The aim of the study was to investigate the uniformity of the muscle motor point location for lower limb muscles in healthy subjects. Fifty-three subjects of both genders (age range: 18-50 years) were recruited. The muscle motor points were identified for the following ten muscles of the lower limb (dominant side): vastus medialis, rectus femoris, and vastus lateralis of the quadriceps femoris, biceps femoris, semitendinosus, and semimembranosus of the hamstring muscles, tibialis anterior, peroneus longus, lateral and medial gastrocnemius. The muscle motor point was identified by scanning the skin surface with a stimulation pen electrode and corresponded to the location of the skin area above the muscle in which an electrical pulse evoked a muscle twitch with the least injected current. For each investigated muscle, 0.15 ms square pulses were delivered through the pen electrode at low current amplitude (<10 mA) and frequency (2 Hz). 16 motor points were identified in the 10 investigated muscles of almost all subjects: 3 motor points for the vastus lateralis, 2 motor points for rectus femoris, vastus medialis, biceps femoris, and tibialis anterior, 1 motor point for the remaining muscles. An important inter-individual variability was observed for the position of the following 4 out of 16 motor points: vastus lateralis (proximal), biceps femoris (short head), semimembranosus, and medial gastrocnemius. Possible implications for electrical stimulation procedures and electrode positioning different from those commonly applied for thigh and leg muscles are discussed.

Recent Advancements in Microneedle Technology for Multifaceted Biomedical Applications

Surface electromyogram (sEMG) signals are easy to record and offer valuable motion information, such as symmetric and periodic motion in human gait. Due to these characteristics, sEMG is widely used in human-computer interaction, clinical diagnosis and rehabilitation medicine, sports medicine and other fields. This paper aims to improve the estimation accuracy and real-time performance, in the case of the knee joint angle in the lower limb, using a sEMG signal, in a proposed estimation algorithm of the continuous motion, based on the principal component analysis (PCA) and the regularized extreme learning machine (RELM). First, the sEMG signals, collected during the lower limb motion, are preprocessed, while feature samples are extracted from the acquired and preconditioned sEMG signals. Next, the feature samples dimensions are reduced by the PCA, as well as the knee joint angle system is measured by the three-dimensional motion capture system, are followed by the normalization of the feature variable value. The normalized sEMG feature is used as the input layer, in the RELM model, while the joint angle is used as the output layer. After training, the RELM model estimates the knee joint angle of the lower limbs, while it uses the root mean square error (RMSE), Pearson correlation coefficient and model training time as key performance indicators (KPIs), to be further discussed. The RELM, the traditional BP neural network and the support vector machine (SVM) estimation results are compared. The conclusions prove that the RELM method, not only has ensured the validity of results, but also has greatly reduced the learning train time. The presented work is a valuable point of reference for further study of the motion estimation in lower limb.

Angle Estimation for Knee Joint Movement Based on PCA-RELM Algorithm

Spasticity is a common post-stroke syndrome that imposes significant adverse impacts on patients and caregivers. This study aims to improve the efficiency of botulinum toxin (BoNT) in managing spasticity, by utilizing a three-dimensional innervation zone imaging (3DIZI) technique based on high-density surface electromyography (HD-sEMG) recordings. Stroke subjects were randomly assigned to two groups: the control group ([Formula: see text]) which received standard ultrasound-guided injections, and the experimental group ([Formula: see text]) which received 3DIZI-guided injections. The amount of BoNT given was consistent for all subjects. The Modified Ashworth Scale (MAS), compound muscle action potential (CMAP) and muscle activation volume (MAV) from bilateral biceps brachii muscles were obtained at the baseline, 3 weeks, and 3 months after injection. Intra-group and inter-group comparisons of MAS, CMAP amplitude and MAV were performed. An overall improvement in MAS of spastic elbow flexors was observed during the 3-week visit ([Formula: see text]), yet no statistically significant difference found with intra-group or inter-group analysis. Compared to the baseline, a significant reduction of CMAP amplitude and MAV were observed in the spastic biceps muscles of both groups at 3-week post-injection, and returned to approximate baseline value at 12-week post injection. A significantly higher reduction was found in CMAP amplitude ([Formula: see text]% versus [Formula: see text]%, [Formula: see text]) and MAV ([Formula: see text]% versus [Formula: see text]%, [Formula: see text]) in the experimental group compared to the control group. The study has demonstrated preliminary evidence that precisely directing BoNT to the innervation zones (IZs) localized by 3DIZI leads to a significantly higher treatment efficiency improvement in spasticity management. Results have also shown the feasibility of developing a personalized BoNT injection technique for the optimization of clinical treatment for post-stroke spasticity using proposed 3DIZI technique.

Improving Botulinum Toxin Efficiency in Treating Post-Stroke Spasticity Using 3D Innervation Zone Imaging

High density surface electromyography (EMG) can be used to estimate muscle innervation zones (IZ). The objective of this study was to compare the differences in the distribution of the biceps brachii (BB) IZ derived from voluntary contractions (VC) and electrical stimulation (ES) of the musculocutaneous nerve. Surface EMG signals were recorded from the medial and lateral BB with two 64-channel high density electrode matrices in eight healthy men. The surface EMG was recorded at different percentages of the maximal voluntary contraction (MVC) force (20–100% MVC) and at different percentages of the current needed to elicit a maximal M-wave (20–100% Imax). The IZs of the medial and lateral BB were identified from the EMG signals and expressed as a row number within a given medial-lateral column. ES current intensity had no significant effect on the group mean IZ location (p > 0.05). However, The IZ during VC was located more proximally with increasing force (p   0.05), but was more proximal in the latter than the former during 60–100% MVC (p < 0.05). ES can be used to detect spatial differences in IZ location free of the confounding effects of muscle shortening and recruitment order of different sized motor units. The method may prove beneficial for locating the IZ in patients who lack voluntary control of their musculature.

Skin wound caused by external injury is usually difficult to be cured by conventional topical administration because of its poor drug diffusion across the stratum corneum. It has been recognized that stratum corneum is the major obstacle for transdermal drug delivery. To address this issue, microneedles (MNs) have been developed to penetrate the stratum corneum of the skin and then form micron-sized pores between the epidermis and the dermis layers. As such, biomacromolecule drugs and/or insoluble drug molecules can be allowed for effective transdermal penetration. A multifunctional microneedle array patch that can avoid wound infection and promote tissue remolding has important value for wound healing. Among others, marine polysaccharides have attracted much attention in multifarious biomedical applications due to their excellent (bio)physical and chemical properties. Herein, we developed a microneedle array patch using a blend of kangfuxin (KFX), chitosan (CS), and fucoidan (FD), named KCFMN, for accelerating full-thickness wound healing. The traditional Chinese medicine KFX extracted from Periplaneta americana (PA) has effective bio-functions in promoting wound healing. The macro-/micro-morphology and (bio)physicochemical properties of such composite microneedles were also studied. We showed that the KCFMN patch displayed noticeable antibacterial properties and good cytocompatibility. In particular, the KCFMN patch significantly accelerated the wound healing development in a full-thickness wound in rats by improving the epithelial thickness and collagen deposition. Thus, this versatile KCFMN patch has great prospects as a dressing for full-thickness wound healing.
