Guidelines for the Echocardiographic Assessment of the Right Heart in Adults: A Report from the American Society of Echocardiography

ESC guidelines for the management of atrial fibrillation developed in collaboration with EACTS

Background— Although studies have suggested that “late-onset” hypertrophic cardiomyopathy (HCM) may be caused by sarcomeric protein gene mutations, the cause of HCM in the majority of patients is unknown. This study determined the prevalence of a potentially treatable cause of hypertrophy, Anderson-Fabry disease, in a HCM referral population. Methods and Results— Plasma α-galactosidase A (α-Gal) was measured in 79 men with HCM who were diagnosed at ≥40 years of age (52.9±7.7 years; range, 40–71 years) and in 74 men who were diagnosed at <40 years (25.9±9.2 years; range, 8–39 years). Five patients (6.3%) with late-onset disease and 1 patient (1.4%) diagnosed at <40 years had low α-Gal activity. Of these 6 patients, 3 had angina, 4 were in New York Heart Association class 2, 5 had palpitations, and 2 had a history of syncope. Prevalence of Anderson-Fabry disease in male patients with late onset hypertrophic cardiomyopathy

Using wide-angle, phased-array, two-dimensional echocardiography, mitral leaflets and their annular attachments were recorded from a view close to the standard apical four-chamber view. The transducer was rotated and recordings were made at 30 degrees rotational intervals around the circumference of the mitral valve annulus. To reconstruct the annulus, the diameters (chords) from each rotational interval were arranged around a reference point. This was done for 12 times during the cardiac cycle. Annular areas were planimetered and circumferences measured. Correlation was good for areas reconstructed and measured by the same observer on separate occasions (r = 0.963) and by two different observers (r = 0.987). In 11 normal subjects the annular area index (area divided by body surface area) increased during diastole to a maximum of 3.8 +/- 0.7 cm2/m2 (mean +/- SD) in late diastole. There was presystolic followed by systolic narrowing to a minimum in midsystole. The mean reduction in area was 26 +/- 3%. The maximal annular circumference was 9.3 +/- 0.9 cm and the mean reduction in circumference was 13 +/- 3%. The overall motion pattern was similar to that reported in experimental studies in the dog. Mitral annular reconstruction may provide new information about normal and abnormal function of the mitral valve apparatus.

Background A warm-water bath (WWB) or sauna bath (SB) has generally been considered inappropriate for patients with severe congestive heart failure (CHF). However, a comprehensive investigation of the hemodynamic effects of thermal vasodilation in CHF has not been previously undertaken. 

Methods and Results To investigate the acute hemodynamic effects of thermal vasodilation in CHF, we studied 34 patients with chronic CHF (mean age, 58±14 years). Clinical stages were New York Heart Association functional class II in 2, III in 19, and IV in 13 patients. Mean ejection fraction was 25±9%. After a Swan-Ganz catheter was inserted via the right jugular vein, the patient had a WWB for 10 minutes at 41°C or an SB for 15 minutes at 60°C. Blood pressure, ECG, echo-Doppler, expiration gas, and intracardiac pressures were recorded before, during, and 30 minutes after each bath. Oxygen consumption increased mildly, pulmonary arterial blood temperature increased by 1.2°C, and heart rate increased by 20 to 25 beats per minute on average at the end of WWB or SB. Systolic blood pressure showed no significant change. Diastolic blood pressure decreased significantly during SB ( P <.01). Cardiac and stroke indexes increased and systemic vascular resistances decreased significantly during and after WWB and SB ( P <.01). Mean pulmonary artery, mean pulmonary capillary wedge, and mean right atrial pressures increased significantly during WWB ( P <.05) but decreased significantly during SB ( P <.05). These pressures decreased significantly from the control level after each bath ( P <.01). Mitral regurgitation associated with CHF decreased during and 30 minutes after each bath. Cardiac dimensions decreased and left ventricular ejection fraction increased significantly after WWB and SB. In an additional study, plasma norepinephrine increased significantly during SB in healthy control subjects and in patients with CHF and returned to control levels by 30 minutes after SB. 

Conclusions Hemodynamics improve after WWB or SB in patients with chronic CHF. This is attributable to the reduction in cardiac preload and afterload. Thus, thermal vasodilation can be applied with little risk if appropriately performed and may provide a new nonpharmacological therapy for CHF.
