doctors to assess overdoses would provide no economy for the psychiatric or social services. Global, regional, and national estimates of levels of preterm birth in 2014: a systematic review and modelling analysis.

Gout is a crystal-deposition disease that results from chronic elevation of uric acid levels above the saturation point for monosodium urate (MSU) crystal formation. Initial presentation is mainly severely painful episodes of peripheral joint synovitis (acute self-limiting 'attacks') but joint damage and deformity, chronic usage-related pain and subcutaneous tophus deposition can eventually develop. The global burden of gout is substantial and seems to be increasing in many parts of the world over the past 50 years. However, methodological differences impair the comparison of gout epidemiology between countries. In this comprehensive Review, data from epidemiological studies from diverse regions of the world are synthesized to depict the geographic variation in gout prevalence and incidence. Key advances in the understanding of factors associated with increased risk of gout are also summarized. The collected data indicate that the distribution of gout is uneven across the globe, with prevalence being highest in Pacific countries. Developed countries tend to have a higher burden of gout than developing countries, and seem to have increasing prevalence and incidence of the disease. Some ethnic groups are particularly susceptible to gout, supporting the importance of genetic predisposition. Socioeconomic and dietary factors, as well as comorbidities and medications that can influence uric acid levels and/or facilitate MSU crystal formation, are also important in determining the risk of developing clinically evident gout.

Global epidemiology of gout: prevalence, incidence and risk factors

Diagnosis of breast cancer and its treatment dramatically affects women’s psychological health. This study investigated the prevalence of depression and anxiety and their related factor in breast cancer women. We conducted a cross-sectional study with a sample of 120 women with breast cancer in Zahedan, Iran, 2020. Data were collected using instruments included: Hospital Anxiety and Depression Scale (HADS), Supportive Care Needs Survey (SCNS-SF34), Multidimensional Scale of Perceived Social Support (MSPSS), The adjustment to illness measurement inventory for Iranian women with breast cancer (AIMI-IBC). We analyzed the data using the ANOVA, independent sample t-test, Kruskal Wallis, Mann‐Whitney U test, Pearson’s and Spearman’s correlation coefficients. The prevalence of depression and anxiety in women with breast cancer was 66.6% and 60%, respectively. A significant positive correlation was found between anxiety and depression and unmet psychological needs, care/support needs, and emotional turmoil coping strategy, while reasonable efforts and avoidance coping strategies and adjusting to disease were negatively correlated with anxiety and depression. Also, linear regression results indicated unmet psychological needs, emotional turmoil coping strategy, and a high level of depression predicted a high anxiety level. A lower level of depression was indicated by reasonable efforts strategy and less level of anxiety. Women with breast cancer reported a high level of depression and anxiety in Zahedan, and clinicians should pay more attention to these patients’ psychological distress. Resolving the unmet need of patients, increasing social support, and using the right coping strategy have an essential role in breast cancer women’s psychological distress control.

Relationship between musculoskeletal disorders and anthropometric indices among bus drivers in Zahedan city

Background: Occupational accidents are a major problem in industries and work environments. Objectives: This study aimed to study the relationship between workers' life quality and occupational accidents. Materials and Methods: This historical cohort study was conducted on 114 wood industry workers (living in the North of Iran) involved in a job related accident over the past 10 years. Among those with no job-related accidents, 118 workers were randomly selected so that both groups were comparable in terms of age, work history and type of job. Standardized form of SF36 for the Iranian culture was used to determine the life quality of workers. Furthermore, demographic factors were added to the SF36 questionnaire. Data were analyzed with the two independent samples t-test, Mann-Whitney and Chi-square test using the SPSS 19 software. Results: In this survey average scores for different aspects of life quality in the case group and control group were as follows, physical functioning 67.23 ± 2.413 and 81.99 ± 2.193 and mental health 59.12 ± 16.76 and 71.52 ± 18.14, respectively. There was an obvious and significant difference (P < 0.05) between physical and mental health of injured workers’ life quality compared to the uninjured group (control group). The results showed that all eight domains of life quality except limitations due to emotional problems were significantly different between the study and control groups. Conclusions: The results of this study show that unsafe work condition and high-risk behavior of workers lead to job-related accidents which affect worker’s health and their life quality.

Study of the Relationship Between Life Quality and Occupational Accidents in Wood Industry

INTRODUCTION In end-stage renal disease, there is a high incidence of secondary hyperparathyroidism. It is proposed that increasing vitamin C levels by dietary supplementation results in a decrease of parathyroid hormone (PTH) in vitamin C-deficient hemodialysis patients with secondary hyperparathyroidism. The aim of this study was the evaluation of vitamin C administration for reduction of serum PTH level in hemodialysis patients. MATERIALS AND METHODS Twenty-one hemodialysis patients with serum PTH levels less than 550 pg/mL (but more than 200 pg/mL) were administered intravenous vitamin C, 200 mg, 3 times per week for 3 months. Blood samples for measurement of PTH were obtained at the beginning of the hemodialysis session every month for three months. RESULTS The mean level of serum biointact PTH was 333.3 ± 141.3 pg/mL (reference range, 7 pg/mL to 82 pg/mL) at baseline, and it decreased to 256.5 ± 137.2 pg/mL at 1 month (P = .03). The mean PTH level was also lower than the baseline value at 2 months (260.1 ± 123.2 pg/mL, P = .03), while it increased to 328.9 ± 176.0 pg/mL at 3 months, which was still slightly lower than the baseline level (P = .13). In 15 patients (71.4%), serum levels of PTH were lower than the baseline at months 1 to 2, while in the remaining 6 (28.6%), it was higher than the baseline value. At 3 months, 5 of the 15 patients with lower PTH levels up to the 3rd month experienced an increase in these levels again. CONCLUSIONS Administration of intravenous vitamin C in hemodialysis patients noticeably decreased level of PTH, but its effect gradually diminished.

Effect of vitamin C on parathyroid hormone in hemodialysis patients with mild to moderate secondary hyperparathyroidism.

Background: Malaria continues to be a global public health challenge, particularly in developing countries. Delivery of prompt and effective diagnosis and treatment of malaria cases, detection of malaria epidemics within one week of onset and control them in less than a month, regular disease monitoring and operational classification of malaria are among the major responsibilities of the national malaria programme. The study was conducted to determine these indicators at the different level of primary health care facilities in malaria-affected provinces of Iran Methods: In this survey, data was collected from 223 health facilities including health centres, malaria posts, health houses and hospitals as well as the profile of all 5, 836 recorded malaria cases in these facilities during the year preceding the survey. Descriptive statistics (i.e. frequencies, percentages) were used to summarize the results and Chi square test was used to analyse data. Results: All but one percent of uncomplicated cases took appropriate and correctly-dosed of anti-malarial drugs in accordance to the national treatment guideline. A larger proportion of patients [85.8%; 95% CI: 84.8 - 86.8] were also given complete treatment including anti-relapse course, in line with national guidelines. About one third [35.0%; 95% CI: 33.6 - 36.4] of uncomplicated malaria cases were treated more than 48 hours after first symptoms onset. Correspondingly, half of severe malaria cases took recommended anti-malarial drugs for severe or complicated disease more than 48 hours of onset of first symptoms. The latter cases had given regular anti-malarial drugs promptly. The majority of malaria epidemics [97%; 95% CI: 90.6 - 100] in study areas were detected within one week of onset, but only half of epidemics were controlled within four weeks of detection. Just half of target districts had at least one health facility/emergency site with adequate supply and equipment stocks. Nevertheless, only one-third of them [33% (95% CI: 0.00 - 67.8)] had updated inventory of malaria foci on quarterly basis. Conclusion: To sum up, malaria case management still constitutes a public health problem in Iran. Additionally, data suggest scarcity in management and evaluation of malaria foci, detection and control of malaria epidemics as well as assignment of emergency sites across different regions of the country. Consequently, massive and substantial investments need to be made at the Ministry of Health to coordinate national malaria control programmes towards achieving determined goals and targets.
