Diaphragm ultrasound as indicator of respiratory effort in critically ill patients undergoing assisted mechanical ventilation: a pilot clinical study
Michele Umbrello,Paolo Formenti,Daniela Longhi,Andrea Galimberti,IR Piva,A. Pezzi,Giovanni Mistraletti,John J. Marini,Gaetano Iapichino +8 more
TL;DR: In patients undergoing in assisted mechanical ventilation, diaphragm thickening is a reliable indicator of respiratory effort, whereas diaphagm excursion should not be used to quantitatively assess diaphragem contractile activity.
Journal ArticleDOI
Physiological and quantitative CT-scan characterization of COVID-19 and typical ARDS: a matched cohort study.
Davide Chiumello,Mattia Busana,Silvia Coppola,Federica Romitti,Paolo Formenti,Matteo Bonifazi,Tommaso Pozzi,Maria Michela Palumbo,Massimo Cressoni,Peter Herrmann,Konrad Meissner,Michael Quintel,Luigi Camporota,John J. Marini,Luciano Gattinoni +14 more
TL;DR: COVID-19-ARDS is a subset of ARDS characterized overall by higher compliance and lung gas volume for a given PaO2/FiO2, at least when considered within the timeframe of this study.
Journal ArticleDOI
Current Concepts of ARDS: A Narrative Review
TL;DR: How a holistic framework of respiratory and hemodynamic support should be provided to patients with ARDS is underline, aiming to ensure adequate gas exchange by promoting lung recruitment while minimizing the risk of ventilator-induced lung injury.
Journal Article
Melatonin reduces the need for sedation in ICU patients: a randomized controlled trial.
Giovanni Mistraletti,Michele Umbrello,Giovanni Sabbatini,S. Miori,M. Taverna,B. Cerri,E. Mantovani,Paolo Formenti,P. Spanu,Armando D'Agostino,Silvia Salini,Alberto Morabito,Franco Fraschini,Russel J. Reiter,G. Iapichino +14 more
TL;DR: Long-term enteral melatonin supplementation may result in a decreased need for sedation, with improved neurological indicators and cost reduction, in patients with critical illness.
Journal ArticleDOI
Tolvaptan for hyponatremia with preserved sodium pool in critically ill patients
Michele Umbrello,E. Mantovani,Paolo Formenti,Claudia Casiraghi,Davide Ottolina,M. Taverna,A. Pezzi,Giovanni Mistraletti,Gaetano Iapichino +8 more
TL;DR: Enteral administration of Tolvaptan seems effective in the treatment of hyponatremia with preserved sodium pool in critically ill patients, and even if the study was underpowered to detect significant side effects or complications of unwarranted fast corrections of hyponsetremia, it is reported no complications.
