Polymer gel dosimeters are fabricated from radiation sensitive chemicals which, upon irradiation, polymerize as a function of the absorbed radiation dose. These gel dosimeters, with the capacity to uniquely record the radiation dose distribution in three-dimensions (3D), have specific advantages when compared to one-dimensional dosimeters, such as ion chambers, and two-dimensional dosimeters, such as film. These advantages are particularly significant in dosimetry situations where steep dose gradients exist such as in intensity-modulated radiation therapy (IMRT) and stereotactic radiosurgery. Polymer gel dosimeters also have specific advantages for brachytherapy dosimetry. Potential dosimetry applications include those for low-energy x-rays, high-linear energy transfer (LET) and proton therapy, radionuclide and boron capture neutron therapy dosimetries. These 3D dosimeters are radiologically soft-tissue equivalent with properties that may be modified depending on the application. The 3D radiation dose distribution in polymer gel dosimeters may be imaged using magnetic resonance imaging (MRI), optical-computerized tomography (optical-CT), x-ray CT or ultrasound. The fundamental science underpinning polymer gel dosimetry is reviewed along with the various evaluation techniques. Clinical dosimetry applications of polymer gel dosimetry are also presented.

Polymer gel dosimetry

The gamma tool was developed to quantitatively compare dose distributions, either measured or calculated. Before computing gamma, the dose and distance scales of the two distributions, referred to as evaluated and reference, are renormalized by dose and distance criteria, respectively. The renormalization allows the dose distribution comparison to be conducted simultaneously along dose and distance axes. The gamma quantity, calculated independently for each reference point, is the minimum distance in the renormalized multidimensional space between the evaluated distribution and the reference point. The gamma quantity degenerates to the dose-difference and distance-to-agreement tests in shallow and very steep dose gradient regions, respectively. Since being introduced, the gamma quantity has been used by investigators to evaluate dose calculation algorithms, and compare dosimetry measurements. This manuscript examines the gamma distribution behavior in two dimensions and evaluates the gamma distribution in the presence of data noise. Noise in the evaluated distribution causes the gamma distribution to be underestimated relative to the no-noise, condition. Noise in the reference distribution adds noise in the gamma distribution in proportion to the normalized dose noise. In typical clinical use, the fraction of points that exceed 3% and 3 mm can be extensive, so we typically use 5% and 2-3 mm in clinical evaluations. For clinical cases, the calculation time is typically 5 minutes for a 1 x 1 mm2 interpolated resolution on an 800 MHz Pentium 4 for a 14.1 x 15.2 cm2 radiographic film.

Evaluation of the gamma dose distribution comparison method

A new formulation of a tissue-equivalent polymer-gel dosimeter for the measurement of three-dimensional dose distributions of ionizing radiation has been developed. It is composed of aqueous gelatin infused with acrylamide and N, N'-methylene-bisacrylamide monomers, and made hypoxic by nitrogen saturation. Irradiation of the gel, referred to as BANG, causes localized polymerization of the monomers, which, in turn, reduces the transverse NMR relaxation times of water protons. The dose dependence of the NMR transverse relaxation rate, R2, is reproducible (less than 2% variation) and is linear up to about 8 Gy, with a slope of 0.25 s(-1)Gy(-1) at 1.5 T. Magnetic resonance imaging may be used to obtain accurate three-dimensional dose distributions with high spatial resolution. Since the radiation-induced polymers do not diffuse through the gelatin matrix, the dose distributions recorded by BANG gels are stable for long periods of time, and may be used to measure low-activity radioactive sources. Since the light-scattering properties of the polymerized regions are different from those of the clear, non-irradiated regions, the dose distributions are visible, and their optical densities are dependent on dose.

Magnetic resonance imaging of radiation dose distributions using a polymer-gel dosimeter.

A detailed study of the photochemistry of iodine and its oxides indicates that iodine species may play an important role in the tropospheric photochemical system. Methyl iodide, often observed in the marine troposphere with an average concentration of 5-10 ppt, is photolyzed and thereby produces I atoms. Chemical interactions with O3, HxOy, and NOx cause I to be converted to other inorganic compounds such as IO, HOI, IONO2, and I2. The production of these species and their subsequent recycling back to I can lead to the catalytic removal of tropospheric O3, the enhancement of the NO2/NO ratio, the destruction of HxOy free radicals, and the conversion of HO2 to OH. Ultimately, tropospheric inorganic iodine is removed by heterogeneous processes. Calculations using a numerical model to simulate tropospheric photochemistry indicate that iodine may have a strong impact upon the atmospheric O3-NOx-HxOy system. The magnitude of these effects is dependent upon the value of several uncertain rate constants and the primary source distributions of CH3I and other organic and inorganic iodine compounds.

Imaging of radiation dose distribution in gel phantoms by magnetic resonance techniques has previously been reported. In this paper a method of producing gels in which the distributions of radiation dose can also be visualized as a color change is reported. The color developed depends qualitatively and quantitatively on the concentrations of solutes in the gel.

Imaging of radiation dose by visible color development in ferrous-agarose-xylenol orange gels

Several investigations have been carried out by a number of researchers over the past few years to evaluate the utility of imaging gel dosimeters for the three-dimensional measurement of radiation fields. These have been proposed to be of particular value in mapping radiation dose distributions associated with emerging and complex approaches to cancer treatment such as conformal (CRT), intensity modulated (IMRT), "gamma knife," and pencil beam radiotherapies. Imaging of the gels has been successfully accomplished with clinical MRI units and via laser-based optical scanning. However, neither of these methods is generally accessible to all potential users, limiting the broader study and implementation of this valuable tool. We report here the design, methodology, and results of a preliminary study carried out to evaluate the utility of a new, inexpensive, and simplified approach to tomographic imaging of gel radiation dosimeters. For the purpose of this initial investigation, an array of liquid scintillation vials was prepared, containing a ferrous sulphate xylenol orange (FSX) gelatin formulation. The FSX formulation undergoes a change in optical absorption characteristics following irradiation, and the resulting color change can be observed visually. The vials were irradiated individually to different doses. Three-dimensional imaging was accomplished by tomographic reconstruction from two-dimensional optical images acquired using a diffuse, fluorescent light source, a digital charge-coupled device camera, single-photon-emission-computed tomography software, and other simple components designed by the authors. The resulting transverse images were evaluated through a region-of-interest (ROI) analysis to obtain the average change in image density in each vial as a function of radiation dose. These measured ROI values were subjected to a linear regression analysis to fit them to a straight line, and to determine the goodness of fit. Results from multiple imaging trials are compared. The correlation coefficients obtained are typically on the order of 0.98, and the p value from analysis of variance is approximately 0.05, indicating a linear and reproducible response for the dosimeter formulation and imaging system.

CCD imaging for optical tomography of gel radiation dosimeters.

Radiation dose distributions are conventionally measured using ionization chambers or diodes in liquid phantoms, or in two dimensions using film. This work describes a new application of magnetic resonance imaging to radiation dose planning. Agarose gels containing ferrous sulfate, sulfuric acid, and benzoic acid have been irradiated with /sup 137/Cs gamma rays and 6-14 MeV electrons, to doses of up to 20 Gy. The dose distributions have been imaged by magnetic resonance, making use of the effect on the T1 proton relaxation times of the radiolytic Fe/sup 3 +/. The image intensity was proportional to doses of up to 10 Gy, and images were stable for at least 24 h postirradiation. The G value for Fe/sup 3 +/ production was about 100 (molecules per 100 eV absorbed).
