In recent times, coronary artery disease (CAD) has become one of the leading causes of morbidity and mortality across the globe. Diagnosing the presence and severity of CAD in individuals is essential for choosing the best course of treatment. Presently, computed tomography (CT) provides high spatial resolution images of the heart and coronary arteries in a short period. On the other hand, there are many challenges in analyzing cardiac CT scans for signs of CAD. Research studies apply machine learning (ML) for high accuracy and consistent performance to overcome the limitations. It allows excellent visualization of the coronary arteries with high spatial resolution. Convolutional neural networks (CNN) are widely applied in medical image processing to identify diseases. However, there is a demand for efficient feature extraction to enhance the performance of ML techniques. The feature extraction process is one of the factors in improving ML techniques’ efficiency. Thus, the study intends to develop a method to detect CAD from CT angiography images. It proposes a feature extraction method and a CNN model for detecting the CAD in minimum time with optimal accuracy. Two datasets are utilized to evaluate the performance of the proposed model. The present work is unique in applying a feature extraction model with CNN for CAD detection. The experimental analysis shows that the proposed method achieves 99.2% and 98.73% prediction accuracy, with F1 scores of 98.95 and 98.82 for benchmark datasets. In addition, the outcome suggests that the proposed CNN model achieves the area under the receiver operating characteristic and precision-recall curve of 0.92 and 0.96, 0.91 and 0.90 for datasets 1 and 2, respectively. The findings highlight that the performance of the proposed feature extraction and CNN model is superior to the existing models.

Detecting Coronary Artery Disease from Computed Tomography Images Using a Deep Learning Technique

In biomedical image analysis, information about the location and appearance of tumors and lesions is indispensable to aid doctors in treating and identifying the severity of diseases. Therefore, it is essential to segment the tumors and lesions. MRI, CT, PET, ultrasound, and X-ray are the different imaging systems to obtain this information. The well-known semantic segmentation technique is used in medical image analysis to identify and label regions of images. The semantic segmentation aims to divide the images into regions with comparable characteristics, including intensity, homogeneity, and texture. UNET is the deep learning network that segments the critical features. However, UNETs basic architecture cannot accurately segment complex MRI images. This review introduces the modified and improved models of UNET suitable for increasing segmentation accuracy.

Review of Semantic Segmentation of Medical Images Using Modified Architectures of UNET

Optimization of FFR prediction algorithm for gray zone by hemodynamic features with synthetic model and biometric data

Biomedical image segmentation (BIS) task is challenging due to the variations in organ types, position, shape, size, scale, orientation, and image contrast. Conventional methods lack accurate and automated designs. Artificial intelligence (AI)-based UNet has recently dominated BIS. This is the first review of its kind that microscopically addressed UNet types by complexity, stratification of UNet by its components, addressing UNet in vascular vs. non-vascular framework, the key to segmentation challenge vs. UNet-based architecture, and finally interfacing the three facets of AI, the pruning, the explainable AI (XAI), and the AI-bias. PRISMA was used to select 267 UNet-based studies. Five classes were identified and labeled as conventional UNet, superior UNet, attention-channel UNet, hybrid UNet, and ensemble UNet. We discovered 81 variations of UNet by considering six kinds of components, namely encoder, decoder, skip connection, bridge network, loss function, and their combination. Vascular vs. non-vascular UNet architecture was compared. AP(ai)Bias 2.0-UNet was identified in these UNet classes based on (i) attributes of UNet architecture and its performance, (ii) explainable AI (XAI), and, (iii) pruning (compression). Five bias methods such as (i) ranking, (ii) radial, (iii) regional area, (iv) PROBAST, and (v) ROBINS-I were applied and compared using a Venn diagram. Vascular and non-vascular UNet systems dominated with sUNet classes with attention. Most of the studies suffered from a low interest in XAI and pruning strategies. None of the UNet models qualified to be bias-free. There is a need to move from paper-to-practice paradigms for clinical evaluation and settings.

Early detection and diagnosis of coronary artery disease could reduce the risk of developing a heart attack. The coronary arteries are optimally visualised using computed tomography coronary angiography (CTCA) imaging. These images are reviewed by specialist radiologists who evaluate the coronary arteries for potential narrowing. A lack of radiologists in the UK is a constraint to timely diagnosis of coronary artery disease, particularly in the acute accident and emergency department setting. The development of automated methods by which coronary artery narrowing can be identified rapidly and accurately are therefore timely. Such complex computer based tools also need to be sufficiently computationally efficient that they can run on servers typically found in hospital settings, where graphical processing units for example are unavailable. We propose a fully automatic two-dimensional Unet model to segment the aorta and coronary arteries on CTCA images. Two models are trained to segment two regions of interest, (1) the aorta and the coronary arteries or (2) the coronary arteries alone. Our method achieves 91.20% and 88.80% dice similarity coefficient accuracy on regions of interest 1 and 2 respectively. Compared with a semi-automatic segmentation method, our model performs better when segmenting the coronary arteries alone. The performance of the proposed method is comparable to existing published two-dimensional or three-dimensional deep learning models. Importantly, the algorithmic and graphical processing unit memory efficiencies are maintained such that the model can be deployed without requiring graphical processing units, and therefore can be used in a hospital setting.
