Postural Sway in Older Patients with Sagittal Imbalance and Young Adults during Local Vibratory Proprioceptive Stimulation

OBJECTIVE
The aim: To develop, substantiate and experimentally test the methodology for the development of coordination abilities in 6-10 years old boys with postural disorders during their physical education training sessions.


PATIENTS AND METHODS
Materials and methods: The experimental (EG) and the control groups (CG) were formed to organize the pedagogical experiment. The EG included 17 boys and the CG consisted of 19 boys. The EG was engaged in sports activities according the developed methodology and the CG followed the current school curricula on physical education.


RESULTS
Results: The methodology for the development of coordination abilities in 6-10 years old boys with postural disorders in the process of their physical education has been developed. Its peculiarity lies in the accentuated influence on the vestibular, motor and visual analyzers with the help of a system of exercises aimed at controlling movements in space in static positions and during movement.


CONCLUSION
Conclusions: The obtained results testify to the effectiveness of the developed methodology. The EG boys had significantly higher indicators of static balance and coordination abilities at body position change, as well as general coordination of movements (p ≤ 0.05) at the end of the academic year than those of the CG. In addition, the number of 6-10 years old boys with various postural disorders decreased by 7.4 % (from 26.8 % to 19.4 %).

Development of coordination abilities in 6-10 years old boys with postural disorders.

Purpose: The reason behind the balance control disorder seen in adolescent idiopathic scoliosis (AIS) has been suggested as a central nervous system dysfunction, yet it has not been investigated in detail whether this problem originates from sensory, motor, or from both systems. This study aimed to reveal the differences in the pathways that provide proprioceptive sense, motor control, and coordination between these 2 systems in female individuals with AIS. Methods: Brain Diffusion Tensor Imaging was applied to 30 healthy individuals and 30 Lenke type 1 AIS patients. All of the individuals included in the study were predominantly right-handed and aged between 10 and 18. Diffusion tensor imaging of both groups were performed bilateral tractography on the corticospinal tract (CS tr), medial lemniscus (ML), superior longitudinal fasciculus (SLF), and inferior longitudinal fasciculus (ILF) tracts using DSI Studio software. Results: Significant differences in the parameters of CS tr, ML, SLF, ILF pathways were found between the AIS and the control groups. In the AIS group, significant differences were found in the fiber count and fiber ratio of the ML that carries the proprioceptive sense and CS tr, which is responsible for the somatomotor system. There were also significant differences between the left and right CS tr, ML, SLF, and ILF pathways of the AIS group (P<0.05). Conclusions: Differences in the CS tr, ML, SLF, and ILF pathways may trigger muscular asymmetry and cause postural instability and thus spinal deformity in AIS. 

Is the Integration Problem in the Sensoriomotor System the Cause of Adolescent Idiopathic Scoliosis?

Proper posture provides the best balance and body stability at minimal muscular effort. It is constantly controlled by the central nervous system, which integrates the stimuli from the proprioceptors (deep feeling sensors), vision receptors, and balance receptors through the subcortical structures. The main purpose of the study was to describe single stance stability and its correlation with the degree of scoliosis and trunk rotation among patients suffering from idiopathic scoliosis and in the control group without scoliosis. The study included 80 patients (69 girls and 11 boys) and 40 healthy children without scoliosis (21 girls and 19 boys). The Cobb angle technique was used to determine the magnitude of the deformity. All subjects were divided into three subgroups according to Bogdanov's classification. Single stance stability with eyes open and eyes closed was assessed with an electronic postural station-Delos Postural Proprioceptive System (DPPS). In case of multiple group comparisons for variables with normal distribution ANOVA with Scheffe, post hoc test was used or Kruskal-Wallis test was used as the nonparametric equivalent. The relationship between the two continuous variables was investigated using either Pearson product-moment correlation or Spearman's rank correlation. In all these calculations, the statistical significance level was set to p < 0.05. The single stance test showed a significant difference between the stability index with eyes open and stability index with eyes closed in study and control groups. The character of these alterations is influenced by the degree of trunk rotation. The degree of scoliosis according to Bogdanov classification does not determine the decrease in stability indexes. In summary, significantly lower values of the stability index during one-leg standing with eyes closed indicated balance impairment, which is mainly connected with inadequate functioning of the proprioceptive system.

The Postural Control Indexes during Unipodal Support in Patients with Idiopathic Scoliosis.

Screening tests for body posture defects and abnormalities conducted over the past several decades have revealed a significant and constantly increasing problem of health risks in children. A sedentary lifestyle, which is considered to be the primary cause, can result in proprioceptive disorders leading to postural instability. The aim of the study was to find the correlation between the level of proprioceptive control and the number of postural disorders. The study involved a representative group of 1090 children aged 8–10 years, from randomly selected primary schools. Subjects who scored from 1 to 5 points in a prior postural screening test were qualified for the examination of the postural control system. The examination of the postural control system was carried out using an electronic station enabling assessment of postural stability and proprioception. A single leg stance test showed that the number of postural disorders does not significantly impact proprioceptive control. Proprioceptive control was found to significantly increase with the age of the children, and girls presented significantly better proprioceptive control in relation to the boys in each age group.

Evaluation of the Functional Status of the Posture Control System in Children with Detected Disorders in Body Posture

Maintaining body balance is a complex function based on the information deriving from the vestibular, visual, and proprioceptive systems. The aim of the study was to evaluate quiet single stance stability in young adults with lumbar derangement syndrome (LDS) and in the control group of the healthy subjects. The second aim of this study was to determine whether pain intensity, degree of disability, and the level of physical activity can influence postural control in patients with LDS. It is important to underline that selecting a homogeneous group of LBP patients using, for example, mechanical diagnosis and therapy method and Quebec Task Force Classification, can result in an increased sensitivity of the study. The study included 126 subjects: 70 patients with LDS (37 women, 33 men) and the control group 56 healthy volunteers (36 women, 20 men). In case of multiple group comparisons for variables with normal distribution, ANOVA post hoc test was used or, as the nonparametric equivalent, Kruskal-Wallis test. In all these calculations, the statistical significance level was set to . The stability index eyes open for the study group was 88.34 and for the control group 89.86. There was no significant difference in the level of postural control between the study and control groups ( ). The level of stability index eyes closed (SI EC) for the study group was 71.44 and for the control group 77.1. SI EC results showed significant differences in proprioceptive control during single leg stance between the study and control groups ( ). The level of pain intensity, the degree of disability, and physical activity level did not influence postural control in the study group with LDS. In summary, patients with LDS showed significantly worse proprioceptive control.
