Abstract: This double-blind parallel groups study investigated the effects of 7.5 mg midazolam on transient insomnia caused by eastward and westward multiple time zone flights. Three groups of six subjects each were treated with either midazolam or placebo during the first four post-flight nights in both directions. Sleep-wake behavior was monitored throughout the study by actigraphic recordings from the wrist of the non-dominant hand in 1 min bins and by morning questionnaires. The results showed that midazolam significantly lengthened sleep and increased the percentages of bins defined as sleep after the eastward flight. These results are discussed in view of recent findings that short-acting benzodiazepines induced phase shifts in mammalian circadian rhythms.

Abstract: A man was previously shown to suffer from a highly localized pontine lesion associated with an almost total absence of rapid-eye-movement (REM) sleep. Sleep recordings conducted 4 years later confirmed the drastic reduction of REM sleep, but demonstrated intact penile erection cycles.

Abstract: The effects of 2-week treatment with 20mg tenoxicam once daily, and 50mg diclofenac twice daily, in comparison with placebo, were investigated in 30 patients with rheumatoid arthritis (RA). Treatment efficacy was assessed by standard clinical parameters and by continuous actigraphic recordings of wrist movements.

Abstract: 38 patients with sleep apnea syndrome treated with uvulopalato-pharyngoplasty were interviewed 23 +/- 10.8 months after the operation (mean age 48.0 +/- 8.6 years, range 28-65). 22 were also studied in the sleep laboratory 10 +/- 10.7 months after operation. 3 months after operation 94% reported improvement in snoring and 77% improvement in excessive daytime sleepiness. 1 and 2 years after the operation snoring had decreased to 74% and 65%, respectively, and excessive sleepiness to 65%, for both periods. There was a decrease of at least 50% in the number of apneic periods per hour of sleep in 68%. Side-effects consisted of nasal regurgitation and vocal changes. Only age was of predictive value: the younger the patient the better the response.
