TL;DR: Pancreaticoduodenectomy with regional lymphadenectomy is adequate for early-stage ampullary cancer but a dismal outcome can be predicted in patients with lymph node metastasis and pancreatic invasion, which can be used to guide individualized, risk-oriented adjuvant therapy.
