Engineering biocompatible implant surfaces

With an ageing population, war, and sports related injuries there is an ever-expanding requirement for hard tissue replacement such as bone. Engineered artificial scaffold biomaterials with appropriate mechanical properties, surface chemistry and surface topography are in a great demand for enhancing cell attachment, cell growth and tissue formation at such defect sites. Most of these engineering techniques are aimed at mimicking the natural organization of the bone tissues and thereby create a conducive environment for bone regeneration. As the interaction between the cells and tissues with biomaterials at the tissue–implant interface is a surface phenomenon, surface properties play a major role in determining both the biological response to implants and the material response to the physiological condition. Hence surface engineering of biomaterials is aimed at modifying the material and biological responses through changes in surface properties while still maintaining the bulk mechanical properties of the implant. Therefore, there has been a great thrust towards development of Ca–P-based surface coatings on various metallic and nonmetallic substrates for load bearing implant applications such as hip joint prosthesis, knee joint prosthesis and dental implants. Typical coating methodologies like ion beam assisted deposition, plasma spray deposition, pulsed laser physical vapor deposition, magnetron sputtering, sol–gel derived coatings, electrodeposition, micro-arc oxidation and laser deposition are extensively studied at laboratory scale. In the present article, attempts are made to give an overview of the basic principles behind the coating techniques as well as advantageous features such as bioactivity and biocompatibility associated with these coatings. A strong emphasis will be given on laser-induced textured and bioactive coatings obtained by the author's research group [A. Kurella, N.B. Since cells are sensitive to topographical features ranging from mesoscale to nanoscale, formation of these features by both pulsed and continuous wave Nd:YAG laser system will be highlighted. This can also be regarded as advancement towards third generation biomaterials which are bioinert, bioactive and which once implanted will stimulate cell adhesion, proliferation and growth at the interface. Further, an overview of various bio-implants and bio-devices and materials used for these kinds of devices, performance factors such as mechanical and corrosion behavior and surface science associated with these materials are also explained. As the present article is aimed at describing the multidisciplinary nature of this exciting field it also provides a common platform to understand this subject in a simple way for students, researchers, teachers and engineers in the fields ranging from medicine, dentistry, biology, materials science, biomedicine, biomechanics to physics.

Corrosion fatigue of biomedical metallic alloys: Mechanisms and mitigation

Great deal of research is still going on in the field of orthopedic and craniofacial implant development to resolve various issues being faced by the industry today. Despite several disadvantages of the metallic implants, they continue to be used, primarily because of their superior mechanical properties. In order to minimize the harmful effects of the metallic implants and its by-products, several modifications are being made to these materials, for instance nickel-free stainless steel, cobalt-chromium and titanium alloys are being introduced to eliminate the toxic effects of nickel being released from the alloys, introduce metallic implants with lower modulus, reduce the cost of these alloys by replacing rare elements with less expensive elements etc. New alloys like tantalum, niobium, zirconium, and magnesium are receiving attention given their satisfying mechanical and biological properties. Non-oxide ceramics like silicon nitride and silicon carbide are being currently developed as a promising implant material possessing a combination of properties such as good wear and corrosion resistance, increased ductility, good fracture and creep resistance, and relatively high hardness in comparison to alumina. Polymer/magnesium composites are being developed to improve mechanical properties as well as retain polymer's property of degradation. Recent advances in orthobiologics are proving interesting as well. This paper thus deals with the latest improvements being made to the existing implant materials and includes new materials being introduced in the field of biomaterials.

Microstructural characterization of as-cast biocompatible Co―Cr―Mo alloys

Both dendritic structure and equiaxed grain structure are produced in cast Co-Cr-Mo alloy by control of the cooling rate of castings during the solidification process, to determine whether a significant improvement in the mechanical properties of the alloy can be obtained. The different structural characteristics of the two kinds of casting are examined by optical microscopy, scanning electron microscopy and transmission electron microscopy. Tensile and fatigue tests as well as hardness measurement are carried out using individually cast test-pieces. Fracture surface appearance characteristics of tensile and fatigue specimens are also studied. It is concluded that the mechanical properties, including both transient and permanent properties, of the equiaxed grain structure castings obtained by fast cooling are superior to those found in the coarse dendritic structure castings.

Effects of cooling rate control during the solidification process on the microstructure and mechanical properties of cast Co-Cr-Mo alloy used for surgical implants

The composition of the Co-Cr-Mo alloy has been modified by additions of nickel and some trace elements aluminium, titanium and boron. In this paper, the first part of the present study, the effects of alloy additions on the microstructures and tensile properties of the as-cast Co-Cr-Mo alloy are discussed. The effects of alloy additions on the fatigue behaviour of the alloy will be discussed in the second part of the present study. It is found that alloy additions do not seem to result in changes in the nature of the casting structure. A directional, coarse dendritic cast structure is produced in the modified alloys as in the base alloY. However, the alloy additions affect the carbide precipitations and the formation of some fine details of the microstructure such as dislocations, stacking faults and twins produced during the solidification. A considerable improvement in the transient mechanical properties, especially in the tensile ductility, is achieved by modifying the base alloy with alloy additions.

Effects of alloy additions on the microstructures and tensile properties of cast Co-Cr-Mo alloy used for surgical implants

As the second part of the present study, the effects of modifying the composition of the Co-Cr-Mo alloy with additions of nickel and some trace elements aluminium, titanium and boron, have been investigated. A great improvement in the fatigue crack growth resistance of the cast alloy is obtained by nickel additions to the base alloy, mainly because of a significant increase in the alloy's stacking fault energy. In addition, the fatigue fracture ductility is observed to be improved strikingly with the nickel additions. Much smaller facets and better ductility with a mixed fatigue crack propagation mode, as compared to the base alloy, are observed in the alloys with low nickel content level, and in the alloys with high nickel content level, localized ductile fatigue striations are observed. It is also indicated that minor additions of such elements as aluminium, titanium and boron can be used to improve further the fatigue crack growth resistance resulting from the elimination of some microstructural casting defects.
