SUMMARY Dirofilariasis represents a zoonotic mosaic, which includes two main filarial species (Dirofilaria immitis and D. repens) that have adapted to canine, feline, and human hosts with distinct biological and clinical implications. At the same time, both D. immitis and D. repens are themselves hosts to symbiotic bacteria of the genus Wolbachia, the study of which has resulted in a profound shift in the understanding of filarial biology, the mechanisms of the pathologies that they produce in their hosts, and issues related to dirofilariasis treatment. Moreover, because dirofilariasis is a vector-borne transmitted disease, their distribution and infection rates have undergone significant modifications influenced by global climate change. Despite advances in our knowledge of D. immitis and D. repens and the pathologies that they inflict on different hosts, there are still many unknown aspects of dirofilariasis. This review is focused on human and animal dirofilariasis, including the basic morphology, biology, protein composition, and metabolism of Dirofilaria species; the climate and human behavioral factors that influence distribution dynamics; the disease pathology; the host-parasite relationship; the mechanisms involved in parasite survival; the immune response and pathogenesis; and the clinical management of human and animal infections.

Human and Animal Dirofilariasis: the Emergence of a Zoonotic Mosaic

Cardiopulmonary nematodes of dogs and cats cause parasitic diseases of central relevance in current veterinary practice. In the recent past the distribution of canine and feline heartworms and lungworms has increased in various geographical areas, including Europe. This is true especially for the metastrongyloids Aelurostrongylus abstrusus, Angiostrongylus vasorum and Crenosoma vulpis, the filarioid Dirofilaria immitis and the trichuroid Eucoleus aerophilus (syn. Capillaria aerophila). The reasons of this emergence are little known but many drivers such as global warming, changes in vector epidemiology and movements in animal populations, may be taken into account. The purpose of this article is to review the knowledge of the most important heartworm and lungworm infections of dogs and cats in Europe. In particular recent advances in epidemiology, clinical and control are described and discussed.

Cardiopulmonary dirofilariasis is a cosmopolitan disease caused by Dirofilaria immitis, which affects mainly canids and felids. Moreover, it causes zoonotic infections, producing pulmonary dirofilariasis in humans. Heartworm disease is a vector-borne transmitted disease, thus transmission depends on the presence of competent mosquito species, which is directly related to favorable climate conditions for its development and survival. Cardiopulmonary dirofilariasis is mainly located in countries with temperate and tropical climates. Europe is one of the continents where animal dirofilariasis has been studied more extensively. In this article we review the current prevalence of canine and feline cardiopulmonary dirofilariasis in the European continent, the transmission vectors, the current changes in the distribution and the possible causes, though the analysis of the epidemiological studies carried out until 2001 and between 2002-2011. The highest prevalences have been observed in the southern European countries, which are considered historically endemic/hyperendemic countries. Studies carried out in the last 10 years suggest an expansion of cardiopulmonary dirofilariasis in dogs towards central and northern Europe. Several factors can exert an influence on the spreading of the disease, such as movement of infected animals, the introduction of new species of mosquitoes able to act as vectors, the climate change caused by the global warming, and development of human activity in new areas. Veterinary controls to prevent the spreading of this disease, programs of control of vectors, and adequate protocols of prevention of dirofilariasis in the susceptible species should be carried out.

Heartworm Disease (Dirofilaria immitis) and Their Vectors in Europe – New Distribution Trends

Faecal specimens randomly taken from 232 healthy dogs living in the area of Thessaloniki were screened for internal parasites. In 39·2% of the examined dogs excretion of Sarcocystis bovicanis (1·3%), Isospora ohioensis/burrowsi (3·9%), Giardia lamblia (0·8%), Hammondia heydorni (0·4%), Dicrocoelium dendriticum (0·8%) Diplopylidium nolleri (0·4%), Joyeuxiella pasqualei (0·8%), taeniids (0·4%), Toxocara canis (22·4%), Toxascaris leonina (1·3%), Uncinaria stenocephala (3·0%), Trichuris vulpis (2·6%), Spirocerca lupi (0·4%), or Linguatula serrata (0·4%) was detected. The overall rate of infection did not show any significant difference concerning sex or age. T. canis, however, was significantly more often found in the youngest age group (1–3 months).

A survey on the prevalence of gastrointestinal parasites of dogs in the area of Thessaloniki, Greece

Blood samples from 50 dogs with episodic weakness and 50 clinically normal dogs, all belonging to hunting breeds, were examined for microfilariae using the modified Knott's method. The prevalence of microfilaraemia in dogs belonging to the episodic weakness group was 10% for Dirofilaria immitis, 30% for Dirofilaria repens and 8% for Acanthocheilonema reconditum. In the clinically normal group, the figures were 14% for Dirofilaria repens and 4% for Acanthocheilonema reconditum. The overall percentage of dogs with microfilaremia in the episodic weakness group was 44 and that of the clinically normal group 16, with a statistically significant difference (P < 0.01). The prevalence of microfilaremia due to the nonpathogenic filarial species (Dirofilaria repens, Acanthocheilonema reconditum) was 34% and 16% for the episodic weakness group and the clinically normal dogs, respectively, with no significant difference.
