Allergic rhinitis is a symptomatic disorder of the nose induced after allergen exposure by an IgE-mediated inflammation of the membranes lining the nose. It is a global health problem that causes major illness and disability worldwide. Over 600 million patients from all countries, all ethnic groups and of all ages suffer from allergic rhinitis. It affects social life, sleep, school and work and its economic impact is substantial. Risk factors for allergic rhinitis are well identified. Indoor and outdoor allergens as well as occupational agents cause rhinitis and other allergic diseases. The role of indoor and outdoor pollution is probably very important, but has yet to be fully understood both for the occurrence of the disease and its manifestations.

In 1999, during the Allergic Rhinitis and its Impact on Asthma (ARIA) WHO workshop, the expert panel proposed a new classification for allergic rhinitis which was subdivided into 'intermittent' or 'persistent' disease.

This classification is now validated. The diagnosis of allergic rhinitis is often quite easy, but in some cases it may cause problems and many patients are still under-diagnosed, often because they do not perceive the symptoms of rhinitis as a disease impairing their social life, school and work.

The management of allergic rhinitis is well established and the ARIA expert panel based its recommendations on evidence using an extensive review of the literature available up to December 1999. The statements of evidence for the development of these guidelines followed WHO rules and were based on those of Shekelle et al. A large number of papers have been published since 2000 and are extensively reviewed in the 2008 Update using the same evidence-based system. Recommendations for the management of allergic rhinitis are similar in both the ARIA workshop report and the 2008 Update. In the future, the GRADE approach will be used, but is not yet available.

Another important aspect of the ARIA guidelines was to consider co-morbidities. Both allergic rhinitis and asthma are systemic inflammatory conditions and often co-exist in the same patients. In the 2008 Update, these links have been confirmed.

The ARIA document is not intended to be a standard-of-care document for individual countries. It is provided as a basis for physicians, health care professionals and organizations involved in the treatment of allergic rhinitis and asthma in various countries to facilitate the development of relevant local standard-of-care documents for patients.

Allergic Rhinitis and Its Impact on Asthma

Background: Allergic rhinitis represents a global health problem affecting 10% to 20% of the population. The Allergic Rhinitis and its Impact on Asthma (ARIA) guidelines have been widely used to treat the approximately 500 million affected patients globally. Objective: To develop explicit, unambiguous, and transparent clinical recommendations systematically for treatment of allergic rhinitis on the basis of current best evidence. Methods: The authors updated ARIA clinical recommendations in collaboration with Global Allergy and Asthma European Network following the approach suggested by the Grading of Recommendations Assessment, Development and Evaluation working group. Results: This article presents recommendations about the prevention of allergic diseases, the use of oral and topical medications, allergen specific immunotherapy, and complementary treatments in patients with allergic rhinitis as well as patients with both allergic rhinitis and asthma. The guideline panel developed evidence profiles for each recommendation and considered health benefits and harms, burden, patient preferences, and resource use, when appropriate, to formulate recommendations for patients, clinicians, and other health care professionals. Conclusion: These are the most recent and currently the most systematically and transparently developed recommendations about the treatment of allergic rhinitis in adults and children. Patients, clinicians, and policy makers are encouraged to use these recommendations in their daily practice and to support their decisions.

Indoor air quality and health

In parallel with changes in lifestyle over the last 50 years (sedentary living in warm houses with extensive furnishing and low ventilation rates), there has been a progressive increase in the prevalence and morbidity of asthma in many parts of the world. The increase has been in perennial rather than seasonal asthma, and a large proportion of the patients are sensitized to one or more of the allergens found predominantly inside houses, that is, indoor allergens. The Third International Workshop on Indoor Allergens and Asthma was designed to discuss recent progress in basic and clinical research in this area, to formulate recommendations for allergen-specific management of asthma, and to consider future research directions. As with the two previous workshops, discussion topics included biology; allergen immunochemistry; molecular biology and immune response; epidemiology of asthma; and the role of allergen avoidance, a, 2 Because of dramatic progress in recent years, the Third International Workshop was expanded to cover not only house dust mite allergens but also allergens from cat, dog, and cockroach, for which immunochemical and epidemiologic data are available. Over the past 5 years there have been significant advances in several areas of research on indoor allergens, including: (1) cloning and expression of recombinant allergens, 3-7 (2) analysis of T-cell responses to indoor allergens, derivation of T-cell clones, and analysis of T-cell epitope specificity and cytokine profiles, s, 9 (3) investigation of the dose-response relationship between exposure to mite, cat, and cockroach allergens and sensitization, 1°-13 and (4) epidemiologic studies on indoor allergens as risk factors for the symptoms of asthma and bronchial hyperreactivity (BHR)? 4-17 Better definition of the allergens has made it possible to analyze their structure and biologic function and to define epitopes recognized by antibodies or T cells. Information obtained from those studies has provided exciting possibilities for developing new vaccines for safe and effective immunotherapy. 9, 18. 19 Studies of T-cell responses to dust mites have confirmed the dominance of T-helper cell (Tin) responses in allergic individuals.

BACKGROUND: Various methods of killing the house dust mite to reduce exposure to allergen are being promoted even though complete data on their effects on allergen concentrations are not available. A study was designed to measure the concentrations of the main house dust mite allergen Der pI in homes treated with either solidified benzyl benzoate (Acarosan) or liquid nitrogen. METHODS: Der pI concentrations were measured in dust collected from mattresses, bedroom carpets, and living room carpets in 10 houses treated with Acarosan and 10 houses treated with liquid nitrogen. Samples were collected before the treatment (in July 1990) and three and six months afterwards (October 1990 and January 1991). Forty untreated houses were concurrently sampled as controls. RESULTS: Der pI concentrations were similar in the three groups at baseline. No significant fall was seen in either of the two treated groups three or six months after treatment. Concentrations in the control houses increased significantly--twofold to threefold in dust sampled from mattresses and bedroom carpets between baseline and October 1990. This increase was not seen in either of the treated groups of houses, but there was no significant difference in the Derp pI concentrations in these houses and the control houses from any site at any time point. CONCLUSIONS: Neither Acarosan nor liquid nitrogen reduced the concentrations of Der pI for as long as six months after application. A small effect was probably present as the rise seen in control houses in the three month samples was not found in the treated houses. This effect, however, is likely to be of little clinical importance and also to be transient as the trend was lost by six months.

BACKGROUND: House dust mite numbers and the concentration of its main allergen, Der p1, depend on ambient temperature and humidity and have been reported to show a seasonal variation in homes in the United States. A study was designed to find out whether Der p1 concentrations vary with the seasons in dust collected from homes in north west England. METHODS: The concentration of Der p1 was measured in dust, collected every three months from April 1990 to April 1991, from mattresses and from bedroom and living room carpets in 40 houses in the south Manchester area. Twenty four hour recordings of indoor relative humidity were made in 20 houses during the sampling day. RESULTS: Mean concentrations of Der p1 from all three sources rose two to three fold in October. This was in contrast to the 1000 fold variation in concentrations of Der p1 present between houses within each season's sample and less than the 10 fold change considered to be of clinical importance. The autumn increase was paralleled by a rise in humidity. There was no statistical correlation, however, between Der p1 concentrations and relative humidity, house type, ventilation, or double glazing. CONCLUSIONS: The results suggest that seasonal variations in exposure to Der p1 exposure in north west England are small and unlikely to be of major clinical importance. The temperature and humidity data showed that the indoor environment remained relatively constant and conducive to mite growth throughout the year.
