Perioperative chemotherapy with FOLFOX4 and surgery versus surgery alone for resectable liver metastases from colorectal cancer (EORTC Intergroup trial 40983): a randomised controlled trial

A long-standing association exists between elevated triglyceride levels and cardiovascular disease* (CVD).1,2 However, the extent to which triglycerides directly promote CVD or represent a biomarker of risk has been debated for 3 decades.3 To this end, 2 National Institutes of Health consensus conferences evaluated the evidentiary role of triglycerides in cardiovascular risk assessment and provided therapeutic recommendations for hypertriglyceridemic states.4,5 Since 1993, additional insights have been made vis-a-vis the atherogenicity of triglyceride-rich lipoproteins (TRLs; ie, chylomicrons and very low-density lipoproteins), genetic and metabolic regulators of triglyceride metabolism, and classification and treatment of hypertriglyceridemia. It is especially disconcerting that in the United States, mean triglyceride levels have risen since 1976, in concert with the growing epidemic of obesity, insulin resistance (IR), and type 2 diabetes mellitus (T2DM).6,7 In contrast, mean low-density lipoprotein cholesterol (LDL-C) levels have receded.7 Therefore, the purpose of this scientific statement is to update clinicians on the increasingly crucial role of triglycerides in the evaluation and management of CVD risk and highlight approaches aimed at minimizing the adverse public health–related consequences associated with hypertriglyceridemic states. This statement will complement recent American Heart Association scientific statements on childhood and adolescent obesity8 and dietary sugar intake9 by emphasizing effective lifestyle strategies designed to lower triglyceride levels and improve overall cardiometabolic health. It is not intended to serve as a specific guideline but will be of value to the Adult Treatment Panel IV (ATP IV) of the National Cholesterol Education Program, from which evidence-based guidelines will ensue. Topics to be addressed include epidemiology and CVD risk, ethnic and racial differences, metabolic determinants, genetic and family determinants, risk factor correlates, and effects related to nutrition, physical activity, and lipid medications.

Triglycerides and Cardiovascular Disease A Scientific Statement From the American Heart Association

Background—The Air Force/Texas Coronary Atherosclerosis Prevention Study (AFCAPS/TexCAPS) is the first primary-prevention study in a cohort with average total cholesterol (TC) and LDL cholesterol (LDL-C) and below-average HDL cholesterol (HDL-C). Treatment with lovastatin (20 to 40 mg/d) resulted in a 25% reduction in LDL-C and a 6% increase in HDL-C, as well as a 37% reduction in risk for first acute major coronary event (AMCE), defined as fatal or nonfatal myocardial infarction, unstable angina, or sudden cardiac death. This article describes the relation between baseline and on-treatment lipid and apolipoprotein (apo) parameters and subsequent risk for AMCEs. Performing planned or unplanned interim analyses on accumulating data in clinical trials is a frequent practice. In this paper, we propose a general one-parameter family of type I error probability spending functions to construct customized group sequential boundaries with unequal increments in information time. This proposed family generalized the spending functions of Lan and DeMets1 and Kim and DeMets.2 We give an example to illustrate the use of this family.

These studies describe and evaluate an instructional program designed to help students with learning disabilities learn about the concept of theme, identify themes in stories, and apply themes to real life. Subjects were fifth- and sixth-grade nondisabled students and students with mild learning disabilities (Study 1) and seventh and eighth graders with more severe disabilities (Study 2). In both studies, the program improved comprehension of theme and identification of a specific theme that had been used in instruction. Instructed students were more successful on both measures than students who received no instruction (Study 1) or traditional instruction (Study 2). Applying a theme to real-life situations and identifying and applying themes not included in instruction were more difficult tasks—especially for Study 2 students. An Instructional Program in Comprehension of Narrative Themes for Adolescents with Learning Disabilities

Under reasonable assumptions a formula is derived to project the number of citations the usual type of scientific research paper will receive. This projection, the paper's life-time-citation rate, is based on the paper's past citations and the regularity of citation patterns. Hypothesis tests of equality of expected lifetime-citation rates per paper and multiple comparisons are also derived. With appropriate precautions comparisons may then be made of the work of scientists in similar fields who have been publishing for different lengths of time. The method is applied to the publications of members of a university department and validated in two ways.
