Cytogenetic, fluorescence in situ hybridization, and immunohistochemistry studies in a malignant pleural solitary fibrous tumor.

Primary renal botryoid rhabdomyosarcoma: diagnosis and outcome

PURPOSE To evaluate the clinicopathological characteristics and prognosis of localized resectable genitourinary sarcomas in adults. MATERIALS AND METHODS Between September, 1996 and November, 2008, 18 consecutive cases of adults (12 men and 6 women; median age 48.8 years) who were treated for primary genitourinary sarcomas were identified. The following variables were analyzed: patient age, gender, body mass index, American Society of Anesthesiologists (ASA) score, primary organ, tumor histology, size, necrosis, Federation Nationale des Centres de Lutte Contre le Cancer (FNCLCC) grade, and surgical margin positivity. Recurrence- free survival and disease-specific survival were the study end-points. RESULTS The most common presenting symptom was a palpable mass (six cases, 33.3%), the most common site was the kidney (six cases, 33.3%), and the most common histological subtype was leiomyosarcoma (eight patients, 44.4%). Complete resection with negative surgical margins was achieved in 13 patients (72.2%). The median follow-up period was 49.9 months (range 6.4 to 147.6). The recurrence- free survival rates at 1, 3, and 5 years were 81.6%, 66.5%, and 66.5%, respectively. Recurrence-free survival only associated significantly with ASA score (p=0.018). The disease-specific survival rate at 1, 3, and 5 years was 88.9%, 76.2%, and 67.7%, respectively. Disease-specific survival was associated significantly only with FNCLCC grade (p=0.042). CONCLUSION Although genitourinary sarcomas in adults are a rare group of tumors with a poor prognosis, some patients may have a favorable prognosis. Our findings suggest that FNCLCC grade is the most important prognostic factor for these patients.

Localized Resectable Genitourinary Sarcoma in Adult Korean Patients: Experiences at a Single Center

Renal rhabdomyosarcoma is a rare malignant mesenchymal tumor with an aggressive clinical course. We describe the case of a 39-year-old woman with a large rhabdomyosarcoma of the left kidney serendipitously discovered ultrasonographically subsequent to cesarean section. To our knowledge, this is the first reported case of renal rhabdomyosarcoma during pregnancy. The importance of ultrasound examination in the differential diagnosis of gestational flank pain is discussed.

Puerperal renal rhabdomyosarcoma: case report and review of the literature.

A case of botryoid-type embryonal rhabdomyosarcoma of the renal pelvis in a 49-year-old woman is reported. The tumor led to hydronephrosis. The surgical resection specimen disclosed a translucent, polypoid mass attached to the wall of the renal pelvis by thin stalk. Light-microscopic examination revealed a large exophytic polypoid tumor with intact surface epithelium, which was negative for dysplasia or carcinoma in situ. There was a condensation of epithelioid to spindle cells underneath the basement membrane, forming a cambium layer. The core of the lesion contained interspersed epithelioid to spindle cells with myxoid change and edema. Cells of the cambium layer as well as interspersed cells in the core exhibited marked cytologic atypia with mitotic figures. Immunohistochemical stains for cytokeratin, S-100 and myoglobin were negative, stains for desmin and actin were positive. Although botryoid-type embryonal rhabdomyosarcomas have been reported to occur at various sites in the genital tract and lower urinary tract, to our knowledge, this is the first reported case of the tumor within the renal pelvis. Also, the occurrence of these tumors in adults is quite rare.

Two cases of solitary fibrous tumor of the pleura with features of malignancy are described. In the first case, the tumor macroscopically showed noncircumscribed growth. Microscopically, on low power examination, the tumor was characteristically "patternless", with alternation of cellular areas and hypocellular, prominently collagenized areas. There was an infiltrative growth present at the margins. Cytological atypias were not present. In the second case, the tumor was macroscopically circumscribed. Microscopically, on low power examination, the tumor had characteristical "patternless" appearance again. Pleomorphic cells with high mitotic activity dominated in cellular areas on high power examination. The infiltrative pattern of growth was not present at the margins. Both tumors were classified as malignant solitary fibrous tumors of the pleura, or fibrosarcomas of the pleura. The criteria of malignancy for solitary fibrous tumor are discussed.

[Solitary fibrous tumor of the pleura--description of two cases with features of malignancy].

Salivary gland carcinomas (SGC) are extremely rare malignancies with only limited treatment options for the metastatic phase of the disease. Treatment with anti-CD47 antibodies could represent a potent therapy for SGCs by promoting the phagocytic clearance of tumor cells through various mechanisms. However, the efficacy of anti-CD47 therapy is largely dependent on the expression of CD47 within the tumor microenvironment (TME).In 43 patients with SGC, we were the first to investigate the CD47 expression in both tumor cells and tumor-infiltrating immune cells (TIIC) in the center and periphery of primary tumors. We also correlated the data with the clinicopathological variables of the patients and offered novel insights into the potential effectiveness of anti-CD47 therapy in SGCs.We observed that the CD47+ tumor cells are outnumbered by CD47+ TIICs in mucoepidermoid carcinoma. In the tumor center, the proportion of CD47+ tumor cells was comparable to the proportion of CD47+ TIICs in most histological subtypes. In low-grade tumors, significantly higher expression of CD47 was observed in TIICs in the periphery of the tumor as compared to the center of the tumor.The reason for a high expression of 'don't eat me' signals in TIICs in the tumor periphery is unclear. However, we hypothesize that in the tumor periphery, upregulation of CD47 in TIICs could be a mechanism to protect newly recruited leukocytes from macrophage-mediated phagocytosis, while also allowing the removal of old or exhausted leukocytes in the tumor center.
