A longitudinal study of psychotropic prescriptions in a teaching nursing home.

TLDR
Although the rates of psychotropic prescriptions are an important entry point for concerns about quality and appropriateness of psychotrop administration to older nursing home residents, longitudinal studies including the prescribing pattern, indications, and efficacy are needed to determine more clinically meaningful indicators of quality care.
Abstract
This study clarifies the methodological pitfalls of using cross-sectional data to characterize episodic and continuous patterns of prescribing psychotropic medications in nursing homes. Although more than half of the residents in a teaching nursing home received a psychotropic drug during their 5 years of residence, less than one-quarter were continuously medicated. The authors conclude that although the rates of psychotropic prescriptions are an important entry point f or concerns about quality and appropriateness of psychotropic administration to older nursing home residents, longitudinal studies including the prescribing pattern, indications, and efficacy are needed to determine more clinically meaningful indicators of quality care. The Role of Norepinephrine in the Behavioral and Psychological Symptoms of Dementia

TL;DR: A number of lines of evidence suggest that NE dysfunction leading to BPSD may result from increased NE activity and/or hypersensitive adrenoreceptors compensating for loss of NE neurons with progression of Alzheimer's disease (AD).

Recommendations for the Management of Behavioral and Psychological Symptoms of Dementia

TL;DR: The management of BPSD can significantly improve the quality of life for the patient and caregiver and pharmacotherapy must be monitored closely for both effectiveness and side effects, with consideration of medication withdrawal when appropriate.

Newer psychotropic medication use in nursing home residents.

TL;DR: To evaluate the use of newer psychotropic agents in nursing home residents in the era of new Health Care Financing Administration (HCFA) guidelines, a large number of residents have had previous use of these agents.

Gender, aggression and serotonergic function are associated with response to sertraline for behavioral disturbances in Alzheimer's disease.

Trait shyness in the elderly: evidence for an association with Parkinson's disease in family members and biochemical correlates.

TL;DR: Previous research suggests that given evidence for marked heritability of shyness, shy elderly should report higher rates of PD in their family members than would nonshy elderly and shy elderly without PD should exhibit psychological and biologic characteristics similar to those reported in PD.

Psychotropic Drug Use and the Risk of Hip Fracture

TL;DR: The hypothesis that the sedative and autonomic effects of psychotropic drugs increase the risk of falling and fractures in elderly persons is supported and the need for studies of this association in other populations is suggested.

Benzodiazepines of long and short elimination half-life and the risk of hip fracture.

TL;DR: Medical record review for a sample of 189 cases suggested that this finding was not due to confounding by dementia, ambulatory status, functional status, or body mass, and it was not altered by sex, age, calendar year, nursing home residence, or history of hospitalization.

A study of antipsychotic drug use in nursing homes: epidemiologic evidence suggesting misuse.

TL;DR: The review of prescriptions for Medicaid patients residing continuously for one year in 173 Tennessee nursing homes provides epidemiologic evidence suggesting misuse of antipsychotic drugs in nursing homes and illustrates the need for investigations of techniques for patient management in Nursing homes which rely less upon psychtropic drugs.

Psychoactive medication use in intermediate-care facility residents.

TL;DR: The data indicate that despite growing evidence of the risks of psychoactive drug use in elderly patients, the nursing home population studied was exposed to high levels of sedative/hypnotic and antipsychotic drug use.

Syncope in an Elderly, Institutionalised Population: Prevalence, Incidence, and Associated Risk

TL;DR: Life-table survival analysis showed no difference in the mortality of subgroups with cardiac, non-cardiac, and unknown aetiologies of syncope, which is likely a manifestation of co-morbid disease rather than an independent contributor to mortality.
