The aim of this study is to evaluate the validity of the use of digital models to assess tooth size, arch length, irregularity index, arch width and crowding versus measurements generated on hand-held plaster models with digital callipers in patients with and without malocclusion. Studies comparing linear and angular measurements obtained on digital and standard plaster models were identified by searching multiple databases including MEDLINE, LILACS, BBO, ClinicalTrials.gov, the National Research Register and Pro-Quest Dissertation Abstracts and Thesis database, without restrictions relating to publication status or language of publication. Two authors were involved in study selection, quality assessment and the extraction of data. Items from the Quality Assessment of Studies of Diagnostic Accuracy included in Systematic Reviews checklist were used to assess the methodological quality of included studies. No meta-analysis was conducted. Comparisons between measurements of digital and plaster models made directly within studies were reported, and the difference between the (repeated) measurement means for digital and plaster models were considered as estimates. Seventeen relevant studies were included. Where reported, overall, the absolute mean differences between direct and indirect measurements on plaster and digital models were minor and clinically insignificant. Orthodontic measurements with digital models were comparable to those derived from plaster models. The use of digital models as an alternative to conventional measurement on plaster models may be recommended, although the evidence identified in this review is of variable quality.

The validity and reliability of ratings of dental and facial attractiveness for epidemiologic use.

Comparison of the outcome of treatment of patients with clefts of the lip and palate can yield valuable pointers for discriminating between beneficial and harmful procedures. The development of a standardized rating scheme to assess the facial appearance of these patients is described. The rating scheme can be used in conjunction with conventional cephalometric analysis and dental arch evaluation. A method is described in which the nasolabial area is masked, thus reducing the influence of the surrounding facial features since it is shown that judges are influenced by general facial attractiveness. An ordinal scale has been evaluated in which four features of the nose and lip are assessed separately and in total by a panel of judges. The nasolabial profile, the nasal symmetry, the nasal form, and vermilion border were assessed using a 5-point scale. Acceptable pooled levels of reliability and reproducibility were obtained. This standardized rating system may be used to differentiate treatment outcome in patients from different treatment centers.

Development of a method for rating nasolabial appearance in patients with clefts of the lip and palate.

The purpose of the present study was to test a new in vivo debonding device and compare in vivo bond strengths recorded by this device with in vitro bond strengths recorded by a universal testing machine such as the Instron. For the in vitro part of the study, 60 extracted premolar teeth were divided into 2 groups of 30 each. Both groups of 30 teeth had 3M Unitek Victory Twin brackets, precoated with Transbond XT composite resin, bonded to them. Shear bond strength tests were carried out in vitro using the universal testing machine on one group of 30 teeth while the debonding device was used on the other group of 30 teeth. The mean shear bond strength of the group debonded using the universal machine was 11.02 MPa and that of the group debonded with the debonding device was 12.82 MPa. For the in vivo part of the study, 8 patients randomly assigned to the research clinician from patients in The University of Alabama School of Dentistry, Department of Orthodontics, had a total of 60 premolar teeth bonded with 3M Unitek Victory Twin brackets. Following comprehensive orthodontic treatment (average time of 23 months), shear bond strength tests were carried out using the debonding device, which can measure debonding forces in vivo. The mean shear bond strength recorded in vivo was 5.47 MPa. Statistically significant differences were found between all 3 groups tested. The results appear to indicate that mean bond strengths recorded in vivo following comprehensive orthodontic treatment are significantly lower than bond strengths recorded in vitro.

Cosmetic improvement is a powerful motivating factor leading people to seek treatment. Those who believe that the rewards of facial improvement will enable them to lead more satisfying and comfortable lives should be encouraged to proceed with the treatment. However, it is absolutely essential that those contemplating treatment be counseled before-hand and fully informed on the prognosis, risks and hazards, advantages and disadvantages of such procedures before embarking on treatment.
