Abstract: Introduction: Discomfort in patients admitted to the ICU occurs due to various reasons and leads to a stressful situation in these patients. Discomfort significantly affects the ability to cope psychologically, the process, and results of treatment. The aim of this study was to investigate the experiences of discomfort and its self-management strategies in patients admitted to the ICU. Methods: This qualitative study was conducted in the period of September 2019 to December 2020 through in-depth interviews with 13 patients admitted to the ICU who were selected by purposive sampling. Interviews continued until data saturation. All interviews were recorded, transcribed, and analyzed using MAXQDA18 software by the conventional Lundman and Graneheim content analysis method. Results: The two main themes including “hospitalization with anxiety” and “coping with the horror of ICU” emerged from the uncomfortable experiences of patients admitted to the intensive care unit. “Hospitalization with anxiety” included five subthemes: “fear of disability and possible death,” “separation from family,” “understanding ambiguity and contradiction in treatment,” “environmental disruptors,” and “painful and unfamiliar devices and treatments.” “Coping with the horror of ICU” included three subthemes: “recourse to spirituality,” “benefiting from psychosocial coping,” and “information search.”

Abstract: The chronic nature of severe mental illnesses is necessary for constant care provided by the family caregivers. Considering the significance of family caregivers in providing care, it is imperative to ensure their self-care if they are to play the role of caregiving effectively and efficiently well. The present study was conducted using the purposive sampling method and in-depth semi-structured interviews among 13 family caregivers and five health team members. The data were analyzed via a conventional content analysis approach. In the data analysis, seven main categories and 18 subcategories emerged: “A troublesome patient and me,” including “Care-induced distress,” “Caring dilemma,” “Emotional fluctuations,” and “Self-forgetfulness”; “Passive–destructive individuality,” including: “Mental stereotypes” and “Unhealthy lifestyle”; “Family turbulence,” including: “Family malfunction” and “Break in family ties”; “The conjoined influencing social factor,” including “Lack of awareness at the family and community level” and “Social stigmatization”; “lack of well-organized health care system,” including “Biomedical-oriented health system,” “Disruption in continuity of care,” and “Problems associated with the support provided by the health system”; “Disturbed economic conditions,” including “Difficulty in life with disrupted financial capability,” “Problems related to education and promoting awareness,” and “Family health care problems”; “Dysfunctional Public mental health,” including “Macro-dysfunction of mental health education,” and “Disruptive advocacy activities to support families of patients with mental disorders.” There are several barriers to family caregiver self-care planning that require health professionals to develop appropriate interventions.

Abstract: This study aimed to explore the spiritual pain concept in the Iranian-Islamic context using a hybrid research model during 2020-2021. During the first phase, international and Iranian-Islamic literature was systematically searched and reviewed. During the second phase, the researchers referred to oncology wards, palliative care centers, and intensive care units and conducted unstructured interviews with 19 dying patients. In the third phase, attributes, and final analysis of spiritual pain was extracted from the first phase, and following the second phase, the definition of spiritual pain was finalized. The results showed that spiritual pain is a type of unique transcendental pain in the context of a continuum, rooted in human nature. At the one end of the continuum, there is the pain of deprivation from worldly pleasures (oneself, the family, and others). At the other end, there is the pain of breaking away from and striving to return to one's origin (God). Exploring spiritual pain in the Iranian-Islamic context can help develop tools and clinical guidelines and plan for the presence of specialists at the bedside to relieve this pain.

Abstract: The study aimed to explore the conditions that accompany conflict of conscience experienced by nurses in intensive care units. A qualitative approach guided the study, which was undertaken between April 2020 and April 2021. A total of 15 registered nurses working in intensive care units were recruited through purposive sampling. Individual semi-structured interviews were conducted. The data were analyzed using a qualitative content analysis. The analysis revealed two themes: "conflict of conscience in relation to the structure" and "Conflict of conscience based on context." Conflict of conscience in relation to the structure consisted of two categories: conflict of interest and conflict of conscience and law. Conflict of conscience based on context consisted of two categories: care dilemmas and arbitrary behaviors with end-stage patients. A variety of factors were accompanied by a conflict of conscience for nurses in intensive care units. Given that conflict of conscience has negative consequences on the health and quality of nursing care, health care managers need to tailor strategies to reduce the negative consequences considering the findings of the study. Abstract: Background and Purpose : The reality of death is a source of concern for humans. Managing issues associated with preparation for the inevitability of death may contribute to onset of death depression for aging individuals. The study purpose was to clarify the death depression concept in older adults including relevant features, antecedents, and consequences to further nursing knowledge development. Methods : Rodgers' evolutionary concept analysis was performed using the following steps: determination of concept and appropriate scope; collection of data for identifying concept attributes, antecedents, and consequences; and evaluation of relevant and alternative concepts for further clarification. A search for articles published between 1995 and 2020 relevant to death depression in older adults was conducted in the databases PubMed, Ovid, ProQuest, CINAHL and Persian databases SID and Magiran. Fifty-three articles met the inclusion criteria and final sampling. Findings were reviewed by two independent researchers familiar with the field. Result : Based on the study findings, death depression is characterized by cognitive, affective, and behavioral attributes. Antecedents of death depression include: health concerns associated with aging such as life-limiting or psychiatric illnesses; loneliness and loss perceptions related to social relationships; socio-cultural context; identity issues; and environmental changes. Consequences of death depression include negative adjustment in later life and the potential for adaptation. Implications for Practice : Death depression in older adults may contribute to adverse mental health sequelae. Therefore, assessment for ruminative death cognitions in older adults may lead to interventions that help prevent the onset of death depression. This study provides a foundation for further research, and it contributes to the development of nursing knowledge via concept clarification.

Abstract: Background: Retinitis Pigmentosa (RP) is a genetic disease that causes irreversible progressive impairment of vision and eventual blindness due to the formation of pigment cells in the retina. Therefore, this disease is one of the problems that can reduce the abilities of patients in different dimensions and cause various injuries in different dimensions of physical, mental, psychological, social and economic for these people due to visual dysfunction. Hence, this disease threatens the health of these patients in every way. Given the genetic and incurable nature of this disease, it is important to examine the health challenges of Retinitis Pigmentosa patients. Therefore, this study was conducted to explain the health concerns of patients with Retinitis Pigmentosa. Retinitis Pigmentosa is a genetic disease that causes irreversible progressive impairment of vision and eventual blindness due to the deposition of retinal pigment cells. Therefore, this disease is one of the problems due to impaired visual function that can reduce the capabilities of patients in various dimensions and cause them various injuries in various physical, mental, psychological, social and economic dimensions. Therefore, this disease always threatens the health of these people in every way. Due to the genetic nature and incurability of this disease, it is important to investigate the health challenges of RP patients, so this study was conducted to explain the health concerns of RP patients. Progressive visual impairment is one of the problems that threatening the health of patients and has the negative effects on their abilities. This study aimed to explore the health concerns of patients with Retinitis Pigmentosa (RP). Methods: The present study was an inductive qualitative method that was performed with the conventional content analysis approach. The study was conducted from March 2016 to March 2017 and 11 patients with non-syndromic Retinitis Pigmentosa participated. The study was conducted at the Retinitis Pigmentosa Patient Support Institute located in Tehran, Iran. Participants were selected by purposive sampling. Data were collected through semi-structured face-to-face interviews. Twelve interviews (one person was interviewed twice) were conducted for at least 30 minutes to 120 minutes ) 70 minutes on average). Data collection continued until data saturation. Data were analyzed using conventional content analysis; so that based on the technique of continuous comparison, subcategories were composed of each category of similar code. Then, by comparing all the subcategories with each other, new categories were created from each category of similar categories. As the comparison progressed, all similar categories came together to form larger categories. MAXQDA software version 2018 was used to organize the findings. Rigor of data was evaluated based on four criteria: credibility, dependability, confirmability and transferability. Results: The main category of "Attrition of personal and social abilities on the dim road of life with RP" was extracted as the main category of study findings. This category means the gradual and progressive damage and destruction of force, mastery and capacity in all physical, mental, psychological and social dimensions of the patient and consequently the need for various individual supports in society. This phenomenon creates progressive visual weakness. This category consisted of two subcategories "perceived health threat" and "perceived support needs". The subcategory "perceived health threat" means the weakening of the condition and damaged abilities in various physical, mental, psychological and social dimensions, which the person has perceived; this subcategory was derived from the sub-subcategories "gradual decline in physical function", " Sustained fluctuations in mental health" and "The weakening of social capability". In addition, the subcategory of "perceived support needs", which means feeling the need for various supports from others following the limitations caused by the disease, were extracted from the sub-subcategories of "need for socio-economic support", "need for educational support”, "need for psychological support" and "need for a personal assistant ". Conclusions: Retinitis Pigmentosa Patients face with the problem of abilities impairment in various physical, mental, and social dimensions and need for support in society and need to receive comprehensive support from different social groups. Therefore, progressive visual impairment always negatively affects the independence and health of Retinitis Pigmentosa patients on a large scale. Therefore, in order to facilitate the management of various problems, maintain their independence, and promote their health, it is necessary to formulate a context-based wholistic care program through multidisciplinary cooperation by identifying their health needs and challenges. Achieving this goal requires more cooperation between different fields of health such as medicine, nursing, rehabilitation, psychology, and social worker.
