Speech Genres and Other Late Essays

This paper reports on an investigation of three interdisciplinary mental health teams. The discussion of the responses highlights the boundaries that exist between different professional roles and areas of responsibility. Whereas there is some evidence of role blurring, which was welcomed by a few respondents, others sought to preserve their own professional identity within the multidisciplinary environment. In a paradoxical sense, the lack of managerial direction and the encouragement of generic working seemed to make some respondents all the more insistent on separate professional identities. We conclude that, far from being a relic of the past or a product of ‘ingrained attitudes’, boundaries between professions are actively encouraged by the experience of interdisciplinary modes of working.

It has generated new kinds of interdisciplinary research, knowledge, and communities of practice globally. It has also acted to bring greater coherence and political force to contributions across a range of related disciplines and traditions.

The Routledge Companion to Health Humanities

Aims and objectives

To investigate the tension between individual and organisational responses to contemporary demands for compassionate interactions in health care.

Background

Health care is often said to need more compassion among its practitioners. However, this represents a rather simplistic view of the issue, situating the problem with individual practitioners rather than focusing on the overall design of care and healthcare organisations, which have often adopted a production-line approach.

Design

This is a position paper informed by a narrative literature review.

Methods

A search of the PubMed, Science Direct and CINAHL databases for the terms compassion, care and design was conducted in the research literature published from 2000 through to mid-2013.

Results

There is a relatively large literature on compassion in health care, where authors discuss the value of imbuing a variety of aspects of health services with compassion including nurses, other practitioners and, ultimately, among patients. This contrasts with the rather limited attention that compassionate practice has received in healthcare curricula and the lack of attention to how compassion is informed by organisational structures and processes. We discuss how making the clinic more welcoming for patients and promoting bidirectional compassion and compassion formation in nursing education can be part of an overall approach to the design of compassionate care.

Conclusions

We discuss a number of ways in which compassion can be enhanced through training, educational and organisational design, through exploiting the potential of brief opportunities for communication and through initiatives involving patients and service users, as well as practitioners and service leaders.

Relevance to clinical practice

The development of contemporary healthcare systems could usefully address the overall design of compassionate care rather than blame individual practitioners for a lack of compassion.

'Am I normal?' Teenagers, sexual health and the internet.

This paper draws on two strands of research and practice in language studies, namely i) studies of communication in health care encounters, and ii) studies of language corpora. It aims to delineate an area of â€˜applied clinical linguisticsâ€™ which draws on these existing sub-disciplines so as to enhance our knowledge of communicative events in clinical settings. To illustrate the potential we draw upon a corpus-informed study of communication in staged telephone conversations between callers and advisers in the UKâ€™s â€˜NHS Directâ€™ health advisory service. Here, the combined application of corpus linguistics and conversation analytic techniques has revealed several hitherto undisclosed features concerning strategies used by health advisors to position the caller as the subject of the interaction, give credentials to the advice and terminate the encounter with a â€˜convergence codaâ€™. The integrated approach which combines corpus linguistic methods with existing frameworks in text analysis of health care contexts seems to offer new possibilities of data and theory building, as well as becoming a resource for practitioners themselves in clinical field settings.
