BACKGROUND: Preventive chemotherapy represents a powerful but short-term control strategy for soil-transmitted helminthiasis. Since humans are often re-infected rapidly, long-term solutions require improvements in water, sanitation, and hygiene (WASH). The purpose of this study was to quantitatively summarize the relationship between WASH access or practices and soil-transmitted helminth (STH) infection. METHODS AND FINDINGS: We conducted a systematic review and meta-analysis to examine the associations of improved WASH on infection with STH (Ascaris lumbricoides, Trichuris trichiura, hookworm [Ancylostoma duodenale and Necator americanus], and Strongyloides stercoralis). PubMed, Embase, Web of Science, and LILACS were searched from inception to October 28, 2013 with no language restrictions. Studies were eligible for inclusion if they provided an estimate for the effect of WASH access or practices on STH infection. We assessed the quality of published studies with the Grades of Recommendation, Assessment, Development and Evaluation (GRADE) approach. A total of 94 studies met our eligibility criteria; five were randomized controlled trials, whilst most others were cross-sectional studies. We used random-effects meta-analyses and analyzed only adjusted estimates to help account for heterogeneity and potential confounding respectively. Use of treated water was associated with lower odds of STH infection (odds ratio [OR] 0.46, 95% CI 0.36-0.60). Piped water access was associated with lower odds of A. lumbricoides (OR 0.40, 95% CI 0.39-0.41) and T. trichiura infection (OR 0.57, 95% CI 0.45-0.72), but not any STH infection (OR 0.93, 95% CI 0.28-3.11). Access to sanitation was associated with decreased likelihood of infection with any STH (OR 0.66, 95% CI 0.57-0.76), T. trichiura (OR 0.61, 95% CI 0.50-0.74), and A. lumbricoides (OR 0.62, 95% CI 0.44-0.88), but not with hookworm infection (OR 0.80, 95% CI 0.61-1.06). Wearing shoes was associated with reduced odds of hookworm infection (OR 0.29, 95% CI 0.18-0.47) and infection with any STH (OR 0.30, 95% CI 0.11-0.83). Handwashing, both before eating (OR 0.38, 95% CI 0.26-0.55) and after defecating (OR 0.45, 95% CI 0.35-0.58), was associated with lower odds of A. lumbricoides infection. Soap use or availability was significantly associated with lower infection with any STH (OR 0.53, 95% CI 0.29-0.98), as was handwashing after defecation (OR 0.47, 95% CI 0.24-0.90). Observational evidence constituted the majority of included literature, which limits any attempt to make causal inferences. Due to underlying heterogeneity across observational studies, the meta-analysis results reflect an average of many potentially distinct effects, not an average of one specific exposure-outcome relationship. CONCLUSIONS: WASH access and practices are generally associated with reduced odds of STH infection. Pooled estimates from all meta-analyses, except for two, indicated at least a 33% reduction in odds of infection associated with individual WASH practices or access. Although most WASH interventions for STH have focused on sanitation, access to water and hygiene also appear to significantly reduce odds of infection. Overall quality of evidence was low due to the preponderance of observational studies, though recent randomized controlled trials have further underscored the benefit of handwashing interventions. Limited use of the Joint Monitoring Program's standardized water and sanitation definitions in the literature restricted efforts to generalize across studies. While further research is warranted to determine the magnitude of benefit from WASH interventions for STH control, these results call for multi-sectoral, integrated intervention packages that are tailored to social-ecological contexts.

Water, Sanitation, Hygiene, and Soil-Transmitted Helminth Infection: A Systematic Review and Meta-Analysis

Polychaetes are usually the most abundant taxon in benthic communities and have been most often utilized as indicator species of environmental conditions. This review finds that, while the use of indicator species for a particular pollutant is not simple, polychaetes can provide a useful means of assessing the effects of poor environmental conditions. Polychaetes may be used as sensitive monitors of water quality especially in terms of the effects of pollutants on life history characteristics. They may also be utilized as general indicators of community diversity but those species indicative of lower diversity may differ geographically and temporally. While sewage is often a mixture of high organic material and other pollutants such as heavy metals and pesticides, high organic situations associated with aquaculture facilities indicates that members of the Capitella capitata species complex and the dorvilleid genus Ophryotrocha are often dominant. Some species of polychaetes are able to live in sediments very high in trace metal content and body burden of these metals often does not reflect sediment concentrations due to regulation by these species. Many species seem relatively resistant to organic contaminants and pesticides and the effects of these pollutants on life history characteristics of these species may provide a more sensitive assay method. Recent studies using biomarkers in polychaetes to indicate general heavy metal or pesticide contamination has shown some success. Polychaete species known to occur in appreciable densities in the Gulf of Nicoya, Costa Rica, and which have been most often used as indicator species of pollution, are listed as potential taxa for environmental monitoring in this tropical estuary. Assessment of spatial and seasonal water quality variation of Oum Er Rbia River (Morocco) using multivariate statistical techniques

Steroid molecules are present in all invertebrates, and some of them have established hormonal roles: this is the case for ecdysteroids in arthropods and, to a lesser extent, for vertebrate-type steroids in molluscs. Steroids are not only hormones, they may also fulfill many other functions in chemical communication, chemical defense or even digestive physiology. The increasing occurrence of endocrine disruption problems caused by environmental pollutants, which interfere in particular with reproductive physiology of vertebrates but also of invertebrates has made necessary to better understand the endocrine physiology of the latter and the role of steroids in these processes. So many attempts are being made to better understand the endocrine roles of steroids in arthropods and molluscs, and to establish whether they also fulfill similar functions in other invertebrate phyla. At the moment, both the precise identification of these steroids, the determination of their origin (endogenous versus exogenous) and of their mechanism of action are under active investigation. This research takes profit of the development of genome sequencing programs on many invertebrate species, which allow the identification of receptors and/or biosynthetic enzymes, when related to their vertebrate counterparts, but the story is not so simple, as will be exemplified by estrogen receptors of molluscs.

Steroids in aquatic invertebrates.

The aquatic environment is generally affected by the presence of environmental xenobiotic compounds. One of the major xenobiotic detoxifying enzymes is glutathione S-transferase (GST), which belongs to a family of multifunctional enzymes involved in catalyzing nucleophilic attack of the sulfur atom of glutathione (γ-glutamyl-cysteinylglycine) to an electrophilic group on metabolic products or xenobiotic compounds. Because of the unique nature of the aquatic environment and the possible pollution therein, the biochemical evolution in terms of the nature of GSTs could by uniquely expressed. The full complement of GSTs has not been studied in marine organisms, as very few aquatic GSTs have been fully characterized. The focus of this article is to present an overview of the GST superfamily and their critical role in the survival of organisms in the marine environment, emphasizing the critical roles of GSTs in the detoxification of marine organisms and the unique characteristics of their GSTs compared to those from non-marine organisms.

Marine Glutathione S-Transferases

The aim of this study was to assess aquatic environmental quality of Oued Souss (Agadir, Morocco). This estuary has been subjected for a long time to large amounts of sewage discharges and industrial effluents. Since November 2002, no waste outlets have been discharged in this site due to their connection to a wastewater purification plant. Firstly, we have compared metal tolerance of the annelid polychaete (Nereis diversicolor) originating from Oued Souss and a relatively clean site (Oualidia, Morocco). Secondly, we have evaluated with a multi-marker approach (acetylcholinesterase [AChE], glutathione-S-transferases [GSTs], catalase, thiobarbituric acid reactive substances [TBARs]) responses of worms to the pollution gradient. Results have shown that worms from Oued Souss have acquired tolerance to copper and zinc due to a long-term sub-lethal metal exposure and this metal tolerance was maintained in spite of the end of wastewater discharges in this site. Higher catalase, GSTs and TBARs values have been observed in worms from Oued Souss sampled before implantation of wastewater treatment. The multi-marker approach confirms that these worms have been submitted to various contaminants. In contrast, high inhibition in AChE activities measured in worms from Oued Souss could be explained by the continuous agricultural influence of nearest areas. The level of contamination was probably maintained since biomarker values were generally higher in worms from Oued Souss when compared to Oualidia.
