Abstract: These investigations and several others that have beenpublishedwithin recentyears compel us us to hold the view that the ratio of the vital capacity to the body length, trunk length, chest circumference,surfacearea or weight or any combination of thesemeasurements, is too variable to admit of any workable standardor normal value. On the other hand the vital capacity of each individual, after he had becomeaccustomedto the use of the spirometer,will be found to be subjectto but small variations as long as good health is maintained. Thereseems to beevidenceto show that a reductionin the vital capacityis ofen the first sign of a progressivedamageto the respiratorytissue.

Abstract: Vitamin B12 deficiency is estimated to affect 10%-15% of people over the age of 60, and the laboratory diagnosis is usually based on low serum vitamin B12 levels or elevated serum methylmalonic acid and homocysteine levels. Although elderly people with low vitamin B12 status frequently lack the classical signs and symptoms of vitamin B12 deficiency, e.g. megaloblastic anemia, precise evaluation and treatment in this population is important. Absorption of crystalline vitamin B12 does not decline with advancing age. However, compared with the younger population, absorption of protein-bound vitamin B12 is decreased in the elderly, owing to a high prevalence of atrophic gastritis in this age group. Atrophic gastritis results in a low acid-pepsin secretion by the gastric mucosa, which in turn results in a reduced release of free vitamin B12 from food proteins. Furthermore, hypochlorhydria in atrophic gastritis results in bacterial overgrowth of the stomach and small intestine, and these bacteria may bind vitamin B12 for their own use. The ability to absorb crystalline vitamin B12 remains intact in older people with atrophic gastritis. The 1998 recommended daily allowance for vitamin B12 is 2.4 micrograms, but elderly people should try to obtain their vitamin B12 from either supplements or fortified foods (e.g. fortified ready-to-eat breakfast cereals) to ensure adequate absorption from the gastrointestinal tract. Because the American food supply is now being fortified with folic acid, concern is increasing about neurologic exacerbation in individuals with marginal vitamin B12 status and high-dose folate intake.

Abstract: Despite major improvements in primary prevention and acute treatment over the last decades, stroke is still a devastating disease. At the beginning of the 21st century, the age-standardized incidence of stroke in Europe ranged from 95 to 290/100,000 per year, with one-month case-fatality rates ranging from 13 to 35%. Approximately 1.1 million inhabitants of Europe suffered a stroke each year, and ischemic stroke accounted for approximately 80% of cases. Although global stroke incidence is declining, rates observed in young adults are on the rise, thus suggesting a need for strategies to improve prevention. In addition, because of the ageing population, the absolute number of stroke is expected to dramatically increase in coming years: by 2025, 1.5 million European people will suffer a stroke each year. Beyond vital prognosis, stroke patients are also at increased risk of poor outcome within the first year of the event including re-hospitalisation (33%), recurrent event (7 to 13%), dementia (7 to 23%) mild cognitive disorder (35 to 47%), depression (30 to 50%), and fatigue (35% to 92%), all of them contributing to affect health related quality of life. Given these observations, an urgent development of acute care provision, as well as resources for post-stroke therapeutic strategies, is needed.

Abstract: Une situation a bien connaitre : La carence en vitamine B12 est frequente chez le sujet âge. La recherche de cette carence doit faire partie du bilan de toute affection susceptible de traduire une carence vitaminique, avec ou sans anemie macrocytaire souvent tardive. Des implications physiopathologiques : Les relations potentielles entre les complications neuro-psychiques d'origine degenerative et les maladies cerebro- ou cardio-vasculaires, principalement par le biais de l'hyperhomocysteinemie, renforcent actuellement l'interet de l'etude de la vitamine B12 chez le sujet âge. Des causes specifiques au sujet âge : Il est important de connaitre les particularites geriatriques des causes des carences en vitamine B12, au premier rang desquelles on trouve les malabsorptions vitaminiques.
