The Nature Of The Chemical Bond

One of the long-standing challenges in experimental physics is the observation of room-temperature superconductivity1,2. Recently, high-temperature conventional superconductivity in hydrogen-rich materials has been reported in several systems under high pressure3–5. An  important discovery leading to room-temperature superconductivity is the pressure-driven disproportionation of hydrogen sulfide (H2S) to H3S, with a confirmed transition temperature of 203 kelvin at 155 gigapascals3,6. Both H2S and CH4 readily mix with hydrogen to form guest–host structures at lower pressures7, and are of  comparable size at 4 gigapascals. By introducing methane at low pressures into the H2S + H2 precursor mixture for H3S, molecular exchange is allowed within a large assemblage of van der Waals solids that are hydrogen-rich with H2 inclusions; these guest–host structures become the building blocks of superconducting compounds at extreme conditions. Here we report superconductivity in a photochemically transformed carbonaceous sulfur hydride system, starting from elemental precursors, with a maximum superconducting transition temperature of 287.7 ± 1.2 kelvin (about 15 degrees Celsius) achieved at 267 ± 10 gigapascals. The superconducting state is observed over a broad pressure range in the diamond anvil cell, from 140 to 275 gigapascals, with a sharp upturn in transition temperature above 220 gigapascals. Superconductivity is established by the observation of zero resistance, a magnetic susceptibility of up to 190 gigapascals, and reduction of the transition temperature under an external magnetic field of up to 9 tesla, with an upper critical magnetic field of about 62 tesla according to the Ginzburg–Landau model at zero temperature. The light, quantum nature of hydrogen limits the structural and stoichiometric determination of the system by X-ray scattering techniques, but Raman spectroscopy is used to probe the chemical and structural transformations before metallization. The introduction of chemical tuning within our ternary system could enable the preservation of the properties of room-temperature superconductivity at lower pressures. Room-temperature superconductivity is observed in a photochemically synthesized ternary carbonaceous sulfur hydride system at 15 °C and 267 GPa.

Abstract Haemophilia is an inherited X-linked bleeding disorder characterised by a deficiency or absence of clotting factor VIII (haemophilia A) or IX (haemophilia B), which can cause musculoskeletal bleeding. The standard treatment for haemophilia is with factor concentrates to replace the missing or deficient clotting factor. However, there is a risk that the immune system develops antibodies against the exogenous factor, known as inhibitors. Managing patients with haemophilia and inhibitors who develop bleeding in unusual sites can be challenging for the treating physician. Here, we present a rare case of patient with severe haemophilia A who was diagnosed with inhibitors after developing bleeding in the left posterior chest wall (extra pleural haematoma). The patient was successfully managed with activated prothrombin complex concentrate (aPCC) (FEIBA: FVIII inhibitor bypassing activity; Baxter AG), and the pain and swelling gradually resolved over three weeks. This case emphasises the importance of clinical suspicion of inhibitor formation in a patient already diagnosed with haemophilia A presenting with unusual bleeding that does not respond to standard treatment.

Abstract Background Despite the availability of factor replacement therapy, including prophylaxis, to treat and prevent bleeding, haemophilic arthropathy continues to be the most common complication of haemophilia and significantly impairs the quality of life in people with haemophilia (PwH). Regular periodic assessment of joint status in PwH is essential to identify early arthropathic changes and prevent the development or progression of haemophilic arthropathy. Kinematic and kinetic assessment are preferable and MRI is the gold standard for diagnosing haemophilic arthropathy, but availability is limited in developing countries. HJHS and ultrasound in conjunction with HEAD-US have been shown to effective in assessing changes indicating arthropathy. Aim This study is designed to identify which radiological assessment tool(s) works best when integrated with clinical assessment tools to examine joint health in PwH, with the aim of establishing a cheap, effective, multimodal approach to joint assessment in clinical practice in low resource settings which can then help to guide treatment. Methods A cross-sectional observational study was carried out among PwH attending a clinic in the department of medicine at Assam Medical College and Hospital, Northeast India, over a one-year time period. The elbow, knee and ankle joints of participants were assessed clinically and functionally using HJHS and FISH and radiologically by Pettersson (X-ray) and HEAD-US (ultrasound) scoring. The resulting data was analysed statistically to ascertain correlation between these parameters. Results Sixty-seven PwH were enrolled in the study, with a mean age of 21.69±8.24 years (median 21 years); 38 (57.72%) had severe factor deficiency. The majority (71.70%) presented with joint bleeds; 40 (59.70%) had the knee as a target joint, followed by the elbow (11; 16.41%) and ankle (6; 8.95%). Mean ABR (annual bleed rate), AJBR (annual joint bleed rate) and ATJBR (annual target joint bleed rate) values had a positive correlation with increased severity of factor deficiency (p <0.001). FISH score increased with factor activity level; HJHS, HEAD-US and Pettersson scores decreased with factor activity level. Significant positive correlation was established between Pettersson score, age and ABR. The degree of association was greatest between HJHS and HEAD-US at 60.62%; between HJHS and Pettersson it was 57.74%. Statistically significant negative correlation was established between FISH and HEAD-US. Conclusion A combined multimodal approach using the HJHS, FISH and HEAD-US scoring systems can provide a cheap, quick, more reliable and accurate option for assessing haemophilic joints, with minimal inter and intra observer reliability. This can support the optimisation of management of joint damage in PwH in resource-constrained settings.

Clinical and radiological assessment of joints in people with haemophilia in Assam, Northeast India: a cross-sectional study

The mixed ligand complexes of Co(II) metal ions with some heterocyclic nitrogen bases along with
1-cyano-1-carboethoxyethylene-2,2-dithiolate ion (CED2−) of the compositions, Co(N-N)(CED)·2H2O
[N-N = o-phen/2,2′-bipy] and Co(N-N)(CED)L2 [N-N = o-phen/2,2′-bipy, CED2– = 1-cyano-1-
carboethoxyethylene-2,2-dithiolate, L = γ-picoline (γ-pic), β-picoline (β-pic), α-picoline (α-pic),
pyridine (py)] were characterized and isolated by analytical data as well as physico-chemical methods
for example infrared spectroscopy, electronic, magnetic susceptibility, molar conductance. The
complexes are soluble in coordinating solvents like DMSO and DMF and decompose below 250 ºC.
The synthesized complexes in DMF solution are non-electrolytic as shown by their molar conductance
data. The calculated magnetic moments suggest that the synthesixed cobalt(II) compounds are
paramagnetic and produce a low-spin environment for the Co(II) ion. Thermal studies reveal that
Co(N-N) contains two coordinated water molecules (CED)·2H2O. Electronic spectra of cobalt(II)
complexes with mixed ligands in DMF solution are consistent with distorted octahedral stereochemistry.
The IR studies indicates that the CED2- ion exhibits bidentate chelating behaviour and that heterocyclic
nitrogen donors coordinate to Co atoms (II). Reaction with heterocyclic nitrogen donors (γ-picoline,
β-picoline, α-picoline, pyridine) signifying the ligand exchange reaction.
