There is increasing evidence that the pathogenesis of cancer proceeds by sequential steps from normal cells to premalignant foci to localized tumors to invasive tumors and to metastatic lesions. The concept of stages of tumor progression was initially analyzed by Foulds (1), and a model for tumor progression based on genetic instability and clonal selection has been pro posed by Nowell (2). Individual steps in tumor progression have been operationally delineated in mouse skin by the responses of cells to different chemicals (3). A progression of phenotypic changes has been described in human tumors, the anatomical localization of which has made it feasible to obtain serial biopsies (2, 4). Sequential somatic chromosomal abnormalities have been reported in human cancers including malignant mel anoma (5), colon cancer (6), gliomas (7), adenocarcinoma of the esophagus (8), and small cell carcinomas of the lung (9). In addition, a multiplicity of different mutations has been carefully documented in human breast cancer (10). The dilemma is that there are too many mutations in human tumors. In this perspective, I will consider the relationships between mutation rates and cancer. At least two mutations are required to account for the chromosomal changes observed in certain human inherited diseases (11). An analysis of the literature indicates that the spontaneous mutation rate in cells is of sufficient magnitude to account for a two mutation hypothesis for the initiation of cancer. However, a larger number of mu tations are observed in many human tumors. The spontaneous mutation rate in somatic cells is not sufficient to account for these multiple mutations. If the multiple mutations in tumors are causally associated with and not just an accompaniment of cancer, then I argue that an early step in tumor progression is one that induces a mutator phenotype. An increased mutation rate in tumors could be the basis for the multiple mutations that characterize many cancers.

Mutator Phenotype May Be Required for Multistage Carcinogenesis

Publisher Summary This chapter summarizes current methods to determine Trx and thioredoxin reductase (TR). Thioredoxin (Trx) is a small (Mr 12,000) multifunctional and ubiquitous protein characterized by having a redox-active disulfide/dithiol within the conserved active site sequence: -Trp-Cys-Gly-Pro-Cys-. Oxidized thioredoxin (Trx-S 2 ) has a disulfide, and reduced thioredoxin [Trx-(SH) 2 ] has a dithiol. Thioredoxin reductase specifically reduces Trx-S 2 to Trx-(SH) 2 using NADPH. The Trx-(SH) 2 form is a powerful protein disulfide reductase. Thus, Trx, TR, and NADPH, collectively called the thioredoxin system, operate as a powerful NADPH-dependent protein disulfide reductase system. Thioredoxin has been isolated and characterized from a wide variety of prokaryotic and eukaryotic species. Mammalian thioredoxins show about 25% sequence identity to the well-characterized E. coli protein with 108 residues. One classic function of the thioredoxin system is to act as a hydrogen donor for ribonucleotide reductase, which is essential for DNA synthesis. Redox control processes involve changes in the activity of an enzyme, a receptor, or a transcription factor via dithiol/disulfide interchange reactions. Mammalian thioredoxin reductase, with its broader substrate specificity, is likely to be involved in multiple signaling systems for redox control of cellular processes.

greater than in normal cells; i.e., cancer cells must exhibit or have exhibited a mutator phenotype (1, 8, 12). The concept that oncogenesis involves a mutator phenotype has been considered by several investigators (1 , 8, 12). An alternative hypothesis to account for the multiple mutations in tumors can be formulated based on mutation-driven clonal repopulation. Let us assume that the first mutation leads to a strong proliferative advan tage. If the mutant cell expands to 10'Â°progeny within the tumor, then a second mutation occurring at a normal frequency of 10@10 would occur with a high probability in cells already harboring the first mutation. Among the early mutations would have to be ones that confer immortality or at least extend cellular life span. If this and each subsequent mutation results in a similar clonal proliferation, multiple mutations could accumulate in each tumor cell in the absence of an increase in the mutation rate. However, this hypothesis is less attrac tive than that of a mutator phenotype, because it requires that each mutation, even a single mutation in one of two recessive alleles, causes a profound growth advantage. Despite this limitation, the fact that tumors are predominantly clonal in origin indicates that many mutations result in cellular proliferation. Moreover, clonal evolution and a mutator phenotype are not exclusive; both may be required to account for the frequency of mutations in cancers.

Microsatellite Instability: Marker of a Mutator Phenotype in Cancer

The stability of the human genome requires that mutations in the germ line be exceptionally rare events. While most mutations are neutral or have deleterious effects, a limited number of mutations are required for adaptation to environmental changes. Drake has provided evidence that DNA-based microbes have evolved a mechanism to yield a common spontaneous mutation rate of approximately 0.003 mutations per genome per replication (Drake 1991). In contrast, mutation rates of RNA viruses are much larger (Holland et al. 1982) and can approach the maximum tolerable deleterious mutation rate of one per genome (Eigen and Schuster 1977; Eigen 1993). Drake calculates that lytic RNA viruses display spontaneous mutation rates of approximately one per genome while most have mutation rates that are approximately 0.1 per genome (Drake 1993). This constancy of germline mutation rates among microbial species need not necessarily mean constancy of the somatic mutation rates. Furthermore, there need not be a constant rate for somatic mutations during development. In this review, we consider mutations in cancer, a pathology in which there appears to be an increase in the rate of somatic mutations throughout the genome. Moreover, within the eukaryotic genome, as in microbes, there are "hot-spots" that exhibit unusually high mutation frequencies. It seems conceivable to us that many tumors contain thousands of changes in DNA sequence. The major question is: how do these mutations arise, and how many are rate-limiting for tumor progression?

The Mutation Rate and Cancer

Cancer recapitulates Darwinian evolution. Mutations acquired during life that provide cells with a growth or survival advantage will preferentially multiply to form a tumor. As a result of The Cancer Genome Atlas Project, we have gathered detailed information on the nucleotide sequence changes in a number of human cancers. The sources of mutations in cancer are diverse, and the complexity of those found to be clonally present in tumors has increasingly made it difficult to identify key rate-limiting genes for tumor growth that could serve as potential targets for directed therapies. The impact of DNA sequencing on future cancer research and personalized therapy is likely to be profound and merits critical evaluation.

Mutational Heterogeneity in Human Cancers: Origin and Consequences

Two-dimensional gel electrophoresis followed by silver-staining was utilized to visualize platelet polypeptides for genetic analysis. A subset of 33 polypeptides that were most suited for scoring was selected. Families consisting of father-mother-child trios were studied. Thirty-six polypeptides of a total of 1,413 scored in children's gels exhibited the combination of a normal and a variant polypeptide. The observed index of heterozygosity of 2.55% is comparable to our previously reported findings for red cell proteins.

Genetic analysis of thirty-three platelet polypeptides detected in two-dimensional polyacrylamide gels.

A subclone of a human diploid lymphoblastoid cell line, TK-6, with consistently high cloning efficiency has been used to estimate the rates of somatic mutations on the basis of protein variation detected by two-dimensional polyacrylamide gel electrophoresis. A panel of 267 polypeptide spots per gel was screened, representing the products of approximately 263 unselected loci. The rate of human somatic mutation in vitro was estimated by measuring the proportion of protein variants among cell clones isolated at various times during continuous exponential growth of a TK-6 cell population. Three mutants of spontaneous origin were observed, giving an estimated spontaneous rate of 6 x 10(-8) electrophoretic mutations per allele per cell generation (i.e., 1.2 x 10(-7) per locus per cell generation). Following treatment of cells with N-ethyl-N-nitrosourea, a total of 74 confirmed variants at 54 loci were identified among 1143 clones analyzed (approximately 601,000 allele tests). The induced variants include 65 electromorphs which exhibit altered isoelectric charge and/or apparent molecular weight and nine nullimorphs for each of which a gene product was not detected at its usual location on the gel. The induced frequency for these 65 structural gene mutants is 1.1 x 10(-4) per allele. An excess of structural gene mutations at ten known polymorphic loci and repeat mutations at these and other loci suggest nonrandomness of mutation in human somatic cells. Nullimorphs occurring at three heterozygous loci in TK-6 cells may be caused by genetic processes other than structural gene mutation.

Estimation of mutation rates based on the analysis of polypeptide constituents of cultured human lymphoblastoid cells.

The feasibility of detecting human somatic structural gene mutations by two dimen- sional electrophoresis has been investigated. A lym- phoblastoid cell line was grown as a mass culture in the presence of ethylnitrosourea, after which cells were regrown as single cell clones. A total of 257 polypeptide spots were analyzed in gels derived from 186 clones. Four structural mutations were detected by visual analysis of the gels. Computer analysis of gels corresponding to the mutant clones was also undertaken. At a spot size threshold of 200 spots to be matched using a computer algorithm, all four mutant polypeptides were detected. These results in- dicate the usefulness of the two-dimensional ap- proach for mutagenesis studies at the protein level.
