Adequate pain management is a compelling and universal requirement in health care. Despite considerable advancements, the adverse physiological and psychological implications of unmanaged pain remain substantially unresolved. Ineffective pain management can lead to a marked decrease in desirable clinical and psychological outcomes and patients' overall quality of life. Effective management of acute pain results in improved patient outcomes and increased patient satisfaction. Although research and advanced treatments in improved practice protocols have documented progressive improvements in management of acute and postoperative pain, little awareness of the effectiveness of best practices persists. Improved interventions can enhance patients' attitudes to and perceptions of pain. What a patient believes and understands about pain is critical in influencing the patient's reaction to the pain therapy provided. Use of interdisciplinary pain teams can lead to improvements in patients' pain management, pain education, outcomes, and satisfaction.

BACKGROUND: Determining and restoring physiological vertical dimension of occlusion (VDO) is the critical step during complete mouth rehabilitation. The improper VDO compromises the aesthetics, phonetics and functional efficiency of the prosthesis.Â  Various methods are suggested to determine the accurate VDO, including the facial measurements in the clinical situations with no pre-extraction records. The generalisation of correlation between the facial measurements to VDO is criticised due to gender dimorphism and racial differences. Hence, it is prudent to verify the hypothesis of facial proportion and correlation of lower third of the face to remaining craniofacial measurements in different ethnic groups. The objective of the study was to evaluate the correlation of craniofacial measurements and OVD in the Saudi-Arabian ethnic group.METHODOLOGY: Total of 228 participants from Saudi-Arabian Ethnic group were randomly recruited in this cross-sectional study. Fifteen craniofacial measurements were recorded with modified digital Vernier callipers, and OVD was recorded at centric occlusion. The obtained data were analysed by using the Spearmanâ€™s correlation and linear regression analysis.RESULTS: The Mean OVD in male participants was higher (69.25 Â± 5.54) in comparison to female participants (57.41 Â± 5.32).Â  The craniofacial measurement of Exocanthion-right labial commissure and the Mesial wall of the right external auditory canal-orbitale lateral had a strong positive correlation with VDO. The strong correlation was recorded with a trichion-upper border of right eyebrow line and trichion-Nasion only in males. Meanwhile, the length of an auricle recorded the positive correlation in female participants.CONCLUSIONS: Being simple and non-invasive technique, craniofacial measurements and linear equations could be routinely utilised to determine VDO.

An Anthropometric Study of Cranio-Facial Measurements and Their Correlation with Vertical Dimension of Occlusion among Saudi Arabian Subpopulations

Background Dental caries is a widespread disease that causes dental tissue destruction and leads to local and general complications. Gram-positive bacteria including Streptococcus mutans, Streptococcus sanguinis, and Enterococcus faecalis take part in dental caries formation. Gram-positive bacteria have cell walls that consistof a thick layer of peptidoglycan which maintains the strength and rigidity of the bacteria, as well as bacteria guard from internal osmotic pressure. The biosynthesis of peptidoglycan involves many enzymes, including the Mur family, penicillin binding protein (PBP), and sortases. Purpose This research has the intention to screen and examine the antibacterial compound of edible plant Kemangi (Ocimum basilicum L.) in terms of how it fights against some oral pathogenic bacteria of E. faecalis ATCC 29212, S. mutans ATCC 25175, and S. sanguinis ATCC 10566. Materials and methods The O. basilicum L. was macerated by several organic solvents to obtain the extracts, before then being purified using several combinations of chromatography methods and the compound was discovered via spectroscopic methods. For the assay against bacteria, the extracts and compounds were tested using agar well diffusion and microdilution assay. Results The isolated compound was identified as β-sitosterol. The compound activity against bacteria was evaluated by in vitro assay against S. sanguinis ATCC 10566 and E. faecalis ATCC 29212 with the MIC and MBC value of 25,000 and 50,000 ppm, respectively. The compound was also tested by in silico study using the molecular docking method. The molecular interaction between β-sitosterol and the protein target showed a lower binding affinity value than the native ligand and other positive controls for each protein. Based on the amino acid residue bound to the ligands, β-sitosterol on MurA and SrtA is not competitive to the positive control, showing potential as a natural antibacterial agent. Meanwhile, on the MurB and PBP, β-sitosterol and positive control do compete with each other. Conclusion The compound, isolated from O. basilicum L. leaf, was determined as β-sitosterol, which has the molecular formula C29H50O. The antibacterial activity of β-sitosterol by in vitro assay showed weak antibacterial activity, yet exhibited the potential to inhibit the biosynthesis of peptidoglycan and prevent bacteria cell wall formation by inhibiting MurA and SrtA activity via docking simulation.

Bio-Mechanism Inhibitory Prediction of β-Sitosterol from Kemangi ( Ocimum basilicum L.) as an Inhibitor of MurA Enzyme of Oral Bacteria: In vitro and in silico Study

Background The management of orofacial pain in Indonesia has not been well performed, which consequently led to an increase in the orofacial pain occurrences and a decreased quality of life. One of the possible reasons for this particular matter is the lack of evaluation on the risk factors that might induce orofacial pain in some individuals. Objective The objective of the current study was to evaluate the risk factors of orofacial pain on productive age population in West Java province, Indonesia. Methods One thousand and fifty-six participants (522 males; 534 females) were recruited for the study. A questionnaire that consists of demographic questions and questions evaluating several assumed risk factors for orofacial pain was used in a single interview. All data was analyzed by using Chi Square test to test the significance, Odds Ratio (OR), as well as Relative Risk (RR) by using SPSS version 23 (IBM Statistic, USA). Results The result of the current study revealed that bruxism (p<0.01), daytime clenching (p<0.01), and unilateral chewing (p<0.01) were significantly related to the occurrence of orofacial pain. It was also found that participants who performed multitude of heavy liftings at work have an increased risk (RR=1.19: 95% CI: 1.04 - 1.35) of having orofacial pain compared to those who do not. Conclusion Risk factors for the occurrence of orofacial pain on productive age population in Indonesian sample consisted of oral parafunctional habits and non-parafunctional habits, such as heavy lifting. Further study in this particular topic is of importance.

Risk Factors of Orofacial Pain: A Population-Based Study in West Java Province, Indonesia

Objective(s). As the most complained oral problems in Indonesia, the therapy of orofacial pain has to be constantly evaluated. The objective of the current study was to evaluate the effectiveness of orofacial pain therapy in Indonesia. Method(s). This study recruited 5412 (3816 female; 1596 male) participants from 27 districts in West Java province. Half of the participants (2714) were recruited from those who were treated at community health centers whilst the rest were those who were treated at private dental clinics. A Likert-scale questionnaire that consists of nine questions that were divided to three subsections was used. The first subsection of the questionnaire evaluated the participants’ post-therapy basic oral functions (three questions), and the second part evaluated the participants’ post-therapy pain intensity and frequency (three questions), whilst the last part evaluated the participants’ post-therapy activities (three questions). All data were then cross-tabulated and correlated by using Spearman correlation. Result. The current study revealed that out of 5412 participants, 4023 (74.33%) participants claimed that the therapy has enabled them to perform their work activity as usual, whilst 2576 (59.2%) claimed that the therapy has decreased the intensity of the pain moderately. A significant (p < 0.01) correlation (r = 0.1) between the type of dental facility visited and the total score of the therapy effectiveness was revealed. Conclusion. The therapy of orofacial pain in Indonesian sample was proven to be effective. Further study evaluating the reasons underlying the current results is of importance.

The Effectiveness of Orofacial Pain Therapy in Indonesia: A Cross-Sectional Study.

Introduction: Tooth restoration and implant-supported was one of the methods to overcome the free end cases. Abutment and implant are two components that are fused together by a screw. Therefore, the main problem to solve are loosened screw and implant or abutment fracture because of increasing unpredictable potential force on the implant, abutment, and screw. The purpose of this research was to describe the distribution stress between the connection of the body of implant and abutment on the submerged and non-submerged design of the implant supported bridge. Method: The submerged and non-submerged design implant have been analyzed using the Finite Element Method under lateral and vertical static load for 180 N. The numeric model for lower jaw posterior segmented bone was determined by computed tomography, and the load measurement was performed to observe the distribution at the connection between the body of implant and the abutment of the implant supported bridge submerge and non-submerged design. Result s : At the lateral load, the distribution strength value was 1.562x10 7 Pa, whilst for the non-submerged was 9.63x10 7 Pa. At the vertical load, the distribution strength value was 1.038x10 7 Pa, whilst for the non-submerged was 3.342x10 7 Pa. At the load of 180 N towards the vertical and lateral on the supported implant bridge, the distribution strength value had a smaller scale compared to the ultimate tensile strength (UTS), which was 1040 MPa (1.04 x 10 9 Pa). Conclusion: Both of the design including the secondary component (abutment) was safe to used as the supporting implant bridge.

Stress distribution analysis on the one-stage non-submerged and two-stage submerged system implant supported bridge

Identification of the unknown body is a medical, legal and humane responsibility of the forensic team to evaluate data relevant to the identification. The purpose of this research was to obtain necessary data to compute and analyze the Mandibular Canine Index (MCI) between Malaysian Indian males and females studying in Universitas Padjadjaran as well as to determine the Standard Mandibular Canine Index (Standard MCI) for the Malaysian Indian community. This research used a descriptive and analytical survey approach and was done though a stratified random proportional sampling technique. The result of this research established a statistical significant difference between males and females Mandibular Canine Index (MCI) with the level of confidence of 95% with the mean of males as 0.260±0.019 and females as 0.236±0.012. The Standard Mandibular Canine Index of the Malaysian Indian community was 0.248. The overall success to predict the sex using this Standard Mandibular Canine Index was 76.67%. It was concluded that Standard Mandibular Canine Index could be used for sex prediction as a supporting technique.
