"The Seventh Report of the Joint National Committee on Prevention, Detection,
Evaluation, and Treatment of High Blood Pressure" provides a new guideline
for hypertension prevention and management. The following are the key messages(1) In persons older than 50 years, systolic blood pressure (BP) of
more than 140 mm Hg is a much more important cardiovascular disease
(CVD) risk factor than diastolic BP; (2) The risk of CVD, beginning at 115/75
mm Hg, doubles with each increment of 20/10 mm Hg; individuals who are normotensive
at 55 years of age have a 90% lifetime risk for developing hypertension; (3)
Individuals with a systolic BP of 120 to 139 mm Hg or a diastolic BP of 80
to 89 mm Hg should be considered as prehypertensive and require health-promoting
lifestyle modifications to prevent CVD; (4) Thiazide-type diuretics should
be used in drug treatment for most patients with uncomplicated hypertension,
either alone or combined with drugs from other classes. Certain high-risk
conditions are compelling indications for the initial use of other antihypertensive
drug classes (angiotensin-converting enzyme inhibitors, angiotensin-receptor
blockers, β-blockers, calcium channel blockers); (5) Most patients with
hypertension will require 2 or more antihypertensive medications to achieve
goal BP (<140/90 mm Hg, or <130/80 mm Hg for patients with diabetes
or chronic kidney disease); (6) If BP is more than 20/10 mm Hg above goal
BP, consideration should be given to initiating therapy with 2 agents, 1 of
which usually should be a thiazide-type diuretic; and (7) The most effective
therapy prescribed by the most careful clinician will control hypertension
only if patients are motivated. Motivation improves when patients have positive
experiences with and trust in the clinician. Empathy builds trust and is a
potent motivator. Finally, in presenting these guidelines, the committee recognizes
that the responsible physician's judgment remains paramount.

The National High Blood Pressure Education Program presents the complete Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure. Like its predecessors, the purpose is to provide an evidence-based approach to the prevention and management of hypertension. The key messages of this report are these: in those older than age 50, systolic blood pressure (BP) of greater than 140 mm Hg is a more important cardiovascular disease (CVD) risk factor than diastolic BP; beginning at 115/75 mm Hg, CVD risk doubles for each increment of 20/10 mm Hg; those who are normotensive at 55 years of age will have a 90% lifetime risk of developing hypertension; prehypertensive individuals (systolic BP 120-139 mm Hg or diastolic BP 80-89 mm Hg) require health-promoting lifestyle modifications to prevent the progressive rise in blood pressure and CVD; for uncomplicated hypertension, thiazide diuretic should be used in drug treatment for most, either alone or combined with drugs from other classes; this report delineates specific high-risk conditions that are compelling indications for the use of other antihypertensive drug classes (angiotensin-converting enzyme inhibitors, angiotensin-receptor blockers, beta-blockers, calcium channel blockers); two or more antihypertensive medications will be required to achieve goal BP (<140/90 mm Hg, or <130/80 mm Hg) for patients with diabetes and chronic kidney disease; for patients whose BP is more than 20 mm Hg above the systolic BP goal or more than 10 mm Hg above the diastolic BP goal, initiation of therapy using two agents, one of which usually will be a thiazide diuretic, should be considered; regardless of therapy or care, hypertension will be controlled only if patients are motivated to stay on their treatment plan. Positive experiences, trust in the clinician, and empathy improve patient motivation and satisfaction. This report serves as a guide, and the committee continues to recognize that the responsible physician's judgment remains paramount.

The Hypertension Primer, Essentials of High Blood Pressure , 2nd edition, released in November 1998, is a major professional, educational, and service effort of the Council for High Blood Pressure Research of the American Heart Association. The Primer is actually an educational experiment that addresses several problematic issues in information organization and dissemination that are likely to become even more problematic in the future. As with the first edition, which appeared in 1993, the work showcases the contributions of America’s foremost experts in the field of hypertension and is intended to serve as a unique introductory reference document in the rapidly expanding field of hypertension. The second edition is a further refinement of the process made possible by the work of the Council and an unrestricted educational grant from Astra Pharmaceuticals. Perhaps the greatest difference with the second edition is that the book will be distributed more effectively through a variety of channels to medical students, residents, fellows, and clinicians and will also be sold directly through the publishers, Lippincott-Williams & Wilkins.

The hypertension primer project : An educational experiment

This month's Hypertension Icon is Dr. Marvin Moser, Editor in Chief of The Journal of Clinical Hypertension. It is not, however, for this reason that he is being honored. For more than 50 years Dr. Moser has been a distinguished leader in efforts to control hypertension and prevent cardiovascular disease. He was there at the beginning when the treatment of hypertension was primitive and when people still believed that treatment would not be beneficial. He has participated in research on almost every new antibypertensive agent as they became available. He was one of the first to recognize that lowering blood pressure would result in the reversal of left ventricular hypertrophy. In addition to being an outstanding clinician, researcher, and teacher, Dr. Moser has been one of the leaders in public education. In his role as Senior Medical Consultant to the National High Blood Pressure Education Program, as well as in many states and national societies, he has been an untiring advocate of more effective treatment of hypertensive vascular disease. Dr. Moser has written 11 books, including four for the general public on the topic of hypertension and cardiovascular disease. The seventh edition of Clinical Management of Hypertension has just been published. He was a co-editor of the Yale University School of Medicine Heart Book. He is the author of more than 450 scientific papers and has lectured extensively in the United States and abroad. Dr. Moser is a worthy addition to the list of icons that have been featured in The Journal.

Cardiovascular disease is still the most common cause of morbidity and mortality in the elderly and hypertension is a significant risk factor for CVD such as stroke, myocardial infarction, and chronic renal disease. Fortunately, hypertension is readily treatable. A clear benefit from treating elevated blood pressure has been demonstrated by several clinical trials. In fact, benefit from treatment of either systolic or diastolic hypertension in this group is much greater than in younger hypertensives. Understanding physiologic and socioeconomic changes with age is an essential part of treatment and will allow for individualized treatment suitable for the elderly. Life-syle modification is a good starting point to control high blood pressure but should not be enforced unrealistically in this group. Once pharmacologic therapy is decided upon it should be started with a lower dose than usual and should be slowly titrated. Monotherapies and combination therapies are available for treatment of hypertension. All clinicians who take care of the geriatric population should be aware of the benefit of antihypertensive therapy since this is one of the most rewarding aspects of preventive medicine.

Hypertension In The Elderly.

Since their introduction more than 30 years ago, calcium antagonists have emerged as one of the most attractive and widely used classes of antihypertensive agents. Of the 20 to 25 million patients receiving medication for hypertension in the United States, about one quarter are taking calcium antagonists. The wide appeal of the calcium antagonists is attributable to several features, including efficacy in virtually all demographic groups, beneficial characteristics such as metabolic neutrality, and the occurrence of relatively few and primarily nuisance-type side effects.1 2 In addition, recent investigations have focused on their possible protective effects on target organs, such as the heart and kidney,3 4 further enhancing their value

Despite these attributes, a number of retrospective analyses have suggested that calcium antagonists may be detrimental and may promote adverse cardiovascular events. On the basis of this and other retrospective analyses, Pahor et al5 proposed that the use of calcium antagonists as first-line antihypertensive agents should be discontinued.

Although meta-analyses and observational studies clearly have limits, Pahor et al5 raised an important question that deserves consideration: whether calcium antagonists, as a group, promote adverse cardiovascular events, specifically coronary artery …
