TL;DR: Access to medicines and vaccines to prevent and treat non-communicable diseases (NCDs) is unacceptably low worldwide and governments can promote access to new and future on-patent medicinal products through coherent and equitable health and trade policies, particularly those for intellectual property.

An observational study of intravenous medication errors in the United Kingdom and in Germany
TL;DR: A high rate of i.v. medication errors of moderate to severe significance was found in two countries and three pharmacy services, with the majority of errors likely to be of 'moderate' to 'severe' outcome.
