Abstract: Цель исследования определение возможностей муль-типараметрической (мп) МРТ в диагностике рака про-статы, значений магнитно-резонансной спектроскопии (МРС), диффузионно-взвешенных изображений (ДВИ) и динамического контрастного усиления (ДКУ). МРТ про-ведено на МР-томографе Achieva (Филипс, Нидерланды) с напряженностью магнитного поля 3 Тл с применением 32 канальной фазированной катушки для тела.В исследовании приняли участие 89 мужчин, из них — 81 (91%) пациент с подозрением на рак предстательной железы.При мпМРТ у 24 (39,4%) были очаги неоплазии в од-ном сегменте одной доли — Т2 а. При Т2b у 6 (9,8%) — опухоль занимала более половины объема одной доли. В Т2 с, при поражении обеих долей, у 15 (24,6%). Распро-странение за пределы капсулы Т3 а — у 7 (11,5%). В Т3b — распространение на семенные пузырьки у 8 (13,1%). При Т4 у одного пациента (1,6%) — распространение опухоли за капсулу железы в стенки прямой кишки и мочевого пу-зыря. Пункционная биопсия проведена 81 пациенту.При МРТ с ДКУ по классификации PI-RADS, к PI-RADS 1 и 2 были отнесены — 14,8% (n=12); к PI-RADS 3–12,3% (n=10); к PI-RADS 4–34,6% (n=28) наблюдений и к PI-RADS 5–38,3% (n=31). Вероятно злокачественные и высоко по-дозрительные образования — 72,9%; подозрения на зло-качественные у 12,3%; доброкачественные и вероятно доброкачественные — 14,8%. При последующем срав-нении данных ДКУ с результатами морфологического заключения в 2,47% наблюдений была гиподиагностика и в 2,4% — гипердиагностика. Морфологически у 81 па-циента с патологией ПЖ у 75,3% (n=61) был рак, а у 24,7% (n=20) — простатит и ДГПЖ.Независимо друг от друга оценивалась информа-тивность каждой методики при мпМРТ: Т2-взвешенных изображений (Т2-ВИ) чувствительность — 86,8%, специ-фичность — 70,0%, точность — 82,7%. ДВИ чувствитель-ность — 90,0%, специфичность — 72,7%, точность — 86,4%. МРС чувствительность — 93,2%, специфичность — 77,2%, точность — 88,8%. ДКУ чувствительность — 98,3%, специ-фичность — 95,0%, точность — 97,5%.Таким образом, мультипараметрический подход в диа-гностике рака предстательной железы увеличивает каче-ство и диагностическую значимость МРТ простаты. Каждая методика несет дополнительную информацию, которая в совокупности дает полную картину поражения ПЖ и яв-ляется необходимой для назначения адекватного лечения.Ключевые слова: рак предстательной железы,классификация PI-RADSKeywords: prostate cancer, classification of PI-RADS

Abstract: Brachytherapy is one of the methods of radiotherapy allowing to deliver a highly effective radiation dose to a tumor through a minimally invasive intervention. Depending on the source, brachytherapy can be low- and high-energy. The article describes the main stages of development of prostate cancer brachytherapy both in Russia and abroad. The main methods of brachytherapy used in modern medicine are described. References to recommendations of the leading radiological organizations on low-energy brachytherapy are provided. The main indications and counterindications for brachytherapy for treatment of prostate cancer using low-energy sources are described. Summary data on the effectiveness of the method depending on prostate cancer prognosis are presented. The results of using low-energy brachytherapy at the branches of the National Medical Research Center of Radiology of the Ministry of Health of Russia are described.

Abstract: Purpose. To develop an artificial neural network for diagnosing and predicting the development of cholecystitis based on an analysis of data on risk factors, and to explore the possibilities of its application in real clinical practice. Materials and methods. The collection of materials was held in at the hospitals of the city of Kursk and included a survey of 488 patients with hepatopancreatoduodenal diseases. 203 patients were suffering from cholecystitis, in 285 patients the diagnosis of cholecystitis was excluded. Analysis of risk factors’ data (such as sex, age, bad habits, profession, family relationships, etc.) was carried out using an internally developed artificial neural network (multilayer perceptron with hyperbolic tangent as the activation function). The computer program “System of Intellectual Analysis and Diagnosis of Diseases” was registered in accordance with established procedure (Certificate No. 2017613090). Results. The use of neural network analysis of data on risk factors in comparison with the processing of information that forms a clinical picture allows the diagnosis of a potential disease with cholecystitis before the onset of symptoms. The training of the artificial neural network with a quantitative output coding the age of probable hospitalization made it possible to generate an array of values, signifficantly (α ≤ 0.001) not differing from the empirical data. The difference between the mean calculated and mean empirical values was 0.45 for the training set and 1.75 for the clinical approbation group. The mean absolute error was within the range of 1.87–2.07 years. Conclusion. 1. The proposed new approach to the diagnosis and prognosis of cholecystitis has demonstrated its effectiveness, which is confirmed in clinical approbation by the levels of sensitivity (94.44%, m = 2.26) and specificity (80.6%, m = 3.9). 2. The error in predicting the age of probable hospitalization of patients with cholecystitis did not exceed 2.29 and 2.38 years for p = 0.95 and p = 0.99, respectively.

The main trend of increasing the effectiveness of screening is the transition to digital technologies. Properly organized screening with the active participation of the population reduces mortality from breast cancer by 30%.
