Ileocolonoscopy is recommended within the first year after surgery where treatment decisions may be affected (Statement 8C).

Thiopurines are more effective than mesalazine or imidazole antibiotics alone in post-operative prophylaxis (Statement 8F).

### 8.1 Epidemiology of post-operative Crohn's disease

In the natural history of CD, intestinal resection is almost unavoidable since about 80% of patients require surgery at some stage. Surgery is unfortunately not curative as the disease inexorably recurs in many patients. The post-operative recurrence rate varies according to the definition used: clinical, endoscopic, radiological, or surgical. It is lowest when the repeat resection rate is considered, intermediate when clinical indices are used and highest when endoscopy is employed as the diagnostic tool.1–10

Data from endoscopic follow-up of patients after resection of ileo-caecal disease have shown that in the absence of treatment, the post-operative recurrence rate is around 65–90% within 12 months and 80–100% within 3 years of the operation. The clinical recurrence without therapy is about 20–25%/year.1,10 It has been demonstrated that the post-operative clinical course of CD is best predicted by the severity of endoscopic lesions. Symptoms, in fact, appear only when severe lesions are present and it is not uncommon to observe patients with fairly advanced recurrent lesions at endoscopy who remain asymptomatic.1 For these reasons, clinical indices such as the CDAI have low sensitivity at discriminating between patients with or without post-operative recurrence.11

These data mandate strategies aimed at interrupting or delaying the natural course of post-operative recurrence. Several medications have been tried in an attempt to prevent post-operative recurrence, mostly with disappointing results. The second European evidence-based Consensus on the diagnosis and management of Crohn's disease: Special situations

This paper describes an effort to identify common metrics for task-oriented human-robot interaction (HRI). We begin by discussing the need for a toolkit of HRI metrics. We then describe the framework of our work and identify important biasing factors that must be taken into consideration. Finally, we present suggested common metrics for standardization and a case study. Preparation of a larger, more detailed toolkit is in progress.

BACKGROUND: Familial adenomatous polyposis (FAP) is a well-described inherited syndrome, which is responsible for <1% of all colorectal cancer (CRC) cases. The syndrome is characterised by the development of hundreds to thousands of adenomas in the colorectum. Almost all patients will develop CRC if they are not identified and treated at an early stage. The syndrome is inherited as an autosomal dominant trait and caused by mutations in the APC gene. Recently, a second gene has been identified that also gives rise to colonic adenomatous polyposis, although the phenotype is less severe than typical FAP. The gene is the MUTYH gene and the inheritance is autosomal recessive. In April 2006 and February 2007, a workshop was organised in Mallorca by European experts on hereditary gastrointestinal cancer aiming to establish guidelines for the clinical management of FAP and to initiate collaborative studies. Thirty-one experts from nine European countries participated in these workshops. Prior to the meeting, various participants examined the most important management issues according to the latest publications. A systematic literature search using Pubmed and reference lists of retrieved articles, and manual searches of relevant articles, was performed. During the workshop, all recommendations were discussed in detail. Because most of the studies that form the basis for the recommendations were descriptive and/or retrospective in nature, many of them were based on expert opinion. The guidelines described herein may be helpful in the appropriate management of FAP families. In order to improve the care of these families further, prospective controlled studies should be undertaken.

Guidelines for the clinical management of familial adenomatous polyposis (FAP)

The PackBots being used by the U.S. military in Afghanistan and the urban search and rescue (USAR) robots that worked the World Trade Center site are just two recent examples of mobile robots moving from the laboratory to the field. What is significant about these new applications is that they invariably involve some form of human-robot interaction (HRI) rather than the full robot autonomy that has motivated most prior research. Conducting HRI research can be extremely difficult because experimentation with physical robots is expensive and time consuming. Few roboticists have experience or interest in conducting human experimentation, and researchers in human factors or human-computer interaction often lack experience in programming robots or access to robotic platforms. HRI is an excellent candidate for simulator based research because of the relative simplicity of the systems being modeled, the behavioral fidelity possible with current physics engines and the capability of modern graphics cards to approximate camera video. In this paper we briefly introduce the USARsim simulation and discuss efforts to validate its behavior for use in Human Robot Interaction (HRI) research.

Sweet's syndrome is characterized by tender, red inflammatory nodules or papules, usually affecting the upper limbs, face or neck. It is part of the group of acute neutrophilic dermatoses that includes pyoderma gangrenosum, but can be distinguished by its appearance, distribution and histological features. Four patients with Sweet's syndrome and Crohn's disease are reported. A total of 30 cases from the literature suggest that Sweet's syndrome is an unusual extraintestinal manifestation of either Crohn's disease or ulcerative colitis. There is a strong predilection for women (87%), patients with colonic disease (100%) and those with other extraintestinal features (77%). The rash is associated with active disease in 67-80%, but may precede the onset of intestinal symptoms in 21% and has been reported 3 months after proctocolectomy for ulcerative colitis.

A video stream from a single camera is often the foundation for situational awareness in teleoperation activities. Poor camera placement, narrow field-of-view and other camera properties can significantly impair the operator's perceptual link to the environment, inviting cognitive mistakes and general disorientation. This paper provides a brief overview of viewpoint control research for 3D virtual environments (VE) to motivate a user study that evaluates the effectiveness of viewpoint controls on a simulated robotic vehicle. Findings suggest that providing a camera that is controlled independently from the orientation of the vehicle may facilitate wayfinding tasks. Moreover, there is evidence to support the use of separate cameras and interfaces for different navigational subtasks.

Human judgment is an integral part of the teleoperation process that is often heavily influenced by a single video feed returned from the remote environment. This limitation on the perceptual links to the environment leaves the operator prone to cognitive mistakes and general disorientation. These faults may be enhanced or muted, depending on the camera mountings and control opportunities that are at the disposal of the operator. These issues form the basis for an experiment to assess the effectiveness of existing and potential teleoperation controls. Findings suggest that providing a camera that is controlled independently from the orientation of the vehicle may yield significant benefits.
