Objective Clinical sepsis-associated biomarkers were utilized in a cecal ligation and puncture (CLP) septic mouse model to provide a reference for investigating pathophysiological mechanisms and evaluating novel therapeutic interventions for sepsis. Methods Sepsis in mice was induced by CLP, and clinical biomarkers were evaluated (survival rate, blood physiological and biochemical indices, cytokines, hepatorenal function parameters, and blood coagulation). Results The mortality rate was >70%. The body temperature, blood pressure, and heart rate decreased within 48 h. Low lactic acid was found at 8 h. The CLP mice showed typical inflammatory symptoms with decreased white blood cells and procalcitonin and increased levels of soluble triggering receptor expressed on myeloid cells-1, interleukin (IL)-6, IL-10, tumor necrosis factor-α, macrophage inflammatory protein (MIP)-1α, MIP-1β, and MIP-2. The platelet count and activated partial thromboplastin time significantly decreased, and the prothrombin time and prothrombin time-international normalized ratio markedly increased. Phenotypes of multiple organ dysfunction were found in the CLP model, including increased liver alanine aminotransferase and aspartate transaminase; significantly reduced total protein, globulin, and serum albumin; increased blood urea nitrogen and creatinine; and decreased blood glucose. Conclusion The clinical features of the CLP mouse model were similar to those of human patients with sepsis.

Acute physiology and chronic health evaluation system has been widely used in evaluation of disease severity and prognosis. It is beneficial to clinical diagnosis, determination of disease state and the improvement of therapeutic and nursing measurements. This paper reviews the process of the establishment and development of prognostic scoring systems for critical disease, outlines the principles and applications, and introduces the application status of Acute Physiology and Chronic Health Evaluation system in the surgical system, cardiovascular system, respiratory system, urinary system, digestive system, and infectious diseases. This paper also discusses the shortcomings as well as the application prospect of this scoring system.

Application status of Acute Physiology and Chronic Health Evaluation in estimating the degree of disease severity and outcomes in patients with critical diseases

Objective To retrospectively analyze the correlation between the prognosis and the lactate clearance rate in severe sepsis patients.Methods The data of 247 severe sepsis patients were col- lected,including the score of APACHEⅡafter admission to ICU,the morbidity of septic shock,the lac- tate clearance rate after 12 hours in ICU and the patients' prognosis.A total of 247 cases were divided in- to survival group and dead group,low lactate clearance rate group and high lactate clearance rate group, and the differences between them were compared for statistical significance.Results There were no differences in age,sex,the score of APACHEⅡafter admission to ICU and the initial blood concentration of lactate(P＞0.05).In survival group,the morbidity of septic shock was significantly less(31.03% vs 83.33%,P＜0.01),but the lactate clearance rate was significantly higher than in dead group[(31.86±16.34)% vs(14.77±10.35)%,P＜0.01];In high lactate clearance rate group,the morbidity of sep- tic shock and the mortality were significantly lower than in the low lactate clearance rate group(29.45% vs 90.48%,P＜0.01;30.67% vs 85.71%,P＜0.01).Conclusion The lactate clearance rate can pre- dict the prognosis of severe sepsis even in early phase.
