TL;DR: Large blinded observational studies are warranted to identify CT characteristics with high accuracy for diagnosis of acute coronary syndromes to determine whether the detection of coronary stenosis and plaque with multidetector CT improves triage and reduces the costs or increases the cost-effectiveness of management of acute chest pain.

TL;DR: A subjective assessment of the pretest probability of acute coronary syndrome by the emergency department caregiver is used because no well established, universally accepted, and validated risk score exists that can be applied to undifferentiated ED populations with chest pain.
