Background A regimen of epirubicin, cisplatin, and infused fluorouracil (ECF) improves survival among patients with incurable locally advanced or metastatic gastric adenocarcinoma. We assessed whether the addition of a perioperative regimen of ECF to surgery improves outcomes among patients with potentially curable gastric cancer. Methods We randomly assigned patients with resectable adenocarcinoma of the stomach, esophagogastric junction, or lower esophagus to either perioperative chemotherapy and surgery (250 patients) or surgery alone (253 patients). Chemotherapy consisted of three preoperative and three postoperative cycles of intravenous epirubicin (50 mg per square meter of body-surface area) and cisplatin (60 mg per square meter) on day 1, and a continuous intravenous infusion of fluorouracil (200 mg per square meter per day) for 21 days. The primary end point was overall survival. Results ECF-related adverse effects were similar to those previously reported among patients with advanced gastric cancer. Rates of postoperative complications were similar in the perioperative-chemotherapy group and the surgery group (46 percent and 45 percent, respectively), as were the numbers of deaths within 30 days after surgery. The resected tumors were significantly smaller and less advanced in the perioperative-chemotherapy group. With a median follow-up of four years, 149 patients in the perioperative-chemotherapy group and 170 in the surgery group had died. As compared with the surgery group, the perioperative-chemotherapy group had a higher likelihood of overall survival (hazard ratio for death, 0.75; 95 percent confidence interval, 0.60 to 0.93; P = 0.009; five-year survival rate, 36 percent vs. 23 percent) and of progression-free survival (hazard ratio for progression, 0.66; 95 percent confidence interval, 0.53 to 0.81; P<0.001). Conclusions In patients with operable gastric or lower esophageal adenocarcinomas, a perioperative regimen of ECF decreased tumor size and stage and significantly improved progression-free and overall survival. Sepsis is the leading cause of death in critically ill patients in the United States. Yet the individual host response to septicemia is variable, depending on the patient's immune response, age, nutritional status, and coexisting conditions, as well as on the virulence of the organism and the size of the inoculum. This review examines evolving concepts of sepsis and discusses new and potential therapies. Recent clinical advances include therapy with activated protein C, stringent control of blood glucose, and early goal-directed therapy to treat cellular oxygen deficit. Future therapies may be focused on modulating the immune response in the light of the characteristics of the specific pathogen, the genetic profile of the patient, and the duration of the disease.

The pathophysiology and treatment of sepsis.

The purpose of this article is to review the status and limitations of anatomic tumor response metrics including the World Health Organization (WHO) criteria, the Response Evaluation Criteria in Solid Tumors (RECIST), and RECIST 1.1. This article also reviews qualitative and quantitative approaches to metabolic tumor response assessment with 18 F-FDG PET and proposes a draft framework for PET Response Criteria in Solid Tumors (PERCIST), version 1.0. Methods: PubMed searches, including searches for the terms RECIST, positron, WHO, FDG, cancer (including specific types), treatment response, region of interest, and derivative references, were performed. Abstracts and articles judged most relevant to the goals of this report were reviewed with emphasis on limitations and strengths of the anatomic and PET approaches to treatment response assessment. On the basis of these data and the authors’ experience, draft criteria were formulated for PET tumor response to treatment. Results: Approximately 3,000 potentially relevant references were screened. In einer prospektiven randomisierten Studie wurde die Wirksamkeit der kontinuierlichen intraarteriellen Cytostatica-Therapie (FUDR) uber eine implantierbare Pumpe (n =19) mit der Bolusinjektion (n= 17) verglichen bei isolierten nicht resektablen Lebermetastasen. Es fand sich folgendes Ergebnis: Ansprechrate bei den Pumpen 58% vs. 41% bei den Ports. Progression bei kontinuierlicher Infusion 10,5% vs. 29,5% bei Bolusinjektion. Die Toxicitat war bei kontinuierlicher Cytostatica-Applikation mit 58% gegenuber 23,5% deutlich erhoht.

228. Intraarterielle Chemotherapie bei Lebermetastasen — Bolusinjektion vs. kontinuierliche Infusion

Peptische Ulzerationen bedingt durch ein Zollinger-Ellison-Syndrom sind eine seltene Entitat. In diesem Fallbericht wird uber einen 55-jahrigen Patienten berichtet, der eine 10-jahrige Anamnese von Refluxbeschwerden, Erbrechen und Diarrhoen beklagte. Eine vollstandige subjektive Beschwerdefreiheit trat trotz der kontinuierlichen Einnahme eines Protonenpumpeninhibitors (PPI) nie ein. Die Diagnose eines gastrinombedingten Zollinger-Ellison-Syndroms wurde zunachst nicht in Erwagung gezogen. Nach kurzzeitigem Absetzen der PPI-Therapie fur wenige Tage kam es aufgrund der starken Stimulation durch das Gastrinom zu einer fulminanten Saureveratzung des distalen Osophagus mit iatrogener oder spontaner Perforation am osophagogastralen Ubergang. Es wird das operative Therapievorgehen mit zweizeitiger Rekonstruktion durch ein Koloninterponat und Entfernung des Primartumors im Duodenum dargestellt.

German hospitals and surgical clinics/departments are facing far-reaching changes. One triggering factor is the imminent reorganization of hospital financing to a system of compensation, which is universally based on diagnosis-related groups (DRGs) and entails a market-economy orientation in the hospital sector. Digital technologies, which facilitate making the necessary adjustments to clinic structures to meet forthcoming challenges, represent another element. The "digital transformation" of the hospital of the future takes place on three levels. The restructuring of the surgical realm runs rather a traditional course by increasing use of information technology, mostly to optimize documentation and existing procedures or to reduce costs. The second sphere reaches substantially further, encompassing reorganization of disease-oriented cooperation between the different medical specialties and enabling the establishment of suitably structured disease-oriented medical centers. This is followed by the third phase, which involves networking clinics or medical centers with private practitioners, aftercare and rehabilitation services, and other disease-oriented care providers.

Die Vagotomie wird auch unter dem Aspekt eines minimalisierten Zugangs keine Renaissance mehr erfahren. Das Bessere ist der Feind des Guten. Daran ist der Helicobacter pylori schuld, der das pathogenetische Prinzip 1Säurea bei der Ulcuskrankheit in den Hintergrund gedrängt hat zugunsten der von ihm ausgelösten Infektion der Magenschleimhaut. Diese Helicobacter-pylori-induzierte Gastritis ist für praktisch alle Ulcera duodeni und für ca. 80% der Ulcera ventriculi verantwortlich. Keine Frage, die Chirurgie hat in diesem Konzept der Ulcuskrankheit praktisch keinen Stellenwert mehr ± mit Ausnahme der Ulcuskomplikationen. Es wird sehr interessant sein zu beobachten, ob die Eradikation des Helicobacter pylori und damit die prophylaktische Behandlung der Gastritis auf die Dauer auch zu einer Veränderung der Epidemiologie bzw. des Spontanverlaufs der Ulcuskrankheit führen wird, und damit zu einer Abnahme der Häufigkeit von Ulcuskomplikationen. Dann würde die Chirurgie auch hier an Bedeutung verlieren. Der Helicobacter pylori ist nicht nur daran schuld, daû der Chirurgie die Bastion 1Ulcuschirurgiea verloren gegangen ist, er steht auch im Verdacht in der Genese des Magencarcinoms entscheidend beteiligt zu sein. Nach allem, was wir heute wissen, spielt er allerdings nur beim überwiegend distal gelegenen intestinalen Magencarcinom eine Rolle. Die höher gelegenen Magencarcinome sind ebenso wie das Kardiacarcinom und das Adenocarcinom des distalen Oesophagus nicht Helicobacter-pylori-assoziiert. Im Gegenteil, die Eradikation des Helicobacter pylori könnte zu einer Zunahme der Refluxkrankheit führen und damit möglicherweise zum Schrittmacher der malignen Degeneration im Barrett-Oesophagus werden. Diese Hypothese muû dringend weiter verfolgt und auf ihre Richtigkeit hin überprüft werden. Insgesamt wäre von einer prophylaktischen Eradikation des Helicobacter pylori ± abgesehen von diesem Problem eines möglichen Schadens ± bestenfalls eine Abnahme der Antrumcarcinome vom intestinalen Typ zu erwarten. Wenn dem so ist, würde der Helicobacter pylori dafür sorgen, daû die Chirurgie des Magencarcinoms künftig schwieriger und aufwendiger wird. Auch hier erweist sich der Helicobacter pylori nicht gerade als Freund des Chirurgen. Bleibt noch seine Rolle in der Genese des Lymphoms. Die Hypothese steht ± ohne Helicobacter pylori kein Lymphom. In diese Richtung deutet auch, daû zumindest das frühe Lymphom nach Eradikation des Helicobacter pylori reversibel ist. Dagegen benötigen fortgeschrittene Lymphome des Magens derzeit noch der Operation. Wer hätte noch vor 10 Jahren gedacht, welche revolutionäre 3⁄4nderung unser Wissen um die Ulcuskrankheit innerhalb kürzester Zeit erfahren würde? Ganze, scheinbar wohlbelegte und unerschütterliche Grundsätze der Ulcuschirurgie sind über Nacht zusammengebrochen. Arbeiten aus den 70er Jahren zu diesem Thema erscheinen heute anachronistisch. Wer weiû, wie lange unser heute so gesichert erscheinendes Wissen Gültigkeit behalten wird?
