Optical properties and chemical composition of the atmospheric aerosol in urban Guangzhou, China

This study examines seasonal variations of the vertical distribution of aerosols through a statistical analysis of the Cloud-Aerosol Lidar and Infrared Pathfinder Satellite Observations (CALIPSO) lidar observations from June 2006 to November 2007. A data-screening scheme is developed to attain good quality data in cloud-free conditions, and the polarization measurement is used to separate dust from non-dust aerosol. The CALIPSO aerosol observations are compared with aerosol simulations from the Goddard Chemistry Aerosol Radiation Transport (GOCART) model and aerosol optical depth (AOD) measurements from the MODerate resolution Imaging Spectroradiometer (MODIS). The CALIPSO observations of geographical patterns and seasonal variations of AOD are generally consistent with GOCART simulations and MODIS retrievals especially near source regions, while the magnitude of AOD shows large discrepancies in most regions. Both the CALIPSO observation and GOCART model show that the aerosol extinction scale heights in major dust and smoke source regions are generally higher than that in industrial pollution source regions. The CALIPSO aerosol lidar ratio also generally agrees with GOCART model within 30% on regional scales. Major differences between satellite observations and GOCART model are identified, including (1) an underestimate of aerosol extinction by GOCART over the Indian sub-continent, (2) much larger aerosol extinction calculated by GOCART than observed by CALIPSO in dust source regions, (3) much weaker in magnitude and more concentrated aerosol in the lower atmosphere in CALIPSO observation than GOCART model over transported areas in midlatitudes, and (4) consistently lower aerosol scale height by CALIPSO observation than GOCART model. Possible factors contributing to these differences are discussed.

Global view of aerosol vertical distributions from CALIPSO lidar measurements and GOCART simulations: Regional and seasonal variations

Seasonal and monthly variations of columnar aerosol optical properties over East Asia determined from multi-year MODIS, LIDAR, and AERONET Sun/sky radiometer measurements

Objective 
This study aimed to evaluate the risk for hepatotoxicity with nimesulide, a non-steroidal anti-inflammatory drug (NSAID) available in Republic of Korea but withdrawn from the market in several countries.


Methods 
A systematic review and meta-analysis were conducted of studies retrieved from PubMed, EMBASE, Cochrane, the Research Information Sharing Service and ClinicalTrials.gov up to September 2017. All studies reporting nimesulide-associated hepatotoxicity in patients as compared with the unexposed or the exposed to other NSAIDs were included. Studies using spontaneous reporting databases were included to estimate reporting odds ratio (ROR) of hepatotoxicity associated with nimesulide exposure. The association between nimesulide use and hepatotoxicity was estimated using relative risk (RR) and ROR with 95% confidence interval (CI).


Results 
A total of 25 observational studies were eligible for review. In a meta-analysis of five observational studies, nimesulide was significantly associated with hepatotoxicity [RR 2.21, 95% CI 1.72–2.83]. From studies using spontaneous reporting databases (n = 6), rates of reported hepatotoxicity were significantly higher in patients using nimesulide, compared with those treated with other NSAIDs [pooled ROR 3.99, 95% CI 2.86–5.57]. Of a total of 33 patients from case studies and series, the majority (n = 28, 84.8%) were female, and the mean age (± standard deviation) was 56.8 (± 15.6) years. Almost half of the patients on nimesulide (45.5%) either required liver transplantation or died due to fulminant hepatic failure, of whom a third developed hepatotoxicity within less than 15 days of nimesulide administration.


Conclusions 
Our study findings support previous reports of an increased risk for hepatotoxicity with nimesulide use and add to existing literature by providing risk estimates for nimesulide-associated hepatotoxicity. As the limited number of studies with primarily observational study designs were included in the analysis, more studies are needed to further describe the effects of dose and length of treatment on the risk for hepatotoxicity.

Cephalosporins that contain the N-methylthiotetrazole side chain (NMTT-cephalosporin) have been reported to be associated with coagulation-related adverse events; however, a comprehensive evaluation regarding the association is lacking. A systematic review and meta-analysis were conducted to assess the safety profile of NMTT-cephalosporins with respect to hypoprothrombinemia and bleeding. The MEDLINE, Embase, Cochrane, and RISS databases were systematically searched for clinical studies up to October 2018. The association between NMTT-cephalosporins and hypoprothrombinemia was estimated using an odds ratio (OR) with a 95% confidence interval (CI). A total of 15 studies on cefamandole, cefoperazone, cefotetan, cefmetazole, and moxalactam were identified and included in the meta-analysis. Hypoprothrombinemia (OR 1.676, 95% CI 1.275-2.203) and prothrombin time (PT) prolongation (OR 2.050, 95% CI 1.398-3.005) were significantly associated with NMTT-cephalosporins, whereas bleeding was not (OR 1.359, 95% CI 0.920-2.009). Subgroup analyses revealed that cefoperazone (OR 2.506, 95% CI 1.293-4.860), cefamandole (OR 3.247, 95% CI 1.083-9.733), and moxalactam (OR 3.367, 95% CI 1.725-6.572) were significantly associated with hypoprothrombinemia. An Antimicrobial Stewardship Program led by a multidisciplinary team could play a critical role in monitoring cephalosporin-related hypoprothrombinemia or PT prolongation in patients with underlying clinical conditions at risk for bleeding. The multidisciplinary team could also assist in communicating the potential safety concerns regarding NMTT-cephalosporin use with healthcare professionals to decrease the risk of adverse events.
