HIGH levels of speech recognition have been achieved with a new sound processing strategy for multielectrode cochlear implants. A cochlear implant system consists of one or more implanted electrodes for direct electrical activation of the auditory nerve, an external speech processor that transforms a microphone input into stimuli for each electrode, and a transcutaneous (rf-link) or percutaneous (direct) connection between the processor and the electrodes. We report here the comparison of the new strategy and a standard clinical processor. The standard compressed analogue (CA) processor presented analogue waveforms simultaneously to all electrodes, whereas the new continuous interleaved sampling (CIS) strategy presented brief pulses to each electrode in a nonoverlapping sequence. Seven experienced implant users, selected for their excellent performance with the CA processor, participated as subjects. The new strategy produced large improvements in the scores of speech reception tests for all subjects. These results have important implications for the treatment of deafness and for minimal representations of speech at the auditory periphery.

Better speech recognition with cochlear implants.

Excellent models have been presented in the literature which relate membrane potential to transverse membrane current and which describe the propagation of action potentials along the axon, for both myelinated and nonmyelinated fibers. There is not, however, an adequate model for nerve excitation which allows one to compute the threshold of a nerve fiber for pulses of finite duration using electrodes that are not in direct contact with the fiber. This paper considers this problem and presents a model of the electrical properties of myelinated nerve which describes the time course of events following stimulus application up to the initiation of the action potential. The time-varying current and potential at all nodes can be computed from the model, and the strength-duration curve can be determined for arbitrary electrode geometries, although only the case of a monopolar electrode is considered in this paper. It is shown that even when the stimulus is a constant-current pulse, the membrane current at the nodes varies considerably with time. The strength-duration curve calculated from the model is consistent with previously published experimental data, and the model provides a quantitative relationship between threshold and fiber diameter which shows there is less selectivity among fibers of large diameter than those of small diameter.

Analysis of a Model for Excitation of Myelinated Nerve

This paper describes the development of a high-density electronic interface to the central nervous system. Silicon micromachined electrode arrays now permit the long-term monitoring of neural activity in vivo as well as the insertion of electronic signals into neural networks at the cellular level. Efforts to understand and engineer the biology of the implant/tissue interface are also underway. These electrode arrays are facilitating significant advances in our understanding of the nervous system, and merged with on-chip circuitry, signal processing, microfluidics, and wireless interfaces, they are forming the basis for a family of neural prostheses for the possible treatment of disorders such as blindness, deafness, paralysis, severe epilepsy, and Parkinson's disease.

Wireless implantable microsystems: high-density electronic interfaces to the nervous system

As the most successful neural prosthesis, cochlear implants have provided partial hearing to more than 120000 persons worldwide; half of which being pediatric users who are able to develop nearly normal language. Biomedical engineers have played a central role in the design, integration and evaluation of the cochlear implant system, but the overall success is a result of collaborative work with physiologists, psychologists, physicians, educators, and entrepreneurs. This review presents broad yet in-depth academic and industrial perspectives on the underlying research and ongoing development of cochlear implants. The introduction accounts for major events and advances in cochlear implants, including dynamic interplays among engineers, scientists, physicians, and policy makers. The review takes a system approach to address critical issues in cochlear implant research and development. First, the cochlear implant system design and specifications are laid out. Second, the design goals, principles, and methods of the subsystem components are identified from the external speech processor and radio frequency transmission link to the internal receiver, stimulator and electrode arrays. Third, system integration and functional evaluation are presented with respect to safety, reliability, and challenges facing the present and future cochlear implant designers and users. Finally, issues beyond cochlear implants are discussed to address treatment options for the entire spectrum of hearing impairment as well as to use the cochlear implant as a model to design and evaluate other similar neural prostheses such as vestibular and retinal implants.

Cochlear implants: a remarkable past and a brilliant future

Introduction THIS is basically a case report on long-term direct electrical stimulation of the auditory nerve in man. The report is supplemented by some historical facts, and by selected comments on electrophysiology, physical chemistry, and perceptual psychology. These comments are sometimes injected between segments of experimental observations and thereby tend to favor one interpretation of the data over others. To counterbalance that trend it was elected to describe as many observations as possible, even though some may seem redundant and others incomplete. This sometimes makes dull reading, but I hope it also makes this report as free from experimenter bias as can be achieved. The study was initially designed to answer questions raised by our own previous experience with electrical stimulation and by the writings of others. We especially planned to study two aspects of perception: (1) the resolution obtainable by discrete stimulation of several small populations of auditory nerve
