Clinical Practice Guideline Otitis Media with Effusion (Update)

Studies of the occurrence of acute otitis media (AOM) in representative populations have con­ firmed earlier studies indicating that the highest at­ tack rates for otitis media are in the first 2 years of hfe. For example, in a population of 147,000 per­ sons in Finland,' the annual incidence of acute otitis media was 25% in children under 10 and 51% in 1-year-old children. An even higher rate of 75.5 at­ tacks per 100 child-years was found in the age group 6 to II months. In Sweden, a long-term prospective study that included children born since 1977 in a population of 235,000 inhabitants has identified a similar age distribution,^ as has a study in the United States.^ The reasons for this age distribution are still incompletely defined. However, a strong association of clinically diagnosed otitis media and infection by respiratory viruses has recently been re­ ported in a population-based study in the United States.\" The most important associated agents were respiratory syncytial and influenza viruses. Infant feeding practices have also shown an association with AOM; bottle feeding and the practice of feed­ ing in the supine position are potentially deleterious factors.\" Recent studies have not added greatly to our understanding of the epidemiology of otitis me­ dia in such high-risk groups as American Indians, Eskimos, and Australian aborigines.
