Successful metastasis depends on cell invasion, migration, host immune escape, extravasation, and angiogenesis. The process of cell invasion and migration relies on the dynamic changes taking place in the cytoskeletal components; actin, tubulin and intermediate filaments. This is possible due to the plasticity of the cytoskeleton and coordinated action of all the three, is crucial for the process of metastasis from the primary site. Changes in cellular architecture by internal clues will affect the cell functions leading to the formation of different protrusions like lamellipodia, filopodia, and invadopodia that help in cell migration eventually leading to metastasis, which is life threatening than the formation of neoplasms. Understanding the signaling mechanisms involved, will give a better insight of the changes during metastasis, which will eventually help targeting proteins for treatment resulting in reduced mortality and longer survival.

High clusterin (CLU) mRNA expression levels in tumors of colorectal cancer patients predict a poor prognostic outcome.

Gastrointestinal stromal tumors are the most common primary mesenchymal tumors of the gastrointestinal tract accounting for 0.1%-3.0% of all gastrointestinal malignancies. The stomach is the most common site (60%) followed by the small bowel (30%-35%) particularly jejunum and ileum, colorectum (5%) and rarely affect esophagus and appendix. Most gastrointestinal stromal tumors arise sporadically, however, less commonly they develop in association with various clinical syndromes like Carney triad, Carney-Stratakis syndrome, familial gastrointestinal stromal tumor syndrome and neurofibromatosis type1 (NF1). We report a 65-year-old male patient presented with gastric mass (7.5 × 4.5 × 3.5 cm) arising from the posterior gastric wall. Histologic examination revealed neoplastic proliferation of spindled and epithelioid cells with focal plexiform pattern and low mitotic activity 3/50 HPF. No evidence of cytological atypia, abnormal mitosis or necrosis. Interestingly enough, there were focal areas of mature bone formation/osseous differentiation associated with calcification. The tumor cells were strongly positive for CD117, DOG1 with focal immunoreactivity against CD34. The morphologic features and the immunoprofile were diagnostic of gastrointestinal stromal tumor. Herein, we present a rare case of gastric gastrointestinal stromal tumor with mature bone formation, osseous metaplasia and calcification. To the best of our knowledge, this is the second case report of gastric gastrointestinal stromal tumor with osseous differentiation and mature bone formation.

Gastric gastrointestinal stromal tumor with osseous differentiation and stromal calcification: A case report and review of literature.

BACKGROUND Desmoplastic small round cell tumor (DSRCT) is a rare lethal malignant tumor with young male predominance. The majority of cases arise in the abdominopelvic region and are hypothesized to have a mesothelial origin. However, extra-abdominal and extraperitoneal DSRCT have been reported. It is extremely uncommon for the pancreas to be a primary site for DSRCT, and only 5 cases have previously been reported in the English literature. Clinically, DSRCT has a wide range of presentations from asymptomatic to life-threatening comorbidity, and it responds poorly to treatment despite aggressive therapy. CASE REPORT We report a previously healthy 9-year-old boy with an incidentally discovered abdominal mass of pancreatic origin. All necessary laboratory investigations were within normal limits. Computed tomographic imaging showed a huge left-side retroperitoneal mass measuring 15 cm in the greatest dimension that was accompanied by vascular encasement. The mass was resected successfully. Histopathological examination along with ancillary tests favored a diagnosis of DSRCT over other small round blue cell tumors. Detection of translocation t(11;22)(p13;q12) with EWSR1-WT1 gene fusion, based on reverse transcription-polymerase chain reaction analysis, confirmed the diagnosis. Approximately 7 months later, the tumor recurred with mesenteric lymph nodes metastasis and the child was placed on palliative therapy. CONCLUSIONS It is worthwhile to consider DSRCT in the differential diagnosis of small round blue cell tumors, even in unusual sites, in a pediatric age group. Due to the poor prognosis, owing to chemotherapy resistance and a high rate of recurrence with significant tumor burden, reaching a precise diagnosis of DSRCT is essential. Almost all cases harbor the hallmark molecular alteration of t(11;22)(p13;q12) with EWSR1-WT1 gene fusion. Debulking surgery paired with a chemotherapy regimen comprising vincristine, doxorubicin, and cyclophosphamide and ifosfamide + etoposide has been shown to improve overall survival rate compared with other chemotherapeutic agents. However, no targeted therapeutic modality has been developed.

Desmoplastic Small Round Cell Tumor of Pancreatic Origin in a Young Child: A Case Report and Review of Literature.

Background Urinary bladder crothelial carcinoma (UCB) is the most common urinary bladder neoplasm. The present study aims at investigating immunostaining of fascin in UCB in relation to clinicopathologiccriteria in Saudi Arabia. Methods This study utilised 122 UCB and 25 apparently normal urothelium archival pathologic samples prior to local or systemic therapy. Tissue microarrays were constructed and the generated TMA blocks were used for Immunohistochemical staining. The mouse anti-fascin monoclonal antibody was used. A 25% was used to specify low and high fascin immunostaining. Results Fascin immunostaining was detected in UCB and apparently normal urothelium. High immunostaining was statistically less frequent than low fascin immunostaining (P≤0.001). In UCB, high fascin immunostaining was associated with older patients (P=0.005) and local disease recurrence (P=0.002). High fascin immunostaining was an independent predictor of local disease recurrence (P=0.002) and associated with poor overall survival (P=0.027). Conclusion High fascin immunostaining in UCB was associated with adverse prognostic factors and may be used as an independent prognostic marker. Fascin was detected in apparently normal urothelium and may contribute to UCB carcinogenesis. Further investigations (molecular and clinical) are required to understand the molecular interaction of fascin with UCB and its possible therapeutic applications.

Fascin expression in urinary bladder urothelial carcinoma correlates with unfavourable prognosis.

Rosai-Dorfman disease is a histiocytic lesion that affects lung rarely. We present a 52-year-old female diagnosed with right intrabronchial mass invading the bronchial wall and the extrabronchial tissues with lymphadenopathy. Multiple bronchoscopic biopsies were not diagnostic. Pneumonectomy was performed and postoperative histology revealed marked mucin impaction and bronchial dilatation. The pulmonary tissue showed areas of hemorrhage and chronic inflammation. The mass exhibited an excessive number of lymphocytes, plasma cells, and numerous histiocytes engulfing them (lymphocytophagocytosis). These histiocytes were S100 protein and CD68 positive. These features are consistent with Rosai-Dorfman disease. Rosai-Dorfman Disease with pulmonary affection can be misdiagnosed as malignancy. Careful histological examination of the specimen for emperipolesis or lymphocytophagocytosis together with S100 protein and CD68 positivity are the clue for proper diagnosis.

Rosai-Dorfman disease with pulmonary involvement mimicking bronchogenic carcinoma.

BACKGROUND/AIM Colorectal carcinoma (CRC) carries a high incidence of morbidity and mortality. Prognosis is related to nodal metastasis and stage. Clusterin is a widely distributed glycoprotein with not yet fully understood functions. Clusterin may be overexpressed in some tumours or under expressed in other tumours. The aim behind this study is to examine the relation of clusterin cytoplasmic immunostaining to tumour characteristics, disease relapse, and survival in CRC. MATERIALS AND METHODS Paraffin blocks of 133 CRCs were retrieved from the Department of Pathology, King Abdulaziz University, Jeddah, Saudi Arabia. Immunostaining was done using antibody to clusterin. Staining expression in 10% of malignant cells was used as a cut-off to determine low immunostaining and high immunostaining. Statistical tests were used to evaluate the association of clusterin immunostaining with clinicopathological parameters. RESULTS Immunohistochemical results showed clusterin low immunostaining in CRC and nodal metastases. No association was found between clusterin immunostaining and tumour grade, age, tumour invasiveness, distant metastases, vascular invasion, nodal metastases, relapse, and survival. CONCLUSION Our study showed low clusterin immunostaining in CRC with lack of association with prognostic indicators in CRC. These results raise the controversy of understanding the role of clusterin in CRC. Further molecular studies are required to explore more about possible mechanisms of clusterin association with tumorigenicity, apoptosis, tumour growth progression, local and vascular invasion, and metastasis of CRC.
