A Panel of International Experts was convened by the Infectious Diseases Society of America (IDSA) in collaboration with the European Society for Microbiology and Infectious Diseases (ESCMID) to update the 1999 Uncomplicated Urinary Tract Infection Guidelines by the IDSA. Co-sponsoring organizations include the American Congress of Obstetricians and Gynecologists, American Urological Association, Association of Medical Microbiology and Infectious Diseases-Canada, and the Society for Academic Emergency Medicine. The focus of this work is treatment of women with acute uncomplicated cystitis and pyelonephritis, diagnoses limited in these guidelines to premenopausal, non-pregnant women with no known urological abnormalities or co-morbidities. The issues of in vitro resistance prevalence and the ecological adverse effects of antimicrobial therapy (collateral damage) were considered as important factors in making optimal treatment choices and thus are reflected in the rankings of recommendations.

International clinical practice guidelines for the treatment of acute uncomplicated cystitis and pyelonephritis in women: A 2010 update by the Infectious Diseases Society of America and the European Society for Microbiology and Infectious Diseases

Objective: The aim was to formulate clinical practice guidelines for pheochromocytoma and paraganglioma (PPGL). Participants: The Task Force included a chair selected by the Endocrine Society Clinical Guidelines Subcommittee (CGS), seven experts in the field, and a methodologist. The authors received no corporate funding or remuneration. Evidence: This evidence-based guideline was developed using the Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) system to describe both the strength of recommendations and the quality of evidence. The Task Force reviewed primary evidence and commissioned two additional systematic reviews. Consensus Process: One group meeting, several conference calls, and e-mail communications enabled consensus. Committees and members of the Endocrine Society, European Society of Endocrinology, and Americal Association for Clinical Chemistry reviewed drafts of the guidelines. en el servicio de urgencias (SU) en el diagnostico microbiologico de la neumonia adquirida
en la comunidad (NAC) en el paciente VIH
Pacientes y metodo: Estudio prospectivo de 2 anos y medio de duracion realizado en
un hospital universitario de tercer nivel Se incluyeron todos los pacientes VIH diagnosticados
de NAC en los que se procedio a la recogida de esputo Si este era de buena
calidad, segun los criterios de Murray, se procedia al cultivo del mismo Se analizaron
las siguientes variables: sexo, numero de linfocitos CD4, carga viral, proteina C reactiva
(PCR), numero de leucocitos, valor de la escala de APACHE II al ingreso y tratamiento
antirretroviral de gran actividad (TARGA) Todos los calculos estadisticos se calcularon
con el paquete SPSS version 140
Resultados: Se incluyeron un total de 120 episodios consecutivos de NAC de los cuales
se curso CU en 91 casos Se obtuvo aislamiento microbiologico en 25 (27%) casos: 20
S pneumoniae, 4 H influenzae, 1 S aureus En cuanto a las variables analizadas no se
objetivo ninguna asociacion significativa entre su valor y la positividad del CU
Conclusion: El rendimiento en el diagnostico etiologico de la NAC en el paciente VIH
mediante el CU es similar al descrito en la literatura La positividad del CU es independiente
de las variables analizadas El CU es una prueba util que, cuando es positivo en
los pacientes VIH con NAC, el germen predominante es S pneumoniae [Emergencias

Usefulness of Coronary Computed Tomography in Real Practice

The twentieth volume of EMERGENCIAS starts here. The Editorial Committee has decided this is a good moment to renew some aspects of the Journal. The most evident outcome of this renovation can be seen on the front cover. Without being a key point, the front cover says much about a journal. EMERGENCIAS’s front cover has evolved significantly since the first issues, number 0 and number 1, were published during the spring and the summer of 1988 (Figure 1). After these first issues, in 1989, the look of the journal was settled with a heading on the lower side and a photograph on the upper side. This front cover design was kept until the beginning of 1992 when, coinciding with a change in the editorial team 1 , a new look was found by members and readers. This kept the same heading design, which was placed on the upper side of the front cover, and the picture was replaced by a list with the contents of the Journal. The decision about this last feature must undoubtedly be considered as very appropriate, especially if we take into account that this has been applied to most scientific journals. Three years later, coinciding with the change in the Editorial Committee 2,3 , in the first issue of 1995 there was an evolution towards a more frequent use of the colour white and the logo of the society became larger. It kept the list of contents on the front cover, which was probably a way for the Editorial Committee to vindicate the role of our specialty in the production of scientific publications. In 1999, EMERGENCIAS underwent its last transformation and acquired the look that has been maintained until the last issue.
