Magnetic resonance classification of lumbar intervertebral disc degeneration.

Herniated intervertebral disc tissue has been shown to produce a number of proinflammatory mediators and cytokines, but there have been no similar studies using discs from patients with discogenic low back pain. We have compared the levels of production of interleukin-6 (IL-6), interleukin-8 (IL-8) and prostaglandin E2 (PGE2) in disc tissue from patients undergoing discectomy for sciatica (63) with that from patients undergoing fusion for discogenic low back pain (20) using an enzyme-linked immunoabsorbent assay. There was a statistically significant difference between levels of production of IL-6 and IL-8 in the sciatica and low back pain groups (p < 0.006 and p < 0.003, respectively). The high levels of proinflammatory mediator found in disc tissue from patients undergoing fusion suggest that production of proinflammatory mediators within the nucleus pulposus may be a major factor in the genesis of a painful lumbar disc.

The prevalence, validity and reliability of high-intensity zones in the annulus fibrosus seen on T2-weighted magnetic resonance images of patients with intractable low-back pain were determined. This sign was readily recognized by two independent observers. It occurred in 28% of 500 patients undergoing magnetic resonance imaging for back pain. The presence of a highintensity zone correlated significantly with the presence of Grade 4 annular disruption and with reproduction of the patient's pain. Its sensitivity as a sign of either annular disruption or pain was modest but its specificity was high, and its positive predictive value for a severely disrupted, symptomatic disc was 86%. This sign is diagnostic of painful internal disc disruption.

High-intensity zone: a diagnostic sign of painful lumbar disc on magnetic resonance imaging.

Objective
Degenerative intervertebral disc disease is common; however, the importance of genetic factors is unknown. This study sought to determine the extent of genetic influences on disc degeneration by classic twin study methods using magnetic resonance imaging (MRI).

Methods
We compared MRI features of degenerative disc disease in the cervical and lumbar spine of 172 monozygotic and 154 dizygotic twins (mean age 51.7 and 54.4, respectively) who were unselected for back pain or disc disease. An overall score for disc degeneration was calculated as the sum of the grades for disc height, bulge, osteophytosis, and signal intensity at each level. A “severe disease” score (excluding minor grades) and an “extent of disease” score (number of levels affected) were also calculated.

Results
For the overall score, heritability was 74% (95% confidence interval [95% CI] 64–81%) at the lumbar spine and 73% (95% CI 64–80%) at the cervical spine. For “severe disease,” heritability was 64% and 79% at the lumbar and cervical spine, respectively, and for “extent of disease,” heritability was 63% and 63%, respectively. These results were adjusted for age, weight, height, smoking, occupational manual work, and exercise. Examination of individual features revealed that disc height and bulge were highly heritable at both sites, and osteophytes were heritable in the lumbar spine.

Conclusion
These results suggest an important genetic influence on variation in intervertebral disc degeneration. However, variation in disc signal is largely influenced by environmental factors shared by twins. The use of MRI scans to determine the phenotype in family and population studies should allow a better understanding of disease mechanisms and the identification of the genes involved.

Genetic influences on cervical and lumbar disc degeneration: A magnetic resonance imaging study in twins

The authors reviewed magnetic resonance (MR) images of 474 consecutive patients referred for lumbar spine MR imaging. Type 1 changes (decreased signal intensity on T1-weighted spin-echo images and increased signal intensity on T2-weighted images) were identified in 20 patients (4%) and type 2 (increased signal intensity on T1-weighted images and isointense or slightly increased signal intensity on T2-weighted images) in 77 patients (16%). In all cases there was evidence of associated degenerative disk disease at the level of involvement. Histopathologic sections in three cases of type 1 change demonstrated disruption and fissuring of the end plates and vascularized fibrous tissue, while in three cases of type 2 change they demonstrated yellow marrow replacement. Thirty patients with failed back surgery syndrome were studied to evaluate the effectiveness of magnetic resonance (MR) imaging with gadolinium-diethylenetriaminepentaacetic acid/dimeglumine (Gd-DTPA) in differentiating postoperative epidural fibrosis (scar) from recurrent disk herniation. Pre- and postcontrast MR images were interpreted without access to other diagnostic, surgical, or pathologic findings. Seventeen patients had surgical and pathologic correlation of the MR findings at 19 disk levels. The precontrast studies had a sensitivity, specificity, and accuracy of 100%, 71%, and 89%, respectively. The enhanced MR studies correctly depicted the character of abnormal epidural soft tissue in 17 patients at all 19 levels. Lumbar spine: postoperative MR imaging with Gd-DTPA.

Vertebral hemangiomas, unlike most bone lesions, show increased signal on T1- and T2-weighted magnetic resonance (MR) images. To define the basis for these signal characteristics, a retrospective review was done of the MR imaging findings in ten vertebral hemangiomas (eight patients), and these were correlated with the findings from plain radiographic, computed tomographic (CT), and histopathologic studies. MR images showed mottled increased signal in T1- and T2-weighted images from the osseous portions of the tumors. In three patients, the extraosseous components failed to show increased signal on T1-weighted images. Chemical shift images and histologic studies demonstrated that adipose tissue caused the increased signal on T1-weighted images. The extraosseous components of the tumor contained little, if any, adipose tissue, which explained the lack of high-intensity signal on T1-weighted images. These signal changes appear to make a specific constellation of findings for the diagnosis of vertebral hemangioma with MR imaging.

To define the time course and mechanism of enhancement of epidural fibrosis after gadolinium-DTPA (Gd-DTPA) injection, we undertook a three-part study in humans and dogs with epidural scar after spine surgery. First, the dynamic in vivo contrast-enhancing properties of epidural scar were assessed by using sequential fast (18-sec) spin-echo sequences after contrast injection. Epidural scar in dogs rapidly enhanced; peak enhancement (101%) was 6 min after injection, with a slower decline toward baseline to 45% after 44 min. Epidural fibrosis in patients followed a similar pattern, with a maximum enhancement of 73% after 5 min. Paraspinal muscle had a lower peak enhancement in both patients (36%) and dogs (22%). Second, vascular injection in two dogs with India ink demonstrated multiple small vessels throughout the epidural scar. Third, light and electron microscopy was performed on epidural scar obtained at reoperation in both patients and dogs. Light microscopy showed multiple small capillaries scattered throughout a background of collagen. Electron microscopy demonstrated a wide variation in the junctions between endothelial cells ranging from "tight" to "loose." Regions of endothelial discontinuity were also visualized. This study suggests that Gd-DTPA diffuses rapidly into the extravascular space in epidural scar, with a slower, net movement toward the intravascular compartment as the agent is renally filtered. The contrast agent transgresses the endothelium through "leaky" intercellular junctions and areas of endothelial discontinuity.
