To secure a first body tissue with a second body tissue, a first anchor is moved along a first path through the first body tissue into the second body tissue. A second anchor is moved along a second path through the first body tissue into the second body tissue. A suture extending between the anchors may be tightened by moving the second anchor along a path which extends transverse to the path of the first anchor. The suture which extends between the anchors may have free ends which are connected with a suture retainer. The free ends of the suture may be interconnected either before or after the anchors are moved along the first and second paths. Alternatively, the suture may be a continuous loop which extends between the two anchors. A guide assembly may be provided to guide movement of the anchors along the two paths. The paths along which the anchors move may intersect so that the anchors may be interconnected at the intersection between the two paths.

Transobturator Sling Suspension for Male Urinary Incontinence Including Post-Radical Prostatectomy

PURPOSE: To use first-pass perfusion computed tomography (CT) to prospectively investigate tumor vascularity in rectal cancer and to determine whether any of the perfusion parameters would predict tumor response to chemotherapy and radiation therapy. MATERIALS AND METHODS: The institutional review board approved this study, and informed prior consent was obtained from participants. Perfusion CT of rectal cancer was performed with four-section multi–detector row CT in 15 patients (13 men, two women; mean age, 62.1 years; age range, 46–84 years). Five patients with prostate cancer served as controls. All patients with rectal cancer underwent 6–8 weeks of chemotherapy and radiation therapy followed by surgery. In nine patients, perfusion CT was repeated after completion of chemotherapy and radiation therapy. Summary A review of the currently published data indicates thattestosterone replacement therapy in older men may beadvantageous in terms of improving bone mineral density,increasing muscle mass and strength, and, in some men,improving libido and mood. However, the long-term clin-ical signiﬁcance of these effects is still uncertain, as largerand longer-term studies are needed. In the short term (upto 3 years), the adverse effects of testosterone replace-ment therapy in older men seem predictable and manage-able, but the longer-term effects on target organs, such asthe cardiovascular system and the prostate, are yet to bedetermined.
