The present invention involves method and apparatus for analyzing two measured signals that are modeled as containing primary and secondary portions. Coefficients relate the two signals according to a model defined in accordance with the present invention. In one embodiment, the present invention involves utilizing a transformation which evaluates a plurality of possible signal coefficients in order to find appropriate coefficients. Alternatively, the present invention involves using statistical functions or Fourier transform and windowing techniques to determine the coefficients relating to two measured signals. Use of this invention is described in particular detail with respect to blood oximetry measurements.

Signal processing apparatus.

A method and an apparatus to analyze two measured signals that are modeled as containing desired and undesired portions such as noise, FM and AM modulation. Coefficients relate the two signals according to a model defined in accordance with the present invention. In one embodiment, a transformation is used to evaluate a ratio of the two measured signals in order to find appropriate coefficients. The measured signals are then fed into a signal scrubber which uses the coefficients to remove the unwanted portions. The signal scrubbing is performed in either the time domain or in the frequency domain. The method and apparatus are particularly advantageous to blood oximetry and pulserate measurements. In another embodiment, an estimate of the pulserate is obtained by applying a set of rules to a spectral transform of the scrubbed signal. In another embodiment, an estimate of the pulserate is obtained by transforming the scrubbed signal from a first spectral domain into a second spectral domain. The pulserate is found by identifying the largest spectral peak in the second spectral domain.

Arterial stiffening is the principal cause of increasing systolic pressure with advancing years and in patients with arterial hypertension. It is associated with progressive arterial dilation and is due to degeneration of the arterial wall, probably as a consequence of repetitive cyclic stress; it increases systolic pressure directly by increasing amplitude of the pressure wave generated by a given flow impulse from the heart and indirectly by increasing wave velocity so that wave reflection from the periphery occurs earlier, augmenting pressure in late systole. The first mechanism affects pressure in both the central and peripheral arteries, the second predominantly in the central arteries. Change in brachial systolic pressure with age underestimates the rise in systolic pressure in the aorta and left ventricle. Arterial stiffness is reduced passively with reduction in arterial pressure. Drugs have little or no direct effect on arterial stiffness but can markedly reduce wave reflection. In patients with stiffened arteries, reduction in wave reflection decreases aortic systolic pressure augmentation. The decreased systolic pressure in central arteries brought about by this mechanism is not detected when systolic pressure is measured in a peripheral (brachial or radial) artery but can be inferred from change in contour of the pressure wave recorded in peripheral arteries.

Arterial stiffness, systolic blood pressure, and logical treatment of arterial hypertension.

A noninvasive technique has been developed and validated for calculating capacitive and oscillatory systemic arterial compliance with the use of pulse wave analysis and a modified Windkessel model. Application of the technique to subjects with hypertension, postmenopausal women with symptomatic coronary artery disease, and appropriate control subjects has confirmed a reduction of oscillatory compliance in the disease states and an increase in capacitive and oscillatory compliances in response to vasodilator drugs. This method should be useful in screening subjects for early evidence of vascular disease and in monitoring the response to therapy.

Noninvasive Pulse Wave Analysis for the Early Detection of Vascular Disease

Measurement of functional residual capacity (FRC) by the open-circuit multiple breath tracer gas washout technique is an established method. A system based upon washout of sulfur hexafluoride (SF6) during mechanical ventilation is described. The central unit in the system is a sensitive and rapid-response infrared SF6 analyzer. SF6 is washed in until the alveolar concentration of SF6 is 0.5%, a concentration so low that the supply of other gases is hardly influenced. During washout, the flow of SF6 from the lungs is calculated by a computer every 10 ms from signals representing expiratory flow and SF6 concentration. The total volume of SF6, washed out, is calculated by integration of SF6 flow. Since the alveolar concentration at the end of washin is known, the lung volume may be obtained. The measurement procedure is highly automated and the result is presented by the computer immediately after washout. Accurate and reproducible results in model lung tests were obtained during air and N2O/O2 ventilation. Comparison with body plethysmography (FRCBOX) in eight sitting healthy subjects gave the following: FRCSF6 = 7 ml + 0.98 X FRCBOX, r = 0.99. Comparison with nitrogen washout (FRCN2) in five postoperative patients gave the following: FRCSF6 = 59 ml + 0.97 X FRCN2, r = 0.97. FRCSF6 during N2O/O2 ventilation was the same as during air/O2 ventilation in a group of paralyzed patients. The measurement system has not been tested in patients with obstructive lung disease.

Measurement of Functional Residual Capacity by Sulfur Hexafluoride Washout

Pulsatile changes in the volume of an arm segment were recorded with an air-filled plethysmograph and related to intra-arterial blood pressure. Alterations in transmural arterial pressure were obtained by changing the pressure in a large chamber surrounding the entire arm. Arterial compliance values were calculated in late diastole when pressure and volume changes were slow. Compliance varied with transmural arterial pressure in a hyperbolic manner, rising steeply at low pressure.

Noradrenaline and hypertensin did not change the arterial compliance values, while dihydroergotamine reduced them. Nitroglycerine caused a pronounced increase in compliance values in doses that did not change cardiac output and arterial pressure.

Vasoactive drugs and elastic properties of human arteries in vivo, with special reference to the action of nitroglycerine

Great investments have been made in computerized ECG interpretation but little attention has been paid to rationalization of such costly activities as recording, labelling and mounting ECGs, and still less to the presentation of ECGs. A system for such tasks is described. It includes a PDP8/E computer and modified ink jet ECG recorders, which write alphanumeric text and function as analogue input- and output-devices for the computer. On-line computer service is given independently to two laboratories for ECG at rest, VCG or exercise ECG. After averaging the ECG signal, the computer writes out a properly labelled ECG in an easily surveyable form. The visual interpretation of the ECG is entered and is then written on a new sheet together with the averaged 12-lead ECG and a rhythm strip. In exercise testing, the computer records and processes the ECG at predetermined intervals. The processing includes an automatic ST classification. A summarized presentationof the ECG before, during and after exercise allows easy visual identification of ECG changes. Experience of the system has mainly been obtained from 2 years of routine exercise ECG recording and to a limited extent from ECG and VCG recording at rest.

A system for computer-assisted ECG recording at rest and exercise.

Electrical noise often hampers the interpretation of ECG and other heart-synchronous signals. Time-coherent averaging is a commonly used method to reduce the noise. This method has been implemented on a computer with 12,000 12-bit words of core memory, suitable for on-line use in any clinical laboratory. An analysis was made of which demands apply to the exactness of synchronization of added heart cycles. The method was tested with respect to these demands and also, on a limited scale, for its efficiency in eliminating ectopic beats from the average.
