The authors review evidence that self-control may consume a limited resource. Exerting self-control may consume self-control strength, reducing the amount of strength available for subsequent self-control efforts. Coping with stress, regulating negative affect, and resisting temptations require self-control, and after such self-control efforts, subsequent attempts at self-control are more likely to fail. Continuous self-control efforts, such as vigilance, also degrade over time. These decrements in self-control are probably not due to negative moods or learned helplessness produced by the initial self-control attempt. These decrements appear to be specific to behaviors that involve self-control; behaviors that do not require self-control neither consume nor require self-control strength. It is concluded that the executive component of the self--in particular, inhibition--relies on a limited, consumable resource.

OBJECTIVE: Selective serotonin reuptake inhibitors (SSRIs) are widely used to treat depression, but the rates, timing, and baseline predictors of remission in “real world” patients are not established. The authors’ primary objectives in this study were to evaluate the effectiveness of citalopram, an SSRI, using measurement-based care in actual practice, and to identify predictors of symptom remission in outpatients with major depressive disorder. METHOD: This clinical study included outpatients with major depressive disorder who were treated in 23 psychiatric and 18 primary care “real world” settings. The patients received flexible doses of citalopram prescribed by clinicians for up to 14 weeks. ContextStudies of selected groups of persons with mental illness, such as those
who are institutionalized or seen in mental health clinics, have reported
rates of smoking to be higher than in persons without mental illness. However,
recent population-based, nationally representative data are lacking.ObjectiveTo assess rates of smoking and tobacco cessation in adults, with and
without mental illness.Design, Setting, and ParticipantsAnalysis of data on 4411 respondents aged 15 to 54 years from the National
Comorbidity Survey, a nationally representative multistage probability survey
conducted from 1991 to 1992.Main Outcome MeasuresRates of smoking and tobacco cessation according to the number and type
of psychiatric diagnoses, assessed by a modified version of the Composite
International Diagnostic Interview.ResultsCurrent smoking rates for respondents with no mental illness, lifetime
mental illness, and past-month mental illness were 22.5%, 34.8%, and 41.0%,
respectively. Lifetime smoking rates were 39.1%, 55.3%, and 59.0%, respectively
(P<.001 for all comparisons). Smokers with any
history of mental illness had a self-reported quit rate of 37.1% (P = .04), and smokers with past-month mental illness had a self-reported
quit rate of 30.5% (P<.001) compared with smokers
without mental illness (42.5%). Odds ratios for current and lifetime smoking
in respondents with mental illness in the past month vs respondents without
mental illness, adjusted for age, sex, and region of the country, were 2.7
(95% confidence interval [CI], 2.3-3.1) and 2.7 (95% CI, 2.4-3.2), respectively.
Persons with a mental disorder in the past month consumed approximately 44.3%
of cigarettes smoked by this nationally representative sample.ConclusionsPersons with mental illness are about twice as likely to smoke as other
persons but have substantial quit rates.

Smoking and Mental Illness: A Population-Based Prevalence Study

Objective: The Canadian Community Health Survey: Mental Health and Well-Being (CCHS 1.2) is the first national study to use a full version of the Composite International Diagnostic Interview. For this reason, and because of its large sample size, the CCHS 1.2 is capable of providing the best currently available description of major depression epidemiology in Canada. Using the CCHS 1.2 data, our study aimed to describe the epidemiology of major depression in Canada. Method: All estimates used appropriate sampling weights and bootstrap variance estimation procedures. The analysis consisted of estimating proportions supplemented by logistic regression modelling. Results: The lifetime prevalence of major depressive episode was 12.2%. Past-year episodes were reported by 4.8% of the sample; 1.8% reported an episode in the past 30 days. As expected, major depression was more common in women than in men, but the difference became smaller with advancing age. The peak annual prevalence occurred in the group aged 15 to 25 years. The prevalence of major depression was not related to level of education but was related to having a chronic medical condition, to unemployment, and to income. Married people had the lowest prevalence, but the effect of marital status changed with age. Logistic regression analysis suggested that the annual prevalence may increase with age in men who never married. Conclusions: The prevalence of major depression in the CCHS 1.2 was slightly lower than that reported in the US and comparable to pan-European estimates. The pattern of association with demographic and clinical variables, however, is broadly similar. An increasing prevalence with age in single (never-married) men was an unexpected finding.

Changes in women's social role over the past years likely influenced the gender gap in substance use and substance-related disorders, with potentially significant prevention and treatment implications. The authors reviewed the literature about gender differences in prevalence estimates and course of substance-related disorders. Male-to-female ratios of prevalence estimates of substance use are narrowing in different countries. The initiation of substance use is progressively taking place at younger ages, the trend being more dramatic among women as compared to men. Women's accelerated progression to dependence (so-called "telescoping effect") is a robust finding among alcohol-dependent individuals, although the effect seems to be weaker among younger individuals. As for other drugs, the literature is weaker and further research is needed. It is concluded that women's earlier age of initiation of substance use, faster progression to dependence and under-representation in addiction treatments should be addressed in future health care planning.

Gender similarities and differences: the prevalence and course of alcohol- and other substance-related disorders.

Aim An exploratory study was conducted to understand the process of recovery from gambling problems. Design Media recruitment was used to identify a resolved (n = 43) and a comparison group of active pathological gamblers (n = 63). Participants Participants showed evidence of significant problems related to gambling as well as high rates of co-morbid mood and substance use disorders. The median length of resolution was 14 months with a range of 6 weeks to 20 years. Findings Resolved gamblers reported a variety of reasons for quitting gambling, related mainly to emotional and financial factors. They did not experience a greater number of precipitating life events compared with active gamblers but did report an increase in positive and a decrease in negative life events in the year after resolution. Both resolved and active gamblers who had relatively more severe problems were more likely to have had treatment or self-help involvement, whereas those with less severe problems, if resolved, were "naturally recovered". Conclusions The results support the need for a continuum of treatment options for problem gamblers and provide helpful information about recovery processes.
