Scores of scholars followed Maurice Duverger's lead, emphasizing how institutions affect the strategic environment facing political actors and the contours of party systems. Other researchers followed Seymour Martin Lipset's and Stein Rokkan's lead, privileging social cleavages as the main features influencing party system development. Recently, scholars have attempted to reconcile or integrate the competing approaches. Pippa Norris's new book fits into this trend by evaluating the two schools of thought side by side, harnessing valuable data produced by the Comparative Study of Electoral Systems (CSES).

Pheochromocytoma crisis typically presents as paroxysmal episodes of headache, tachycardia, diaphoresis or hypertension. We describe an uncommon case of recurrent non-hypertensive heart failure with systolic dysfunction in a young female due to pheochromocytoma compression. It presented as acute pulmonary oedema while straining during pregnancy and later on as cardiogenic shock after a recreational body massage. Such crisis occurring during pregnancy is rare. Moreover, of the few reported cases of pheochromocytoma-induced cardiogenic shock, recreational body massage has not yet been reported as a trigger for this condition.

Unexpected triggers for pheochromocytoma-induced recurrent heart failure.

Objective To compare direct planimetry of aortic valve area (AVA) by cardiac magnetic resonance (CMR) imaging with transthoracic echocardiography (TTE), using the continuity equation. Methods 15 symptomatic patients with aortic stenosis were studied. AVA was measured with CMR from steady state free precession imaging by planimetry. AVA was also calculated by TTE images using the continuity equation. Results The evaluation of AVA by both CMR and TTE was possible in twelve out of fifteen patients. CMR was able to determine the AVA in all fifteen patients. AVAs obtained by CMR and TTE were very similar and a good correlation existed between the values obtained by either technique. Conclusion CMR planimetry is highly reliable and reproducible. AVAs obtained by CMR compare well with those obtained by TTE. Therefore, CMR planimetry of AVA with steady state free precession is a useful diagnostic tool, particularly if uncertainty exists.

The complementary role of cardiac magnetic resonance imaging in the evaluation of patients with aortic stenosis.

h lowing distribution of types: romanesque (n = 22; Figure 1), gothic (n = 20; Figure 2) and crenel (n = 17; Figure 3), which is similar to the distribution described for international series reported in the literature. Systemic hypertension at rest or during exercise persists in a significant number of patients after early repair of coarctation of the aorta (CoA). Recoarctation explains only a small percentage of these cases, and recent studies have suggested intrinsic anomalies in aortic arch geometry (‘‘gothic arch’’) as a possible cause, irrespective of whether the repair was surgical or percutaneous.1,2 We retrospectively assessed all magnetic resonance imaging (MRI) studies in a consecutive series of patients followed in our institution, analyzing the prevalence of the various types of aortic arch geometry: gothic, romanesque and crenel. All the studies were performed using a Signa 1.5T (GE Medical Systems, Milwaukee, WI, USA). Anatomical types were classified by two physicians experienced in MRI (>1500 exams between them), based on images acquired using black blood, cine or angiographic techniques. The type of aortic arch could be classified in 59 of the 77 consecutive patients with corrected CoA. Uhl's disease, also known as Uhl anomaly, is a rare disease secondary to selective but uncontrolled apoptosis of right ventricular myocytes during the perinatal period, after complete cardiac development, leading to the absence of right ventricular myocardium and the direct apposition of endocardium to epicardium without a myocardial layer in between, resulting in right ventricular failure. The present paper describes a case of Uhl's disease with an uncommon presentation. A 28-year-old man was admitted with dyspnea and cyanosis. Transthoracic echocardiography showed severe dilation of the right chambers, impaired right ventricular systolic function and a large ostium secundum atrial septal defect (ASD). Cardiac catheterization revealed pulmonary hypertension, with increased pulmonary capillary wedge pressure (mean 19mmHg) and Qp:QS 0.88:1. At this point, the authors considered that a main diagnosis of ASD could not explain the clinical features and hemodynamic data. A primary disease of the right ventricle was the most likely hypothesis and cardiac magnetic resonance imaging was performed, which demonstrated an extremely thin-walled right ventricle, with almost complete absence of right ventricular free wall myocardium, compatible with Uhl's disease.
