Dental resin composites have revolutionized dental care and enabled minimally invasive dentistry to preserve healthy tooth structure and provide natural-appearing esthetic results; these materials are now good alternatives to metal and amalgam restorations. Nevertheless, dental composites are being further developed to enhance their long-term clinical performance and longevity. With a complete understanding of the various material characteristics and design strategies of dental composite systems, frontiers of dental restoration research can aim towards creating novel materials that can produce very similar properties, functionalities, and internal structures as hard dental tissues. In this review article, the authors present an overview of the synthesis of the advanced dental composite systems with the recent research and development fields over the last 5 years. This review also explores how to control and optimize the required properties of the composites, ideally to increase the longevity/durability of restorations preventing recurrent caries. Research studies and commercial products are introduced to forecast the demands and trends of resin-based dental composites, in order to assist clinicians and researchers in optimal selection of materials to fulfill mechanical, physical, biological, and functional requirements.

Dental resin composites: A review on materials to product realizations

The aim of this research was to study the impact of using a short fiber-reinforced composite (SFRC) core on the fatigue performance and fracture behavior of direct large posterior composite restorations. Moreover, the influence of the consistency (flowable or packable) of occlusal composite coverage was assessed. A total of 100 intact molars were collected and randomly distributed into five groups (n = 20). Deep mesio-occlusal-distal (MOD) cavities were prepared in all groups. After adhesive treatment and rebuilding the missing interproximal walls with conventional composite, the specimens in four experimental groups were restored by an SFRC core (everX Flow), which was applied and cured either in bulk or in oblique layers (each 2 mm thick). Packable (G-aenial Posterior) or flowable (G-aenial Injectable) conventional composites were used as a final occlusal layer. The control group was restored with only packable conventional composite. Fatigue survival was measured for all specimens using a cyclic loading machine until a fracture occurred or a total of 25,000 cycles was achieved. Kaplan–Meyer survival analyses were conducted, followed by pairwise log-rank post hoc comparisons. The static load-bearing capacity of surviving teeth was tested using a universal testing machine. Fracture patterns were evaluated visually. There was no statistically significant (p > 0.05) difference in terms of survival between the tested groups. All groups for which flowable SFRC was used showed statistically significantly higher load-bearing capacities compared to the control group (p < 0.05). There were no significant differences regarding fracture resistance among the fiber-reinforced study groups. Regarding the fracture pattern during the survival analysis, all specimens that received SFRC showed a dominantly restorable type of fracture, while the control specimens presented a dominantly non-restorable type. The use of flowable SFRC as a reinforcing core for large MOD direct restorations showed promising achievements regarding fracture behavior.

Fracture Behavior of Short Fiber-Reinforced Direct Restorations in Large MOD Cavities.

Various material properties are involved in the success of endodontically treated restorations. At present, restorative composites are commonly employed as core build-up materials. This study aimed to systematically review the literature to assess the effect of using composite core materials on the in vitro fracture of endodontically treated teeth. Two different reviewers screened the literature, up to June 2021, in five distinct electronic databases: PubMed (MedLine), Scopus, Scielo, ISI Web of Science, and EMBASE. Only in vitro studies reporting the effect of the use of composite core materials on the fracture resistance of endodontically treated teeth were included. A meta-analysis was carried out using a software program (Review Manager v5.4.1; The Cochrane Collaboration, Copenhagen, Denmark). The risk of bias in each study was assessed following the parameters of another systematic review. A total of 5016 relevant papers were retrieved from all databases. After assessing the title and abstract, five publications remained for qualitative analysis. From these, only three studies remained for meta-analysis. The fracture strength of endodontically treated teeth where a core build-up composite was used was statistically significantly higher than the control (p = 0.04). Most of the analyses showed a high heterogenicity. The in vitro evidence suggests that the composite core build-up with higher filler content tended to improve the fracture resistance of the endodontically treated teeth, in comparison with conventional composite resins. This research received no external funding. Considering that this systematic review was only carried out on in vitro papers, registration was not performed. Furthermore, there were no identified clinical studies assessing core build-up materials; therefore, more well-designed research on these materials is needed.

Effect of Composite Core Materials on Fracture Resistance of Endodontically Treated Teeth: A Systematic Review and Meta-Analysis of In Vitro Studies.

Fatigue performance of endodontically treated molars restored with different dentin replacement materials.

OBJECTIVES The aim of this in vitro study was to investigate the fatigue survival and fracture behavior of endodontically treated (ET) premolars restored with different types of post-core and cuspal coverage restorations. MATERIALS AND METHODS MOD cavities were prepared on 108 extracted maxillary premolars. During the endodontic treatment, all teeth were instrumented with rotary files (ProTaper Universal) to the same apical enlargement (F2) and were obturated with a matched single cone obturation. After the endodontic procedure, the cavities were restored with different post-core and overlay restorations (n = 12/group). Three groups (A1-A3) were restored with either conventional composite core (PFC; control) or flowable short-fiber-reinforced composite (SFRC) core with/without custom-made fiber posts and without overlays. Six groups had similar post-core foundations as described above but with either direct PFC (B1-B3) or indirect CAD/CAM (C1-C3) overlays. Fatigue survival was tested for all restorations using a cyclic loading machine until fracture occurred or 50,000 cycles were completed. Kaplan-Meyer survival analysis was conducted, followed by pairwise post hoc comparisons. RESULTS None of the restored teeth survived all 50,000. Application of flowable SFRC as luting-core material with fiber post and CAD/CAD overlays (Group C3) showed superior performance regarding fatigue survival (p < 0.05) to all the other groups. Flowable SFRC with fiber post and direct overlay (Group B3) showed superior survival compared to all other direct techniques (p < 0.05), except for the same post-core foundation but without cuspal coverage (Group A3). CONCLUSIONS Custom-made fiber post and SFRC as post luting core material with or without cuspal coverage performed well in terms of fatigue resistance and survival when used for the restoration of ET premolars. CLINICAL RELEVANCE The fatigue survival of direct and indirect cuspal coverage restorations in ET MOD premolars is highly dependent on whether the core build-up is fiber-reinforced or not. The combination of short and long fibers in the form of individualized post-cores seems to offer a favorable solution in this situation.

Fatigue performance of endodontically treated premolars restored with direct and indirect cuspal coverage restorations utilizing fiber-reinforced cores.

The aim was to examine the influence of short-fiber composite (SFC) core on the fracture-behavior of different types of indirect posterior restorations. In addition, the effect of thickness ratio of SFC-core to the thickness of the veneering conventional composite (PFC) on fracture-behavior of bi-structured composite restorations was evaluated. MOD cavities with removed palatal cusps were prepared on 90 intact molars. Five groups of direct overlay restorations (n = 10/group) were fabricated having a SFC-core (everX Flow) with various thicknesses (0, 1, 2, 3, 4 mm) and layer of surface PFC (G-aenial Anterior), remaining the thickness of the bi-structure restoration to be 5 mm. Four groups of CAD/CAM-made restorations (Cerasmart 270 and e-max CAD) were fabricated either with 2-mm layer of SFC-core or without fiber reinforcement. Intact teeth (n = 10) were used as control group. Restorations were statically loaded until fracture. Fracture patterns were evaluated visually. Data were analyzed using ANOVA (p = 0.05). With indirect overlay restorations, no statistically significant differences (p > 0.05) were observed in the load-bearing capacities between restorations reinforced by 2-mm SFC-core (bi-structured) and those fabricated from plain restorative materials. ANOVA displayed that direct overlay restorations made from 4-mm layer thickness of SFC-core had significantly higher load-bearing capacities (3050 ± 574 N) (p < 0.05) among all the groups tested. Restorations (direct/indirect) combining SFC-core and a surface layer of conventional material demonstrated encouraging achievement in reference to fracture behavior. The use of flowable short-fiber composite as reinforcing base with large direct and indirect restorations may result in more repairable failure.

Influence of short-fiber composite base on fracture behavior of direct and indirect restorations.

The aim of this paper was to evaluate the fracture resistance and failure type of maxillary incisor teeth, rebuilt with various types of post-core restorations and full crowns made of either direct conventional particulate filler composite (PFC, G-aenial Anterior, GC, Tokyo, Japan) or indirect CAD/CAM restorations (composite Cerasmart 270 and glass ceramic LiSi Block from GC). One hundred (n = 10/group) central incisors were cut and divided into 10 experimental groups restored with different approaches. In approach A, teeth were restored with a core build-up composite (Gradia Core, GC) for a core and full crown of PFC. Approach B had teeth restored using composite core and prefabricated fiber posts, and a complete crown of either PFC or CAD/CAM. Approach C contained teeth restored with a core of short fiber-reinforced composite (everX Flow, GC) and prefabricated fiber posts, and a complete crown of either PFC or CAD/CAM. In approach D, the teeth had a core of short fiber-reinforced composite only, and a complete crown of either PFC or CAD/CAM restorations. The root canals were prepared, and when posts were used, they were luted with either a dual-cure resin cement (LinkForce, GC) or everX Flow. As the control, sound teeth (n = 10) were used. Restorations were quasi-statically loaded until fracture. Failure type was visually investigated. The interface between the fiber post and luting cement was investigated using SEM, before and after completion of the loading test. The data were analyzed by analysis of variance (p = 0.05) followed by Tukey’s test. None of the restorative approaches restored the fracture load strength of intact teeth (p < 0.05). Restorations with additional fiber posts (Approaches B and C) had higher load-bearing capacity (p < 0.05) than restorations without fiber posts (Approaches A and D). Restorations that had short fiber-reinforced composite cores with or without fiber posts presented more repairable failures. Using short fiber-reinforced composite as post-luting and core build-up material with conventional fiber posts proved to be a promising method to strengthen severely damaged incisors.

Influence of Post-Core and Crown Type on the Fracture Resistance of Incisors Submitted to Quasistatic Loading.

Purpose The aim of this study was to evaluate the effect of multiple firings on the bond strength between yttria-tetragonal zirconia polycrystals (Y-TZP) and 2 types of resin cements. Materials and methods Sixty 3Y-TZP specimens (LAVA Frame Multi) were divided into 3 groups depending on the following firing procedures: (1) 2-firing cycles, (2) 5-firing cycles, (3) 10-firing cycles. Two samples from each group were investigated by using SEM to determine the morphological changes. All specimens were treated with 125 µm airborne-particle abrasion and the surface roughness of each specimen was measured. The specimens from each firing group were then further divided into 2 subgroups (n = 9) to apply 2 types of resin cement (MDP-free resin cement: RelyX Unicem-RU, and MDP containing resin cement: Panavia F 2.0-PA). The shear bond strength (SBS) test was performed and failure types of all the debonded specimens were classified by using a stereomicroscope as adhesive, cohesive, and mixed. The statistical analysis of surface roughness and SBS data were performed by using 1-way ANOVA and 2-way ANOVA followed by Tukey-HSD tests (α=.05). Failure modes were calculated as a percentage for each group. Results The bond strength of RU and PA to the specimens obtained with 2 firings were not statistically different from each other (P=.1). However, the SBS values of PA were found to be significantly higher than RU for the specimens obtained with 5 and 10 firing cycles (P=.001 and P=.02, respectively). Surface roughness analysis revealed no statistical difference between groups (P=.2). The SEM analysis of samples fired 5- and 10- times showed irregularities and boundary loss in zirconia grains, and empty spaces between zirconia grains. Conclusion The bond strength of PA cement was higher than that of RU to the zirconia subjected to repeated firings (5 and 10 firing cycles). When zirconia is subjected to multiple firings, using MDP-containing resin cement can be recommended.
