Abstract: The British Thoracic Society has published new comprehensive guidelines for the management of pulmonary nodules. These guidelines are significantly different from those previously published, as they use two malignancy prediction calculators to better characterize the risk of malignancy. There are recommendations for a higher nodule size threshold for follow-up (≥5 mm or ≥80 mm3) and a reduction of the follow-up period to 1 year for solid pulmonary nodules; both of these will reduce the number of follow-up CT scans. PET-CT plays a crucial role in characterization also, with an ordinal scale being recommended for reporting. Radiologists will be the key in implementing these guidelines, and routine use of volumetric image-analysis software will be required to manage patients with pulmonary nodules correctly.

Abstract: A total of 40 patients with perennial allergic rhinitis were treated with either azelastine nasal spray 0.14 mg/nostril twice daily (0.56 mg/day) or cetirizine tablets 10 mg once daily. Treatment was for a period of eight weeks. The rhinitis symptoms were evaluated according to a four-point scale (0 = absent, 3 = severe). The Total Rhinitis Symptom Score (TRSS) was derived from the sum of the individual symptom scores. Symptoms were assessed at baseline prior to treatment and at weeks 2, 4 and 8. Compared baseline, TRSS for both the azelastine and cetirizine groups were less at each assessment during treatment, a slight non-significant advantage was seen in the azelastine group. At the end of the study, physicians rated global efficacy as being "good" or "excellent" in 73.7% of azelastine patients and 55.5% of cetirizine patients. Both treatments were well tolerated and no serious adverse events were reported, however, two cetirizine patients withdrew from the study because of somnolence. In conclusion, azelastine has been shown to be at least as effective as cetirizine in the relief of the symptoms of perennial allergic rhinitis.

Abstract: The COVID‐19 pandemic has produced unique challenges for society in general and healthcare workers in particular. For intensivists and anaesthetists the clinical workload has been incomparable, with multiple challenges for those providing frontline care. This has included staffing dramatically expanded intensive care units, redeployment, cross‐skilling and embracing new ways of working. It has also included involvement in many less high profile roles, such as hospital logistics planning, risk management and supply chain management to ensure critical care and emergency anaesthesia and surgery had space, staff, systems and equipment.

Abstract: Objective Colorectal cancer (CRC) screening aims to detect asymptomatic disease and thus provide the opportunity for early diagnosis and treatment. This study assesses the prevalence of significant symptoms in patients found to have CRC detected through the NHS National Bowel Cancer Screening Programme (NHS NBCSP) pilots. Method All patients in the NHS NBCSP pilots with a positive faecal occult blood completed a standardized symptomatology questionnaire before colonoscopy. This data was entered into the NHS BCS pilot database, data from the English arm has been analysed retrospectively. Results There were 200 patients diagnosed with colorectal cancer. Of these, 28.5% were Dukes A, 35% Dukes B, 31% Dukes C1 and 5.5% Dukes C2. Some 81.5% reported experiencing GI symptoms. Symptoms considered significant included rectal bleeding, change in bowel habit, tenesmus and peri-anal discomfort, reported in 47.7%, 24%%, 36.5% and 15.5% of patients respectively. In addition to this, 27% reported urgency, 20.5% reported abdominal pain and 29% reported upper GI symptoms. Discussion This data suggests a high prevalence of significant symptoms amongst patients with screening-detected CRC. It is possible that these patients would have presented via routine colorectal services if the awareness of symptoms of colorectal cancer were increased.
