Product redesign using functional backtrack with digital twin

The recent increase in the number of disasters over the world has once again brought to the agenda the question of preparedness of the hospitals, which are the most necessary units of healthcare pillar to resist these disasters. The COVID-19 epidemic disease, which has affected the whole world, has caused a large number of people to die in some countries simply because of the inadequate and incomplete planning and lack of readiness of hospitals. For this reason, determining the disaster preparedness level of hospitals is an important issue that needs to be studied and it is important in terms of disaster damage reduction. In this study, a fuzzy hybrid decision-making framework is proposed to assess hospital disaster preparedness. The framework covers three important decision-making methods. For the first phase, Intuitionistic Fuzzy Analytic Hierarchy Process (IF-AHP) is used to assign relative weights for several disaster preparedness criteria considering uncertainty. Secondly, Intuitionistic Fuzzy Decision Making Trial and Evaluation Laboratory (IF-DEMATEL) is applied to identify interrelations among these criteria and feedback. Finally, via the VlseKriterijumska Optimizacija I Kompromisno Resenje (VIKOR) method, priorities of hospitals regarding disaster readiness are obtained. A case study involving the participation of 10 Colombian tertiary hospitals is carried out to show the applicability of this fuzzy hybrid approach.

A fuzzy hybrid decision-making framework for increasing the hospital disaster preparedness: The colombian case

Disasters such as earthquakes, typhoons, floods and COVID-19 continue to threaten the lives of people in all countries. In order to cover the basic needs of the victims, emergency logistics should be implemented in time. Location-routing problem (LRP) tackles facility location problem and vehicle routing problem simultaneously to obtain the overall optimization. In response to the shortage of relief materials in the early post-disaster stage, a multi-objective model for the LRP considering fairness is constructed by evaluating the urgency coefficients of all demand points. The objectives are the lowest cost, delivery time and degree of dissatisfaction. Since LRP is a NP-hard problem, a hybrid metaheuristic algorithm of Discrete Particle Swarm Optimization (DPSO) and Harris Hawks Optimization (HHO) is designed to solve the model. In addition, three improvement strategies, namely elite-opposition learning, nonlinear escaping energy, multi-probability random walk, are introduced to enhance its execution efficiency. Finally, the effectiveness and performance of the LRP model and the hybrid metaheuristic algorithm are verified by a case study of COVID-19 in Wuhan. It demonstrates that the hybrid metaheuristic algorithm is more competitive with higher accuracy and the ability to jump out of the local optimum than other metaheuristic algorithms.

Level of emergency and disaster preparedness of public hospitals in Northwest Ethiopia: A cross-sectional study

Indonesia is a country prone to experiencing natural hazards and disasters, which have frequently damaged public infrastructure, including hospitals. The role of hospitals is crucial to alleviate the impact of disasters. However, there is still a lack of study that analyzes the factors that influence the readiness of hospitals in emergency situations. Filling in this gap, the aim of this paper is to analyze and rank hospitals across West Java and Yogyakarta, Indonesia by the resilience of their emergency management approaches. This research seeks to measure hospital resiliency during emergencies and disasters. Results indicate that the emergency and disaster management coordination, response and disaster recovery planning, communication and information management, logistics and evacuation, human resources, finance, patient care and support services, decontamination and security are key attributes for the decision-making matrix. Based on the Hospital Safety Index tool, this research proposes the Hospital Emergency and Disaster Management (HEDM) index by combining the key attributes and sub-attributes using the Technique for Order of Preference by Similarity to Ideal Solution (TOPSIS) as a multi-attribute decision-making technique. The paper concludes that the anticipated benefits of analyzing the resilience of hospitals by using HEDM is the identification of the most susceptible hospitals based on their levels of readiness and resiliency in areas which are prone to experiencing disasters. This prioritization is important for resource allocation and budget planning.

Developing hospital emergency and disaster management index using topsis method

Introduction: Hospital is an overly complex facility that plays an important role in medical response delivery during disasters. Many hospitals have collapsed or been damaged and become non-functional because of a disaster. Hospital needs to have the ability to run its normal functions during a disaster. However, studies show that hospitals located in disaster areas are not wellprepared to response to disasters due to damages caused by the disasters as well as confusion over roles and responsibilities, poor communication, lack of planning, suboptimal training, and a lack of hospital integration into community disaster planning. This can lead to a situation where the hospital is not prepared to response to disaster. Hospital preparedness plan is part of the disaster plan and should be built on a standardized protocol. Indonesia has potential risks of disasters. Nevertheless, information on the current levels of safety as well as the emergency and disaster management in hospitals is limited. This study aimed to evaluate the readiness of hospitals in Indonesia to respond to disasters.

Methods: The Indonesian version of HSI checklist from the World Health Organization (Pan American Health Organization/WHO; 2015) was used to assess hospital emergency and disaster management was translated into Bahasa Indonesia. This checklist consists of several sub-modules. The score from the overall modules was then used to categorize the Emergency and Disaster Management in a hospital into one of the following three categories: A) safe; B) at risk; or C) inadequate.

Results: All public hospitals in this study (n=5) were categorized as level B hospital, meaning that the hospitals only had limited emergency and disaster response as well as recovery planning. Coordination regarding emergency and disaster management activities was limited.

Conclusion: Hospital Disaster Preparedness is very important in disaster management. Therefore, it is mandatory that hospitals have good commitment and policies as well as good support for Hospital Safety, especially for Hospital Disaster Preparedness.
