The Efficacy of Bone Replacement Grafts in the Treatment of Periodontal Osseous Defects. A Systematic Review

Biology of bone transplantation.

The present study was undertaken to determine the effect of four periodontal regenerative procedures on the connective tissue attachment level. The procedures tested were: 1) the modified Widman flap procedure, 2) the modified Widman flap procedure combined with transplantation of previously frozen autogenous red marrow and cancellous bone, 3) the modified Widman flap procedure in combination with implantation of beta tricalcium phosphate, and 4) periodic root planing and soft tissue curettage. Eight adult Rhesus monkeys, divided into four equal groups, were used. Periodontal pockets were produced around contralateral teeth in a standardized manner. In each group of animals, the pockets on one side of the jaws were subjected to one of the above-mentioned surgical treatments, while the contralateral pockets remained as unoperated controls. Three weeks before surgery, a carefully designed plaque control program was instituted and continued until the animals were sacrificed 12 months after surgery. In histologic sections, linear measurements along the root surfaces were made from the cemento-enamel junction (CEJ) to the most apical cells of the junctional epithelium (JE). These measurements from operated and unoperated sites were then compared. The data revealed that healing following the four different regenerative procedures resulted in the reformation of an epithelial lining (long junctional epithelium) along the treated root surfaces, with no new connective tissue attachment.

Histometric evaluation of periodontal surgery. II. Connective tissue attachment levels after four regenerative procedures

The purpose of this study was to test the bone-forming capacity of demineralized freeze-dried bone (DFDBA) and autologous bone grafts in extraction sockets. Seven paired sites were grafted with either DFDBA or autologous bone. The sites were reentered between 3 and 13 months for the purposes of obtaining biopsies of the grafted sites and to place endosseous implants. Biopsies from 6 of the 7 grafted sites were evaluated for new bone formation. DFDBA sites revealed the presence of dead particles of DFDBA with no evidence of bone formation on the surfaces of the implanted particles and no evidence of osteoclastic resorption of the bone particles. Biopsies from the 6 autologous sites revealed vascular channels with woven and lamellar bone. Some specimens had retained cortical, non-vital bone chips. These bone chips were undergoing active osteoclastic resorption. The results of this study questions the use of DFDBA as a bone inductive graft material. J Periodontol 1994;65:1128–1133.

A Comparison of Demineralized Freeze‐Dried Bone and Autologous Bone to Induce Bone Formation in Human Extraction Sockets

Enhanced bone formation by controlled growth factor delivery from chitosan-based biomaterials.

Freeze-dried cortical bone allografts of a fine particle size were implanted into wide three-wall, two-wall, one-wall, combination, and furcation defects. Of the 97 defects treated, 23 manifested complete bone regeneration; 39 showed greater than 50%; and 24, less than 50% osseous repair. Twelve defects failed to demonstrate any bony regeneration, of which nine were furcation involvements. From the preliminary data available, there is strong evidence which indicates that freeze-dried bone allografts have definite potential as grafting material in certain periodontal osseous defects. However, final determination must await the outcome of a larger number of cases and also histologic evidence.

Clinical Evaluation of Freeze-dried Bone Allografts in Periodontal Osseous Defects*, †

The excisional new attachment procedure was used to treat 75 suprabony pockets on 32 teeth in 9 patients. One-year postoperative measurements demonstrated an overall mean pocket reduction from 4.7 mm to 2.0 mm, of which 2.1 mm (77%) was new attachment and 0.6 mm was recession. Average amount of new attachment and percentage of new attachment were greatest on the midlingual surfaces of the teeth treated in this study. The results of this study showed the excisional new attachment procedure to be a simple, effective, and predictable procedure for achieving pocket elimination by means of a clinical new attachment of the gingival tissues in suprabony pockets.
