To investigate whether health-related functioning mediates the effect of psychological job demands on sickness absence in nurses. Nurses face high job demands that can have adverse health effects resulting in sickness absence. Prospective cohort study with 1-year follow-up. Data for 2964 Norwegian nurses were collected in the period 2008-2010. At baseline, psychological job demands were measured with the Demand-Control-Support Questionnaire. Health-related functioning was assessed by the Mental Composite Score and the Physical Composite Score of the SF-12 Health Survey (2nd version). Sickness absence (no = 0, yes = 1) was self-reported at 1-year follow-up. Interaction and mediation analyses were conducted stratified by tenure ( 6 years) as a registered nurse. A total of 2180 nurses (74%) with complete data were eligible for analysis. A significant three-way interaction between job demands, control and support was found in newly licensed nurses (tenure <1-year). Baseline psychological job demands were positively associated with sickness absence at 1-year follow-up. This association was substantially weakened when Mental Composite Score and Physical Composite Score were introduced as mediator variables, indicating a partial mediation effect that was particularly pronounced in newly licensed nurses. Psychological job demands did not modify the effect of health-related functioning on sickness absence. Both mental and physical health-related functioning mediated between psychological job demands and sickness absence. Nurse managers should pay attention to health-related functioning, because poor health-related functioning may predict sickness absence, especially in newly licensed nurses.

Abstract

Aim  To explore a model designed to examine direct and indirect effects of variables on acute and chronic fatigue



Background  Although influencing factors for fatigue have been studied extensively, direct and indirect effects of predictors for acute and chronic fatigue remain poorly understood Therefore, we proposed an original prediction model for acute and chronic fatigue in Chinese nurses based on previous studies



Design  Cross-sectional, descriptive, correlational design



Methods  We used a multi-stage sampling process to finally include 581 nurses working in the general hospitals in Chengdu, China Data were collected between November 2007–March 2008 by using questionnaires A path analysis was applied to test the fit of the hypothesized model and analysis of variance



Results  The final model fits the data well A total of 44·8% of variance in acute fatigue was directly and indirectly explained by job demand, job control, support at work, exposure to hazards in work environments and sleep quality, and only indirectly influenced by intershift recovery and shift work Moreover, 61·5% of total chronic fatigue was directly and indirectly accounted for by anxiety, depression, job dissatisfaction, intershift recovery, and acute fatigue, and only indirectly influenced by job control, support at work, exposure to hazards in work environments, shift work, and sleep quality



Conclusion  Acute and chronic fatigue is affected by different factors and nurse managers should generate specific interventions to decrease them

A model for predicting acute and chronic fatigue in Chinese nurses.

Background: The Occupational Fatigue Exhaustion/Recovery Scale (OFER) was designed to assess occupational fatigue in nurses. Although the original English version of this instrument has shown high degrees of reliability and validity, a Chinese version of this scale has yet to be verified. Purpose: The aim of this study was to evaluate the psychometric properties of the OFER in a population of Chinese nurses. Methods: The scale was translated using translation and back-translation. The validities and reliabilities were evaluated on 923 qualified participants using content validity index, concurrent validity, factorial validity, internal consistency reliability, and test-retest reliability. Results: The content validity index for the OFER was .92. The correlation coefficients between the scores of the OFER subscales and the criteria in this study (varying from -.498 to .705) verified that the OFER has acceptable concurrent validity. Principal component analysis and confirmatory factor analysis revealed that three factors correspond to the structure of the original instrument and that recovery mediates the relationship between acute and chronic fatigue. The Cronbach's alpha for the chronic fatigue, acute fatigue, and intershift recovery subscales were .83, .85, and .86, respectively. Test-retest reliabilities with correlation coefficients from .61 to .78 were found in the three subscales. Conclusions/Implications for Practice: OFER is a reliable and valid instrument for assessing work-related fatigue in Chinese nurses. However, further improvement of the acute fatigue subscale is recommended. The OFER has the potential to elicit information that is useful for assessing fatigue in nurses in China. Furthermore, as it differentiates between acute and chronic fatigue, OFER may be an effective tool for guiding the development and implementation of various, related intervention measures.

Psychometric Properties of the Chinese Version of the Occupational Fatigue Exhaustion/Recovery Scale: A Test in a Nursing Population.

Aims To identify the level of workplace fatigue among midwives and factors influencing their fatigue. Background Midwives who play an important role in medical care are prone to experience workplace fatigue, which negatively affects their well-being and work quality. Methods A multi-centre cross-sectional study was conducted among 666 Chinese midwives from 38 hospitals in March 2019. Data were collected by four questionnaires of self-designed demographic questions, the Pittsburgh Sleep Quality Index, the Social Support Self-Rating Scale and the 14-item Fatigue Scale. Descriptive statistics, univariate analysis and multiple linear regression were used to analyse the data. Results Midwives had moderate levels of fatigue with the mean scores of physical fatigue, mental fatigue and total fatigue being 9.53, 6.25 and 15.79, respectively. Multiple linear regression results showed that sleep quality, social support, job satisfaction, occupational injuries, adverse life events, frequency of irregular meals and employment type were statistically significant factors influencing fatigue among the participants. Conclusions Physical and mental fatigue were generally common among midwives and were affected by personal-related and work-related factors, sleep quality and social support. Implications for nursing management Nurse administrators have the opportunity to advocate for improved health policy under the two children rule to prevent workplace fatigue amongst midwives.

Factors associated with workplace fatigue among midwives in southern China: A multi-centre cross-sectional study.

目的 调查医护人员慢性疲劳综合征(CFS)情况,分析其与生活事件、个性和应付方式等心理社会因素关系.方法对615名医护人员进行CFS和心理社会因素调查,将符合诊断标准的117人作为病例组,其他498人作为对照组,进行非条件Logistic回归病例对照研究.对39个变量进行单因素,对有显著性关联的14个因素进行变量调整,将有显著意义的10个变量拟合主效应模型.结果经常感到疲劳的医护人员232人,伴头痛219人、肌肉酸痛207人和睡眠障碍180人.危险因素包括急诊重症科(or=4.50)、内科(or=2.44)、护士(or=1.86)和年龄(or=1.40);聪慧(or=4.61)、怀疑(or=3.44)和有恒(or=1.75);正性社交其他类生活事件(or=1.58)和正性工作类学习生活事件(or=1.30);退避(or=1.52)和自责(or=1.43).结论医护人员是CFS的高危人群;心理社会因素是CFS的危险因素。
